dc.description.abstractÜveitik katarakt cerrahisinde postoperatif bakımda yüksek doz sistemik steroid kullanımı bir tabu olarak kabul görmüş durumdadır. Biz buna alternatif olarak posterior subtenon steroid enjeksiyonlarını 2018 yılından beri rutin olarak kullanmaktayız. Biz kendi tedavi protokolümüz (ilave sistemik steroid kullanılmadan sadece posterior subtenon trimasinolon asetonid (STTA) enjeksiyonu) ile hastalarımızda klinik başarının iyi olduğu ve steroid ile ilişkili komplikasyon oranlarının ise oldukça düşük olduğu yönünde bir gözlemde bulunduk. Çalışmamızın amacı bu gözlemimizi dökümante etmek ve kendi rutinimizde uygulamakta olduğumuz tedavi protokolünün etkinliğini tanımlamaktır. Göztepe Prof. Dr. Süleyman Yalçın Şehir hastanesinde 2018-2024 yılları arasında Göz Hastalıkları Kliniğinin Uvea Biriminde takipli olan 1644 hastanın dosyası retrospektif olarak incelendi, fakoemülsifikasyon yöntemi ile katarakt ekstraksiyonu ve göz içi lens (GİL) implantasyonu yapılan 85 üveitik katarakt hastasının 108 gözü çalışmaya alındı. Tüm hastalara preoperatif üveit polikliniğinde detaylı göz muayenesi yapılarak intraoperatif inflamasyonun kontrol altında olduğundan emin olundu. Ameliyat öncesinde en az 3 ay intraoküler enflamasyonu sakin olan hastalar ameliyata alındı ve sistemik tedavi alan hastaların immünosüpresyon tedavisinde veya dozajında son 3 ayda herhangi bir değişiklik yapılmadı. Peroperatif veya erken postoperatif dönemde rutin olarak hastaların üveite yönelik kullanmakta olduğu sistemik tedavilerinde değişiklik yapılmadı. Üveit atağı durumu söz konusu olduysa ilgili üveitin eşlik ettiği sistemik etiyolojiye göre (Behçet, satkoidoz, ankilozan spondilit gibi) tedavi edildi. Non-enfeksiyoz üveitik katarakt cerrahisi olan tüm hastaların yaş, cinsiyet, sistemik hastalıklar ve ameliyatı planlanan göze dair demografik özellikleri kaydedildi. Pre-op ve post-op en iyi düzeltilmiş görme keskinliği (EİDGK), göz içi basınç (GİB), santral makula kalınlığı (SMK), intraoperatif ve 6 aylık takipteki postoperatif komplikasyonları incelendi. Takip muayeneleri ameliyattan 1gün, 7gün,14gün, 1, 3 ve 6 ay sonra yapıldı. İstatiksel analiz IBM SPSS Statistics 23 paket programı kullanılarak analiz edilmiştir. Ameliyat edilen gözlerden 53'ü sağ (%49,1), 55'i sol (%50,9) gözü içeriyordu. Çalışmamızda final EİDGK ile üvetin anatomik lokalizasyonu arasında istatistiki olarak anlamlı bir farklılaşma olduğu görülmüştür (p<0,05), ve tüm anatomik grupların görme keskinliğindeki değişim açısından gruplar arası anlamlı bir fark saptandı (p=0.001). Ameliyat sonrası EİDGK değerleri ön üveit grubunda 0,10 ± 0,27 Log-MAR, orta üveit grubunda 0,16 ± 0,19 Log-MAR, arka üveit grubunda 0,25 ± 0,32 Log-MAR, panüveit grubundan 0,35 ± 0,51 Log-MAR olarak tespit edildi. Ayrıca final EİDGK (Log-MAR) ile üveit tanı grubu arasında da istatistiki olarak anlamlı bir farklılaşma olduğu görülmüştür (p<0,05). Ameliyat sonrası final EİDGK değerleri idiopatik non-enfeksiyöz ön üveit grubunda 0,14 ± 0,18 Log-MAR, Behçet üveiti grubunda 0,25 ± 0,51 Log-MAR, idiopatik non-enfeksiyöz orta üveit grubunda 0,18 ± 0,22 Log-MAR, Fuchs üveiti grubunda 0,06 ± 0,15 Log-MAR, Sarkoidoz üveitinde 0,41 ± 0,54 Log-MAR olarak tespit edildi. Olgularımızda ameliyat öncesi ortalama GİB 15,68 ± 3,83 idi, ameliyat sonrası takip periyodunun 7.gün ortalama GİB 12,62 ± 3,47, final GİB 12,90 ± 2,82 olduğunu izledik. Çalışmamızda gözlerin %29,6'sında geçici GİB artışı olduğunu, %3,7'sinde postoperatif sekonder glokom ve bu gözlerin yalnızca %0,92'sinin glokom ameliyatı gerektirdiğini gözlemledik. Postoperatif 7. gün atak oranımız toplam 43 göz (%47,3), postoperatif 1.ay atak oranımız 11göz (%11,6) içermekte idi. Hastalarımızdan post-op 3 olguda (%2,7) pupiller membran gelişti. Çalışmamızda pre-op pupiller sineşi ile post-op pupiller membran oluşumu arasında istatistiki olarak anlamlı bir ilişki olduğu görülmüştür (p<0,05). Post-op pupiller membran oluşan gözlerin tümü pre-op seklüzyo pupil olan hastalardı. 108 gözden sadece 9 gözde (%8,3) arka kapsül kesafeti (AKK) gelişti ve 3 gözde (%2,7) görme düzeyi ve kalitesini etkilediğinden İdriyum alüminyum garnet (YAG) lazer uygulanarak arka kapsül temizliği yapıldı. Ameliyat sonrası 5 gözde (%4,6) posterior sineşi, 1 olguda (%0,9) periferik anterior sineşi (PAS), 5 gözde (%4,6) makula ödemi, 4gözde (%3,7) post-op epiretinal membran (ERM) ve 1 hastada (%0,9) retina dekolmanı gelişti.İntraoperatif posterior STTA, uzun süreli sistemik steroid kullanımından muzdarip olan ve eşlik eden sistemik hastalıkları olan hastalar için özellikle uygun olabilir. Sonuçlarımız hastaların peroperatif oral kortikosteroid kullanım süresinden tasarruf sağlayabileceğini açıkça göstermektedir. dc.description.abstractUsing high-dose systemic steroids in postoperative care in uveitic cataract surgery has been accepted as a taboo. As an alternative, we have been routinely using posterior subtenon steroid injections since 2018. We observed that our patients developed fewer complications with our own treatment protocol (only posterior subtenon trimacinolone acetonide (STTA) injection without additional systemic steroids) and that the rate of steroid-related complications was quite low. The aim of our study is to document this observation and to describe the effectiveness of the treatment protocol we apply in our routine. The files of 1644 patients who were followed up in the Uvea Unit of the Ophthalmology Clinic at Goztepe Prof. Dr. Suleyman Yalchın City Hospital between 2018-2024 were retrospectively reviewed, and 108 eyes of 85 uveitic cataract patients who underwent cataract extraction and IOL implantation with phacoemulsification were included in the study. All patients underwent a detailed eye examination in the preoperative uveitis clinic to ensure that intraoperative inflammation was under control. Patients with calm intraocular inflammation for at least 3 months before surgery were taken into surgery, and no changes were made to the immunosuppression treatment or dosage of patients receiving systemic treatment in the last 3 months. No changes were made to the systemic treatments routinely used by patients for uveitis in the perioperative or early postoperative period. If there was a uveitis attack, it was treated according to the systemic etiology accompanying the relevant uveitis (such as Behcet, squamous cell disease, ankylosing spondylitis). The age, gender, systemic diseases, and demographic characteristics of the eye planned to be operated on were recorded for all patients who underwent non-infectious uveitic cataract surgery. Preoperative and postoperative best corrected visual acuity (BCVA), intraocular pressure (IOP), central macular thickness (CMT), intraoperative and 6-month postoperative complications were examined. Follow-up examinations were performed 1 day, 7 days, 14 days, 1, 3 and 6 months after surgery. Statistical analysis was performed using IBM SPSS Statistics 23 package program. 53 of the operated eyes were right (49.1%) and 55 were left (50.9%). In our study, a statistically significant difference was observed between the final BCVA and the anatomical localization of the uvea (p<0.05), and a significant difference was found between the groups in terms of visual acuity change in all anatomical groups (p=0.001). Postoperative BCVA values were determined as 0.10 ± 0.27 Log-MAR in the anterior uveitis group, 0.16 ± 0.19 Log-MAR in the intermediate uveitis group, 0.25 ± 0.32 Log-MAR in the posterior uveitis group, and 0.35 ± 0.51 Log-MAR in the panuveitis group. In addition, a statistically significant difference was observed between the final BCVA (Log-MAR) and the uveitis diagnosis group (p<0.05). Postoperative final BCVA values were determined as 0.14 ± 0.18 Log-MAR in the idiopathic non-infectious anterior uveitis group, 0.25 ± 0.51 Log-MAR in the Behçet uveitis group, 0.18 ± 0.22 Log-MAR in the idiopathic non-infectious intermediate uveitis group, 0.06 ± 0.15 Log-MAR in the Fuchs uveitis group, and 0.41 ± 0.54 Log-MAR in the sarcoidosis uveitis group. In our cases, the mean preoperative IOP was 15.68 ± 3.83, the mean postoperative follow-up period was 7th day 12.62 ± 3.47, and the final IOP was 12.90 ± 2.82. In our study, we observed that 29.6% of the eyes had transient IOP increase, 3.7% had postoperative secondary glaucoma, and only 0.92% of these eyes required glaucoma surgery. Our postoperative 7th day attack rate included a total of 43 eyes (47.3%), and our postoperative 1st month attack rate included 11 eyes (11.6%). Pupillary membrane developed in 3 cases (2.7%) of our patients postoperatively. In our study, a statistically significant relationship was observed between preoperative pupillary synechiae and postoperative pupillary membrane formation (p<0.05). All eyes with postoperative pupillary membrane were patients with preoperative seclusio pupil. Only 9 eyes (8.3%) out of 108 eyes developed ACC and 3 eyes (2.7%) underwent posterior capsule cleaning by applying YAG laser because it affected the level and quality of vision. Postoperatively, posterior synechiae developed in 5 eyes (4.6%), peripheral anterior synechiae (PAS) in 1 case (0.9%), macular edema in 5 eyes (4.6%), postoperative ERM in 4 eyes (3.7%) and retinal detachment in 1 patient (0.9%). Intraoperative posterior STTA may be particularly suitable for patients suffering from longterm systemic steroid use and those with extensive systemic disease. Our results clearly show that patients can save time from perioperative oral corticosteroid use.
