Diyabeti olan hastaların psikososyal durumunun glisemik kontrol üzerine etkisi

Özet
Diyabet, tüm dünyada sıklığı giderek artan yaygın bir sağlık sorunudur ve tedavisi kişinin özbakımı, yakınlarının sosyal desteği ve psikososyal durumunda iyilik ile sürdürülebilir. Bu çalışmada hastanemiz diyabet obezite polikliniğine başvuran diyabet hastalarının özbakım, sosyal destek ve psikososyal durumunun hastaların altı aylık HbA1c ölçümlerindeki değişime etkisinin araştırılması amaçlanmıştır. Çalışmaya Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesi Diyabet Obezite polikliniğine başvuran 18 yaş ve üzeri, çalışmaya katılmayı kabul eden 130 diyabet hastası dahil edilmiştir. Çalışmaya dahil edilen hastalara olgu rapor formu, çok boyutlu diyabet anketi ve depresyon anksiyete stres ölçeği yanıtlandırılmıştır. Hastaların 6 ay önce, 3 ay önce ve güncel bakılan HbA1c ölçümleri hastane yazılımı ve hasta dosyalarından alınmıştır. Çalışmada elde edilen bulgular değerlendirilirken, istatistiksel analizler için SPSS 26 (Statistical Package for Social Sciences) programı kullanıldı. Katılımcıların %69,2'si (n:90) kadın, %30,8'i (n:40) erkektir. Katılımcıların yaşları 23 ile 82 arasında değişmekte olup yaş ortalamaları 56,73±10,66'dır. Son HbA1c ölçümleri ortalaması 7,69±1,86 görülmüştür. Altı aylık HbA1c değişimleri ise 0,33±1,09 (p:0,002) olarak saptanmıştır. DASS-21 (Depresyon Anksiyete Stres Ölçeği) alt boyutları ile 6 aylık HbA1c değişimi arasında anlamlı ilişki saptanmamıştır. DASS-21'in depresyon alt boyutu ile ÇBDA (Çok Boyutlu Diyabet Anketi) alt boyutları arasındaki ilişki incelenmiştir. Depresyon ile engel algısı arasında pozitif, destek algısı arasında negatif yönlü anlamlı ilişki saptanmıştır (p:0,001; p:0,003). Anksiyete ile özyeterlilik algısı arasında negatif yönlü anlamlı ilişki saptanmıştır (p:0,009). Stres ile engel algısı arasında pozitif, destek algısı arasında negatif yönlü anlamlı ilişki saptanmıştır (p:0,001; p:0,033) Diyabetli hastaların destek algısındaki eksiklikleri glisemik kontrollerini olumsuz etkilemektedir. Bu da komplikasyonları, mortalite ve morbiditeleri beraberinde getirebileceğinden hastalara sağlık kurum ve kuruluşlarınca özbakım ve destek algısı ile psikososyal durumlarının gelişmesini sağlayacak olanaklar sağlanmalıdır.
Diabetes is a common health problem with an increasing prevalence worldwide and its treatment can be sustained with self-care, social support of relatives and good psychosocial status. The aim of this study was to investigate the effect of self-care, social support and psychosocial status on the change in six-month HbA1c measurements of patients with diabetes who applied to the diabetes obesity outpatient clinic of our hospital. The study included 130 diabetic patients aged 18 years and over who applied to Göztepe Prof. Dr. Süleyman Yalçın City Hospital Diabetes Obesity outpatient clinic and agreed to participate in the study. A case report form, multidimensional diabetes questionnaire and depression anxiety stress scale were answered by the patients included in the study. The HbA1c measurements of the patients 6 months ago, 3 months ago and current HbA1c measurements were obtained from the hospital software and patient files. SPSS 26 (Statistical Package for Social Sciences) software was used for statistical analyses. 69.2% (n:90) of the participants were female and 30.8% (n:40) were male. The ages of the participants ranged between 23 and 82 years with a mean age of 56.73±10.66 years. The mean of the last HbA1c measurements was 7.69±1.86. Six-month HbA1c changes were 0.33±1.09 (p:0.002). No significant relationship was found between the sub-dimensions of DASS-21 (Depression Anxiety Stress Scale) and 6-month HbA1c change. The relationship between the depression sub-dimension of DASS-21 and the Multidimensional Diabetes Questionnaire (MDQ) sub-dimensions was analysed. There was a significant positive correlation between depression and perception of obstacle and a significant negative correlation between perception of support (p:0.001; p:0.003). A significant negative relationship was found between anxiety and self-efficacy perception (p:0,009). There was a positive significant relationship between stress and perception of obstacle and a negative significant relationship between perception of support (p:0,001; p:0,033). The lack of support perception of patients with diabetes negatively affects their glycaemic control. Since this may lead to complications, mortality and morbidity, patients should be provided with opportunities to improve their self-care and support perception and psychosocial status by health institutions and organisations.
