Diyabeti olan hastaların insülin tedavisi karşı tutum ve davranışları

Özet
Bu çalıs¸manın amacı, diyabetli bireylerin insülin tedavisine yo¨nelik tutumlarını ve bu tutumların arkasındaki psikososyal etmenleri anlamaktır. Çalıs¸maya, Go¨ztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesi Diyabet-Obezite poliklinigˆine bas¸vuran, daha o¨nce tanı almıs¸ veya yeni tanı konulan, insülin ve/veya oral antidiyabetik kullanan, 18 yas¸ üstü, gebe olmayan ve kendi kendine enjeksiyon yapmasına engel tes¸kil eden bir komplikasyonu bulunmayan 225 hasta dahil edilmis¸tir. Hastalarla yüz yüze go¨rüs¸ülerek sosyodemografik o¨zelliklerini belirlemek için Hasta Tanımlama Formu, depresyon, anksiyete ve stres belirtilerini o¨lçmek amacıyla DASS-21 Ölçegˆi ve insülin tedavisine kars¸ı tutum ve davranıs¸larını o¨lçmek için İnsülin Tedavisine Yo¨nelik Tutum Ölçegˆi (İTDÖ) uygulanmıs¸tır. Bu aras¸tırmada elde edilen veriler, SPSS (Statistical Package for Social Sciences) 22.0 paket programı ile analiz edilmis¸tir. Katılımcıların %66,2'si (n=149) kadın ve %33,8'i (n=76) erkektir. Katılımcıların yas¸ları 18 ile 84 arasında degˆis¸mekte olup yas¸ ortalaması 53,50 ± 14,33'tür. Hastaların %80,4'ü (n=181) Tip 2 Diyabetlidir. Ortalama diyabet tanı süresi 15,30 ± 10,01 yıldır. Hastaların %81,3'ünde (n=183) ek kronik hastalık bulunmakta olup %52,9'u (n=119) hipertansiyonlu hastadır. Diyabetli hastalarının 75'i yalnızca oral antidiyabetik ilaç, 75'i yalnızca insülin, 75'i ise oral antidiyabetik + insülin tedavisi almaktadır. Çalıs¸mamızda İTDÖ pozitif tutum alt boyutu skor ortalaması 9,51 ± 2,85; İTDÖ negatif tutum alt boyutu skor ortalaması 41,28 ± 1,01; İTDÖ toplam puan ortalaması 50,80 ± 11,63 bulunmus¸tur. Katılımcıların %54,7'sinde anksiyete, %44,4'ünde depresyon, %30,7'sinde stres oldugˆu go¨zlemlenmis¸tir. Yas¸ ile İTDÖ toplam puanı arasında pozitif yo¨nde orta derecede anlamlı korelasyon vardır (r=0,405; p<0,001). Kadınlarda, İTDÖ negatif tutum alt boyutu ve İTDÖ toplam puan düzeyi erkeklere go¨re daha yüksektir (p=0,022 ve p=0,034). Tip 2 Diyabetli hastalarda, Tip 1 Diyabetli hastalara go¨re İTDÖ negatif tutum alt boyut ve İTDÖ toplam puan düzeyi daha yüksektir. Diyabet egˆitimi almamıs¸ hastaların İTDÖ negatif tutum alt xi boyut ve İTDÖ toplam puanı, diyabet egˆitimi almıs¸ hastalara go¨re daha yüksektir (p=0,002; p=0,001). Kadın hastalarda erkek hastalara go¨re DASS-21 depresyon ve anksiyete puanı daha yüksektir (p=0,005 ve p<0,001). Diyabet tipi ve tedavi grupları ile DASS-21 skorları arasında anlamlı bir fark yoktur. Sadece oral antidiyabetik alan hastaların İTDÖ toplam ve negatif alt boyut puanı, insülin kullanan hastalara go¨re daha yüksektir (p<0,001). Çalıs¸mamızda, diyabetli bireylerde kadın olmanın, ileri yas¸ın, diyabet egˆitimi almamıs¸ olmanın, kısa diyabet süresinin ve sadece oral antidiyabetik kullanımının, insüline yo¨nelik olumsuz tutumlarla ilis¸kili oldugˆunu belirledik. Ayrıca, kadınlar ve düs¸ük gelir seviyesine sahip bireyler arasında depresyon, anksiyete ve stres düzeylerinin belirgin s¸ekilde yüksek oldugˆunu tespit ettik.
The aim of this study is to understand the attitudes of diabetic individuals towards insulin therapy and the psychosocial factors behind these attitudes. The study included 225 patients who applied to the Diabetes-Obesity Clinic at Go¨ztepe Prof. Dr. Süleyman Yalçın City Hospital, who were previously diagnosed or newly diagnosed, using insulin and/or oral antidiabetics, over the age of 18, not pregnant, and without any complications that would prevent self-injection. Face-to-face interviews were conducted with the patients using the Patient Identification Form to determine their sociodemographic characteristics, the DASS-21 Scale to measure symptoms of depression, anxiety, and stress, and the Attitude Towards Insulin Therapy Scale (ATITS) to measure their attitudes and behaviors towards insulin therapy. The data obtained in this research were analyzed using the SPSS (Statistical Package for Social Sciences) 22.0 package program. Of the participants, 66.2% (n=149) were female and 33.8% (n=76) were male. The ages of the participants ranged from 18 to 84 years, with a mean age of 53.50 ± 14.33 years. 80.4% (n=181) of the patients had Type 2 Diabetes. The average duration of diabetes diagnosis was 15.30 ± 10.01 years. 81.3% (n=183) of the patients had additional chronic diseases, with 52.9% (n=119) having hypertension. Seventy-five patients were on oral antidiabetics only, seventy-five were on insulin only, and seventy-five were on a combination of oral antidiabetics and insulin therapy. In our study, the mean score for the positive attitude subscale of ATITS was 9.51 ± 2.85; the mean score for the negative attitude subscale of ATITS was 41.28 ± 1.01; and the mean total ATITS score was 50.80 ± 11.63. It was observed that 54.7% of the participants had anxiety, 44.4% had depression, and 30.7% had stress. There was a moderately significant positive correlation between age and the total ATITS score (r=0.405; p<0.001). Women had higher scores on the negative attitude subscale and total ATITS score compared to men (p=0.022 and p=0.034). Patients with Type 2 Diabetes had higher scores on the negative attitude subscale and total ATITS score compared to those xiii with Type 1 Diabetes. Patients who had not received diabetes education had higher scores on the negative attitude subscale and total ATITS score compared to those who had received diabetes education (p=0.002; p=0.001). Women had higher DASS-21 depression and anxiety scores compared to men (p=0.005 and p<0.001). There was no significant difference in DASS-21 scores between diabetes types and treatment groups. Patients on oral antidiabetics only had higher total and negative subscale ATITS scores compared to those using insulin (p<0.001). In our study, we found that being female, older age, lack of diabetes education, shorter duration of diabetes, and use of only oral antidiabetics were associated with negative attitudes towards insulin. Additionally, we identified significantly higher levels of depression, anxiety, and stress among women and individuals with lower income levels.
