Assitli karaciğer sirozu tanılı yatan hastaların önceki ve güncel hepatorenal sendrom tanı kriterlerine göre değerlendirilmesi ve klinik sonlanımları

dc.description.abstractHepatorenal sendrom (HRS) sirozun mortalitesi yüksek önemli bir komplikasyonudur. HRS tanı kriterleri yıllar içerisinde birçok kez değişikliğe uğramıştır. Çalışmamızda önceki ve güncel kriterlere göre HRS tanı alma oranları arasındaki farklılıkları, HRS prevalansını, alt tiplerini ve sonlanımlarını araştırdık. Retrospektif, gözlemsel, klinik çalışmaya gastroenteroloji kliniğine Ekim 2015- Mart 2023 tarihleri arasında yatan 18 yaş ve üzeri 212 assitli karaciğer sirozu hastası ardışık olarak dahil edildi. Hastaların demografik, komorbidite ve yatış öncesi tedavi özellikleri, hastalık süresi ve şiddeti, yatış süresi ve sayısı, siroz komplikasyonları, siroz etiyolojileri, laboratuvar verileri, yatıştaki tedavi özellikleri ve klinik sonlanımları (mortalite, yoğun bakım) kaydedildi. Olguların önceki ve güncel tanı kriterlerine göre HRS sıklıkları, HRS alt tip tanıları, akut böbrek hasarı (ABH) gelişen hasta sıklıkları belirlendi. HRS'si olan ve olmayan olguların klinik özellikleri karşılaştırıldı. International Ascites Club tarafından 2007'de yayınlanan kriterler önceki kriterler, 2015'te International Club of Ascites tarafından yayınlanan kriterler ise güncel kriterler olarak kabul edildi. Hastaların 68'inde (%32.1) yatış esnasında ABH geliştiği gözlendi [34'ü (%50) prerenal ABH, 16'sı (%23.5) intrensek böbrek hastalığına bağlı ABH, 18'i (%26.5) HRS-ABH]. Önceki kriterlere göre HRS tanısı alan hasta sayısı 16 (%7.5) bulunurken güncel kriterlere göre HRS tanısı alan hasta sayısı 1.69 kat artarak 27 (%12.7) bulundu. Önceki kriterlere göre HRS tanısı alan hastaların 7'si Tip-1 HRS, 9'u Tip-2 HRS grubuna girerken, güncel kriterlere göre HRS tanısı alan hastaların 18'inin HRS-ABH, 9'unun HRS-ABH olmayan gruba girdiği görüldü. Mortalite sıklığı güncel kriterlere göre HRS tanısı alanlarda %33.3 (9 hasta), önceki kriterlere göre HRS tanısı alanlarda %43.8 (7 hasta) idi. HRS'li olgularda diğer olgulara göre ABH nedenli yatış sıklığı (p=0.001), ortalama MELD 3.0 skoru (p=0.001), Child C hasta sıklığı (p=0.043), tekrar yatış sıklığı (p=0.011), assit öyküsü olan hasta sıklığı (p=0.001), yoğun bakım ihtiyacı gelişen hasta sıklığı (p=0.001)üre (p=0.001) ve kreatinin düzeyleri (bazal, yatış ve son değer) yüksek (tümü için p<0.01), glomeruler filtrasyon hızı düzeyleri (bazal, yatış ve son değer) düşüktü (tümü için p<0.01). Bu çalışmanın sonuçları hastaneye yatan assitli siroz hastalarının yaklaşık üçte birinde ABH ve %12.7'sinde HRS geliştiğini ortaya koymuştur. Güncel HRS kriterlerinin önceki kriterlere göre 1.69 kat fazla HRS tanısı koydurmuş olması bu hastalarda erken ve doğru tanı konulması açısından güncel kılavuz önerilerini doğrular nitelikte görünmektedir. Öte yandan bilinen assiti olan, siroz şiddeti yüksek, tekrar yatış sıklığı fazla olan olgularda, bazal renal fonksiyonların ve buna göre yatış anındaki ve takibindeki renal fonksiyon değişimlerinin dikkatli değerlendirmesinin hem HRS'nin erken tanısı hem de HRS nedenli yoğun bakım ihtiyacı ve mortalite üzerine olumlu etki yapacağı aşikardır.
dc.description.abstractHepatorenal syndrome (HRS) is an important complication of liver cirrhosis with high mortality and morbidity. The diagnostic criteria for HRS have been changed many times over the years. In our study, we investigated the prevalence, subtypes and clinical outcomes of HRS and whether there is a difference between the rates of HRS diagnosis according to previous and current criteria. In this retrospective, observational, clinical study, 212 patients with liver cirrhosis with ascites aged 18 years and older who were hospitalized in the gastroenterology clinic between October 2015 and March 2023 were consecutively included. Demographic characteristics, comorbidities, pre-hospitalization treatment characteristics, disease duration, disease severity, length of hospitalization, number of rehospitalizations, cirrhosis complications, cirrhosis etiologies, laboratory data, treatment characteristics during hospitalization and clinical outcomes (mortality, intensive care unit referral) were recorded. The frequency of HRS according to previous and current diagnostic criteria, HRS subtype diagnoses, and the frequency of patients with acute kidney injury (AKI) were determined. Clinical characteristics of patients with and without HRS were compared. The criteria published by the International Club of Ascites in 2007 were accepted as the previous criteria and the criteria published by the International Club of Ascites in 2015 were accepted as the current criteria. We observed that 68 (32.1%) patients developed AKI during hospitalization [34 (50%) prerenal AKI, 16 (23.5%) AKI due to intrinsic kidney disease, 18 (26.5%) HRS-AKI]. While the number of patients diagnosed with HRS according to the previous criteria was 16 (7.5%), the number of patients diagnosed with HRS according to the current criteria increased 1.69-fold to 27 (12.7%). While 7 of the patients diagnosed with HRS according to the previous criteria were classified as Type-1 HRS and 9 as Type-2 HRS, 18 of the patients diagnosed with HRS according to the current criteria were classified as HRS-AKI and 9 as HRS-NAKI. The mortality rate was 33.3% (9 patients) in patients diagnosed with HRS according to current criteria and 43.8% (7 patients) in patients diagnosed with HRS according to previous criteria. The frequency of hospitalization due to AKI (p=0.001), mean MELD 3.0 score (p=0.001), frequency of patients with Child C (p=0.043), frequency of rehospitalization (p=0.011), frequency of patients with a history of ascites (p=0. 001), frequency of patients requiring intensive care unit referral (p=0.001), urea (p=0.001) and creatinine levels (baseline, hospitalization and endpoint) were high (p<0.01 for all), glomerular filtration rate levels (baseline, hospitalization and endpoint) were low (p<0.01 for all). The results of this study revealed that approximately one third of hospitalized cirrhosis patients with ascites developed AKI and 12.7% developed HRS. The fact that the current HRS criteria resulted in a diagnosis of HRS 1.69 times higher than the previous criteria seems to confirm the current guideline recommendations in terms of early and accurate diagnosis in these patients. On the other hand, in patients with known ascites, high severity of cirrhosis and high frequency of hospitalization, careful evaluation of baseline renal function and changes in renal function at the time of admission and during follow-up will have a positive effect on both the early diagnosis of HRS and the need for intensive care unit referral and mortality due to HRS.
