Kamu özel işbirliği modelinin sağlık kamu hizmetine etkisi

Abstract

Sağlık hakkı uluslararası sözleşmeler ve Anayasa tarafından koruma altına alınmış, vazgeçilemez, ertelenemez ve ikame edilemez temel bir insan hakkıdır. Sağlık hakkının korunması ve günün şartlarına uygun şekilde geliştirilmesi devletin sorumluluğundadır. Hiç şüphesiz kamusal sağlık hizmetleri devlete verilen sorumluluk kapsamında sağlık hakkının geliştirilmesine yönelik sunulan en temel kamu hizmetidir. Sağlık hizmetleri toplumsal ihtiyaç ve beklentiler çerçevesinde şekillenmekte ve hızla değişmektedir. Devlet, yüksek teknoloji gerektiren sağlık hizmetinde mevcut finansal kaynaklar ile değişim hızına yetişememektedir. Bu problemin çözüm yolu olarak sağlık hizmetlerinin finansmanında, hizmetin tüm paydaşlarının katılımının sağlandığı iktisadi bir model olarak kamu-özel işbirlikleri gelişmiştir. Ülkemiz sağlık hizmetlerinde kamu ve özel sektör iş birliği uzun bir süredir uygulanmasına rağmen şehir hastaneleri modeli sonrası önemli değişimler yaşanmıştır. Son otuz yıldır idare kendisine yüklenen hizmet sunum ve yönetim sorumluluklarını liberal politikalar çerçevesinde özel sektör ile paylaşmayı tercih etmektedir. Kamu-özel işbirliği ile finanse edilen şehir hastanelerinde de sağlık hizmeti sunumu parçalanmış ve parçalanan hizmetler bütüncül halde özel sektör ortağının sunumuna bırakılmıştır. Gelecekte sağlık kamu hizmetlerinde idarenin planlama ve denetleme sorumluluklarının da parçalanarak özel sektöre devredilmesi sonucu ile karşılaşılabilecektir. Hastaneleri

The right to health is a fundamental human right that is protected by international conventions and the Constitution and cannot be given up, postponed or substituted. It is the responsibility of the state to protect the right to health and to develop it in accordance with the conditions of the day. Undoubtedly, public health services are the most basic public service provided for the development of the right to health within the scope of the responsibility given to the state. Health services are shaped within the framework of social needs and expectations and are changing rapidly. The state cannot keep up with the speed of change with the financial resources available in the health public service, which requires high technology. As a solution to this problem, public-private partnerships have developed as an economic model in which all stakeholders of the service are involved in the financing of health services. Although public and private sector cooperation has been implemented for a long time in our country's health services, significant changes have occurred after the city hospitals model. For the last thirty years, the administration has preferred to share its health service delivery and management responsibilities with the private sector within the framework of liberal policies. In city hospitals financed by public-private partnership, health service provision was also fragmented and the fragmented services were left to the private sector partner in a holistic manner. In the future, the planning and supervision responsibilities of the administration in health public services may be fragmented and transferred to the private sector.
