Acil serviste eritrosit süspansiyonu transfüzyonu uygulanan hastaların transfüzyon sonrası erken dönemdeki serum potasyum düzeylerindeki değişimin, torba kanın bekleme süresiyle ilişkisinin araştırılması

AMAÇ: Bu çalışmada, eritrosit süspansiyonu transfüzyonu uygulandıktan sonraki erken dönemde hastaların serum potasyum düzeyleri ve diğer kan gazı parametrelerindeki değişimin, torba kanın bekleme süresiyle ilişkisinin araştırılması amaçlanmıştır. YÖNTEM: Bu retrospektif çalışma, İstanbul Medeniyet Üniversitesi Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesi Acil Tıp Kliniği'ne başvuran ve semptomatik anemi ya da aktif kanama nedeniyle eritrosit süspansiyonu transfüzyonu yapılan hastalardan, çalışmamıza dahil edilme kriterlerine uygun olan 23 hastaya ait hastane otomasyon sistemindeki kayıtların incelenmesiyle gerçekleştirildi. Hastaların 1 ünite ışınlanmamış ES transfüzyonundan önce ve sonrasındaki 15-30. dakikada alınan kan gazı tetkiklerindeki serum potasyum düzeyleri, pH değerleri, kan laktat konsantrasyonları, hemoglobin ve hematokrit düzeyleri incelendi. Hastalar, 14 günden daha az süreli kan transfüze edilenler ile 14 gün ve daha uzun süre süreli kan transfüze edilenler olarak iki gruba ayrılarak, potasyum düzeyleri ve diğer kan gazı parametrelerindeki değişimler değerlendirildi. BULGULAR: Çalışma %43,5'i erkek, %56,5'i kadın olmak üzere toplam 23 hastada yapılmış ve hastaların yaşları 23 ile 87 arasında, yaş medyan değeri 75 olarak saptanmıştır. Tüm hastalara baktığımızda, transfüzyon öncesi ve sonrası potasyum, laktat, Hb, Hct düzeylerinde anlamlı fark görülmüştür. 14 günden daha az süreli kan transfüze edilen 14 hastada transfüzyon öncesi kan potasyum medyan değeri 3,9 (3,4-5,3) mmol/L, transfüzyon sonrası kan potasyum medyan değeri 4,05 (3,4-5,5) mmol/L idi ve iki medyan değeri arasında istatistiksel açıdan anlamlı bir fark bulunmamıştır (p=0,063). Hastaların transfüzyon öncesi kan laktat medyan değeri 2,3 (1-7,1) mmol/L, transfüzyon sonrası kan laktat medyan değeri 1,25 (0,6-6,1)mmol/L idi ve iki medyan değeri arasında istatistiksel açıdan anlamlı bir fark görülmüştür (p<0,001). 14 gün ve daha fazla süreli torba kanın transfüzyonu yapılan 9 hastanın transfüzyon öncesi kan potasyum medyan değeri 3,8 (3,2-4,5) mmol/L, transfüzyon sonrası kan potasyum medyan değeri 3,9 (3,4-5,5) mmol/L idi ve iki medyan değeri arasında istatistiksel açıdan anlamlı fark olduğu saptanmıştır (p=0,049). Hastaların transfüzyon öncesi kan laktat medyan değeri 1,5 (0,8-4) mmol/L, transfüzyon sonrası kan laktat medyan değeri 1,7 (0,7-2,7) mmol/L'di ve iki medyan değeri arasında istatistiksel açıdan anlamlı bir fark bulunmamıştır (p=0,110). Tüm hastaların transfüzyon öncesi ve sonrası Hb ve Hct düzeylerinde anlamlı değişim saptanmıştır. SONUÇ: Klinik olarak bulgu vermesi beklenmeyen düzeylerde de olsa, 14 günden uzun süre depolanmış kan transfüze edilen grupta, transfüzyon öncesi ve sonrası potasyum değerlerinde istatistiksel olarak anlamlı artış saptanmıştır. Bu yüzden potasyumu tolere edemeyen hastalarda 14 günden taze kan kuıllanılması önerilir. Laktat düzeylerine baktığımızda, 14 günden daha kısa süre depolanmış kan trasnfüzyonu yapılmış olan grupta, transfüzyon öncesi ve sonrası arasında anlamlı fark bulunmuştur. Taze kan kullanmanın çok kısa sürede dokulara oksijen sunumunu artırmada ve hipoksinin etkilerini geri döndürmede etkili olduğu gösterilmiştir. Hastaları yaş ve cinsiyet açısından değerlendirdiğimizde, tüm hastalarda, <14 gün depolanmış kan transfüze edilen ve ?14 Gün depolanmış kan transfüze edilen grupta klinik parametreler arasında anlamlı fark saptanmadı.
OBJECTIVES: In this study, our aim was investigating the relationship between the change in serum potassium levels of patients in the early period after transfusion and the storage time of packed blood. MATERIALS AND METHODS: This retrospective study was carried out by examining the records in the hospital automation system of 23 patients who met the inclusion criteria of our study, from patients who applied to the Emergency Department of Istanbul Medeniyet University Göztepe Prof. Dr. Süleyman Yalçın City Hospital and who underwent transfusion due to symptomatic anemia or active bleeding. Serum potassium levels, pH, lactate concentrations, hemoglobin and hematocrit levels in blood gas tests of the patients taken before and 15 to 30 minutes after 1 unit of non-irradiated RBC transfusion were examined. The patients were divided into two groups as those who were transfused RBCs stored less than 14 days and those who were transfused RBCs stored for 14 days or longer. Changes in potassium levels and other blood gas parameters were evaluated and compared between these 2 groups. RESULTS: The study was conducted in 23 patients, 43.5% male and 56.5% female, and the age of the patients was between 23 and 87, and the median age was 75. When we evaluate all patients, a significant difference was observed between pre-transfusion and post-transfusion potassium, lactate, Hb, Hct levels. In 14 patients who were transfused with RBCs stored less than 14 days, the pre-transfusion blood potassium median value was 3.9 (3.4-5.3) mmol/L, and the post-transfusion blood potassium median value was 4.05 (3.4-5.5) mmol/L and there was no significant difference between the two median values (p=0.063). The median blood lactate value of the patients before transfusion was 2.3 (1-7.1) mmol/L, and the median blood lactate value after transfusion was 1.25 (0.6-6.1) mmol/L, and a significant statistical difference between the two median values was observed (p<0.001). In 9 patients who were transfused with RBCs packed for 14 days or more, the median blood potassium value before transfusion was 3.8 (3.2-4.5) mmol/L, and the median blood potassium value after transfusion was 3.9 (3.4-5, 5) mmol/L, and a statistically significant difference was found between the two median values (p=0.049). The median blood lactate value of the patients before transfusion was 1.5 (0.8-4) mmol/L, and the median blood lactate value after transfusion was 1.7 (0.7-2.7) mmol/L, and there was no statistical difference between the two median values. no significant difference was found in terms of (p=0.110). Significant changes were found in Hb and Hct levels before and after transfusion in all patients. CONCLUSION: A statistically significant increase was found in the pre- and post-transfusion potassium values in the group transfused with blood stored for more than 14 days, albeit at levels that are not expected to cause clinical manifestations. Therefore, it is recommended to use blood products stored for less than 14 days in patients who would not be able to tolerate potassium load. When we look at the lactate levels, a significant difference was found between before and after transfusion in the group that had transfusions with blood stored for less than 14 days. Using fresh blood has been shown to be effective in increasing oxygen delivery to tissues and reversing the effects of hypoxia in a very short time. When we evaluated the patients in terms of age and gender, there was no significant difference in clinical parameters in all patients, in the group with <14 days of stored blood transfused and ?14 days of stored blood transfused.
