Özet

Bu çalışmanın amacı, sezaryen operasyonu sonrası Pfannenstiel insizyon hattına buz paketi uygulamasının ameliyat sonrası hastaların hissettiği ağrı skorları ve opioid gereksinimi üzerine etkisini araştırmaktır. Kör olmayan, prospektif randomize olan bu çalışmaya İstanbul Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesi Kadın Hastalıkları ve Doğum Kliniği'nde Nisan 2023- Haziran 2023 tarihleri arasında sezaryen ile doğum yapan 60 hasta dahil edildi. Bilgisayar temelli bir program ile katılımcılar iki kola randomize edildi. Grup I: Sezaryen yapılan ve postoperatif dönemde buz paketi uygulanan hastalar. Grup II: Sezaryen yapılan ve postoperatif dönemde buz paketi uygulanmayan hastalar (kontrol grubu). Bilgisayarlı randomizasyon sonucu karar verilen buz paketi uygulanacak hastalara önceden eşit boyutlarda ve dolulukta hazırlanan, içinde buz olan ağzı kapalı buz poşetleri postoperatif 24 saat boyunca buz paketleri içine yeni buzlar konarak buz uygulaması devam edildi. Görsel ağrı skalası (VAS) kullanılarak sezaryen ameliyatı öncesi preoperatif dönemde serviste yattıkları sırada, postoperatif 2. saatte, postoperatif 12.saatte ağrı skorları kaydedildi. Ayrıca postoperatif 24.saat, postoperatif 48.saat ve postoperatif 7.günde ise Cerrahi Hastalarda Kısa Ağrı Envanteri kullanılarak ağrı skorları hesaplandı. Postoperatif dönemde serviste opioid uygulanma durumu ve dozunun miligram cinsinden değeri, ilave nsaii uygulanma durumu, ilave parasetamol uygulanma durumu kaydedildi. Verinin istatistiksel analizi IBM SPSS 28.0 (IBM Corp. Released 2021. IBM SPSS Statistics for Windows, Version 28.0. Armonk, NY: IBM Corp.) istatistik paket programında yapılmıştır. Bu çalışmanın sonucunda ağrı skorlarına bakıldığında preoperatif, postoperatif 2. Saat (p=0,098) ve 2. saatte ağrı skorlarında değişim (p=0,17) açısından gruplar arasında anlamlı farklılık bulunmamıştır. Bunun yanında buz uygulanan grupta yer alanlarda postoperatif 12. saat VAS değeri kontrol grubundakilere göre daha düşük bulunurken (p<0,001); 12. saatteki ölçümlerin preoperatif ölçüme göre değişimine bakıldığında VAS xiii değerindeki artış kontrol grubunda anlamlı olarak daha fazla bulunmuştur (p<0,001). Postoperatif 24. saat kısa ağrı envanterine göre ağrı skorlarının gruplar arası karşılaştırılmasında buz uygulanan grubun ağrı skorlarının kontrol grubundakilere göre daha düşük olduğu bulunmuştur (p=0,002). Postoperatif 48. saat kısa ağrı envanterine göre ağrı skorlarının gruplar arası karşılaştırılmasında buz uygulanan grubun kontrol grubundakilere göre daha düşük ağrı skoruna sahip olduğu bulunurken (p=0,002); değişimler açsından incelendiğinde anlamlı farklılık bulunmamıştır (p=0,58). Postoperatif 7. gün kısa ağrı envanterine göre ağrı skorlarının gruplar arası karşılaştırılmasında buz uygulanan grubun kontrol grubundakilere göre daha düşük ağrı skoruna sahip olduğu bulunmuştur (p=0,003). İlave non-steroid antiinflamatuvar ilaç (NSAİİ) uygulanması açısından karşılaştırıldığında buz uygulanan grubun (%23,3) kontrol grubuna göre (%60) daha düşük oranda uygulandığı görülmüştür (p=0,004). Postoperatif serviste opioid kullanımı açısından gruplar karşılaştırıldığında istatistiksel olarak anlamlı farklılık bulunmamıştır (p=1). Çalışmamızın sonucunda buz paketi uygulanan grubun daha düşük ağrı skorları ve daha az ilave NSAİİ ihtiyacı ile ilişkili olduğu bulundu. Bu durumda sezaryen sonrası postoperatif dönemde insizyon hattına buz uygulaması, postoperatif ağrıyı ve ilave analjezik kullanımını azaltmak amacıyla önerilebilir.

The aim of this study was to investigate the effect of ice pack application to the Pfannenstiel incision line after cesarean section on postoperative pain scores and opioid requirement. This non-blinded, prospective randomized study included 60 patients who delivered by cesarean section between April 2023 and June 2023 at the Obstetrics and Gynecology Clinic of Istanbul Göztepe Prof. Dr. Süleyman Yalçın City Hospital. Participants were randomized into two arms by a computer-based program. Group I: Patients who underwent cesarean section and received ice packs in the postoperative period. Group II: Patients who underwent cesarean section and did not receive ice packs in the postoperative period (control group). Patients who were decided to receive ice packs as a result of computerized randomization were given sealed ice packs containing ice, which were prepared in equal sizes and fullness beforehand, and ice application was continued by placing new ice in the ice packs for 24 hours postoperatively. Using Visuel Analog Scale (VAS), pain scores were recorded in the preoperative period before cesarean section, while the patients were lying in the ward, at the 2nd postoperative hour, and at the 12th postoperative hour. In addition, pain scores were calculated using the Brief Pain Inventory in Surgical Patients at postoperative 24th hour, postoperative 48th hour and postoperative 7th day. In the postoperative period, the status of opioid administration in the ward and its dose in milligrams, the status of additional nsaii administration, and the status of additional paracetamol administration were recorded. Statistical analysis of the data was performed using IBM SPSS 28.0 (IBM Corp. Released 2021. IBM SPSS Statistics for Windows, Version 28.0. Armonk, NY: IBM Corp.) statistical package program. As a result of this study, there was no significant difference between the groups in terms of pain scores at preoperative, postoperative 2nd hour (p=0.098) and change in pain scores at 2nd hour (p=0.17). In addition, while the postoperative 12th hour VAS value was found to be lower in the ice-treated group than in the control group (p<0.001), the increase in VAS xv value was found to be significantly higher in the control group when the change in the 12th hour measurements compared to the preoperative measurement was analyzed (p<0.001). In the comparison of pain scores according to the brief pain inventory at postoperative 24th hour between the groups, it was found that the pain scores of the ice application group were lower than those of the control group (p=0.002). In the comparison of pain scores according to the brief pain inventory at postoperative 48th hour between the groups, it was found that the ice-applied group had lower pain scores than those in the control group (p=0.002); no significant difference was found when analyzed in terms of changes (p=0.58). In the comparison of pain scores according to the brief pain inventory on the 7th postoperative day between the groups, it was found that the ice group had a lower pain score than the control group (p=0.003). When compared in terms of additional non-steroidal anti-inflammatory drugs (NSAIDs) administration, it was observed that the ice-treated group (23.3%) had a lower rate of administration compared to the control group (60%) (p=0.004). When the groups were compared in terms of opioid use in the postoperative ward, no statistically significant difference was found (p=1). The results of our study showed that ice pack application was associated with lower pain scores and less need for additional NSAIDs. In this case, ice application to the incision line in the postoperative period after cesarean section may be recommended to reduce postoperative pain and additional analgesic use.
