Çocukluk çağındaki akut lösemi tanısı ile takipli hastaların tedavi öncesi ve sonrası viral seroloji ve bağışıklık durumlarının değerlendirilmesi

Özet

Giriş: Akut lösemi çocukluk çağının en sık görülen kanserlerinden biridir. Hem hastalığın kendisi hem uygulanan yoğun kemoterapi kemik iliği hücrelerinde özellikle granülosit ve lenfosit sayılarında belirgin süpresyona neden olarak immün sistemin zayıflamasına ve enfeksiyonlara karşı açık hale gelmesine yol açar. Tedavinin tamamlanmasından sonra bile aşılama ile önceden kazanılmış koruyucu serum antikor konsantrasyonlarının kaybolduğunun veya spesifik antikor konsantrasyonlarının azaldığının belirlenmiş olması, kemoterapisi tamamlanan çocukların tekrar aşılanmasının gerekli olduğunu göstermektedir. Bu nedenle hastaların tanı aldıkları ve tedavileri tamamlandıktan sonraki dönemlerde antikor düzeylerinin değerlendirilmesi önemlidir. Amaç: Bu çalışmada akut lösemili çocuklarda tanı anı ve kemoterapi sonrasındaki hemogram ve immünoglobulin değerleri ile aşı yanıtları ve diğer enfeksiyöz etkenlerin serolojisi değerlendirilerek immün sistemin seyrini değerlendirmeyi amaçladık. Gereç-Yöntem: Bu çalışmada 2012-2021 tarihleri arasında hastanemizin çocuk hematoloji-onkoloji servisinde akut lösemi tanısıyla takip ve tedavi edilen 162 hastanın dosyaları geriye dönük olarak değerlendirildi. Hastaların yaş, cinsiyet, tanı, down sendromu ile birlikteliği; tanı anında ve kemoterapi sonrası hemogram (lökosit-nötrofil-lenfosit sayıları, hemoglobin ve trombosit değerleri) ve immünoglobulin değerleri ile aşıyla önlenebilen hastalıkların (Kızamık-Kızamıkçık-Kabakulak-Su çiçeği-Hepatit A-Hepatit B ve Tetanoz) antikorları ve diğer enfeksiyöz etkenlerin (sitomegalovirüs-epsteinbarr virüs-parvovirüs-toxoplazma gondii-hepatit c virüsü-human immunodeficiency virüsü-herpes simplex 1/2 virüs)serolojileri değerlendirildi. Bulgular: Çalışmaya katılan hastaların 77 (%47,5)'si kız, 85 (%52,5)'i erkekti. Tanı yaşları 2 ay ile 215 ay arasında olup ortalama yaş 77,4 ay (54,1) olarak bulundu. Hastalarımızın 143 (%88,3)'ü ALL tanılı, 19 (%11,7)'u AML tanılı idi. ALL tanılı 143 hastanın 127 (%78.4)'si B-ALL iken 16 (%9.88)'i T-ALL idi. ALL-BFM protokolüne göre yapılan risk grubu sınıflamasına göre 58 (%40.6)'i SRG, 65 (%45.5)'i MRG ve 20 (%14)'si HRG saptandı. Hastalarımızın 8 (%4,94)'inde down sendromu (DS) birlikteliği vardı. DS'lu hastaların 3 (%37,5)'ü ALL tanılı iken 5 (%62,5)'i AML tanılı idi. İlk tanıda Hepatit A, Hepatit B ve kızamıkçık antikor pozitifliği saptanan hastaların tedavi sonrasında koruyucu antikor titreleri değerlendirildi. Koruyucu antikor kayıpları kız-erkek, 2 yaş altı ve üstü, ALL-AML ve risk gruplarına göre karşılaştırılmıştır. Hepatit A için koruyucu antikor kaybının 2 yaş üstü hastalarda daha fazla olduğu %86,2 oranla 2 yaş altındaki hastalara göre anlamlı fark görülmüştür (p<0.05). Sonuç: Çocukluk çağı lösemilerinde sağkalım artmakta olmasına rağmen hastalığın kendisi, uygulanan kemoterapi veya her ikisi birden B lenfositlerini, T lenfositlerini (CD4+ ve CD8+), NK ve plazma hücrelerini etkileyerek immunsupresyona neden olmaktadır. Sadece hücre sayıları ve immünoglobulin seviyeleri değerlendirilirse, fonksiyonel bozukluklar gözden kaçabilir. Bu nedenle hümoral fonksiyonların gösterilmesi amacıyla aşı yanıtlarının ve lenfosit fonksiyonlarının değerlendirilmesi önemlidir.

Introduction: Acute leukemia is one of the most common cancers of childhood. Both the disease itself and the intense chemotherapy applied cause significant suppression in bone marrow cells, especially in the granulocyte and lymphocyte numbers, resulting in weakening of the immune system and making it vulnerable to infections. The fact that the previously acquired protective serum antibody concentrations were lost or the specific antibody concentrations decreased with vaccination even after completion of treatment indicates that it is necessary to revaccinate children whose chemotherapy has been completed. For this reason, it is important to evaluate the antibody levels of the patients in the periods when they are diagnosed and after their treatment is completed. Aim: In this study, we aimed to evaluate the course of the immune system in children with acute leukemia by evaluating hemogram and immunoglobulin values at the time of diagnosis and after chemotherapy, vaccine responses and serology of other infectious agents. Material and Methods: In this study, the files of 162 patients who were followed up and treated with the diagnosis of acute leukemia in the pediatric hematology-oncology service of our hospital between 2012-2021 were evaluated retrospectively. Age, gender, diagnosis, association with down syndrome of the patients; At the time of diagnosis and after chemotherapy, hemogram (leukocyte-neutrophil-lymphocyte counts, hemoglobin and thrombocyte values) and immunoglobulin values and antibodies of vaccine-preventable diseases (Measles-Rubella-Mumps-Variculum-Hepatitis A-Hepatitis B and Tetanus) and other infectious agents (cytomegalovirus -epsteinbarr virus-parvovirus-toxoplasma gondii-hepatitis c virus-human immunodeficiency virus-herpes simplex 1/2 virus) serologies were evaluated Findings: Of the patients participating in the study, 77 (47.5%) were female and 85 (52.5%) were male. Age at diagnosis was between 2 months and 215 months, with a mean age of 77.4 months (54.1). 143 (88.3%) of our patients were diagnosed with ALL and 19 (11.7%) were diagnosed with AML. While 127 (78.4%) of 143 patients diagnosed with ALL were B-ALL, 16 (9.88%) were T-ALL. According to the risk group classification made according to the ALL-BFM protocol, 58 (40.6%) SRG, 65 (45.5%) MRI and 20 (14%) HRG were detected. Eight of our patients (4.94%) had Down syndrome (DS). While 3 (37.5%) patients with DS were diagnosed with ALL, 5 (62.5%) patients were diagnosed with AML. The protective antibody titers of the patients who were found to be positive for Hepatitis A, Hepatitis B and rubella antibodies at the initial diagnosis were evaluated after treatment. Protective antibody losses were compared according to female and male, under 2 years old and over, ALL-AML and risk groups. A significant difference was observed in patients younger than 2 years of age, 86.2%, where the loss of protective antibodies for hepatitis A was higher in patients older than 2 years of age (p<0.05). Results: Although survival is increasing in childhood leukemia, the disease itself, chemotherapy or both cause immunosuppression by affecting B lymphocytes, T lymphocytes (CD4+ and CD8+), NK and plasma cells. Functional disorders may be overlooked if only cell counts and immunoglobulin levels are evaluated. Therefore, it is important to evaluate vaccine responses and lymphocyte functions in order to demonstrate humoral function.
