Huzurevlerinde yaşayan 65 yaş üstü kişilerde pnömokok, influenza ve COVİD-19 aşılamalarının COVİD-19 hospitalizasyonuna ve progresyonuna etkisi

Özet

AMAÇ: Bu çalışmadaki amacımız huzurevlerinde yaşayan 65 yaş üstü kişilerde; pnömokok, influenza ve COVID-19 aşılamalarının COVID-19 enfeksiyonuna yakalanma durumuna, hospitalizasyona ve hastalığın progresyonuna etkisini değerlendirmektir. YÖNTEM: Bu çalışma İstanbul ili Üsküdar ilçesine bağlı tüm huzurevi ve yaşlı bakım merkezlerinde gerçekleştirilmiştir. Araştırma retrospektif kohort şeklinde planlanmıştır. 01.12.2021-31.01.2022 tarihleri arasında verilerin bir kısmı yüz yüze anket yöntemiyle toplanmış; kalan kısmı 01.12.2021- 31.03.2022 tarihleri arasında sağlık bilgi sistemleri üzerinden elde edilmiştir. Anketin yüz yüze gerçekleştirilen kısmında; yaş, cinsiyet, eğitim durumu, oda paylaşım durumu, yemeklerini çoğunlukla yediği yer, sosyal alan kullanım sıklığı, kronik hastalık bilgileri ve bağışıklık durumunu etkileyebilecek sağlıklı yaşam davranışları sorgulanırken; kayıtlardan elde edilen veriler ile kişilerin COVID-19 tanı ve izlem bilgileri ile pnömokok, influenza ve COVID-19 aşılanma durumlarına erişilmiştir. Kategorik yapıdaki verilerin karşılaştırılmasında ise Chi-Square testi, sayısal değişkenlerin karşılaştırılmasında Student-t testi, tek değişkenli analizlerde anlamlı çıkan değişkenlerin COVID-19 enfeksiyonu tanısı, hastaneye yatış,yoğun bakım ünitesine kabul ve COVID-19 ilişkili ölüm insidansı üzerindeki öngörücü etkisi Cox regresyon analizi ile değerlendirilmiştir. BULGULAR: Çalışmaya dâhil edilen 637 kişi içinde COVID-19 tanı sıklığı %49, hastaneye yatış sıklığı %22,4, yoğun bakıma yatış sıklığı %12,2, entübasyon sıklığı %10,4, mekanik ventilasyon sıklığı %11,1, COVID-19 ilişkili ölüm sıklığı %9,7 olarak tespit edilmiştir. En fazla yapıldığı yıl %32,5 ile 2020 yılı olmak üzere %40,2'si daha önce pnömokok aşısı olmuş, 2020 ve 2021 yılı sırasıyla %38,3 ve %45,6 olmak üzere %63,7'si en az 1 doz influenza aşısı olmuş, %85,7'si en az 1 doz COVID-19 aşısı olmuş kişilerdir. Araştırmamızın sonuçlarına göre; erkek cinsiyet, ortaokul mezunu olmak, 2 kişilik odada kalmak, sosyal alanları her zaman kullanmak COVID-19 tanı alma durumuna karşı riskli bulunmuş, COVID-19 aşı dozunun artması koruyuculuğunun artmasını sağlamıştır. Hastaneye yatış durumunu etkileyen faktörler incelendiğinde; erkek cinsiyet ve kronik hastalık durumu riskli bulunurken, 4 doz COVID-19 aşısı olmak koruyucu bulunmuş, tanı anında COVID-19 aşısı ile beraber influenza aşısı olmasının koruyucu etkisi pnömokok aşısının da eklenmesi ile artmıştır. Çalışmamızda hastaneye yatan COVID-19 tanılı kişilerden erkeklerin, 85 yaş üstünde olanların ve kronik hastalığı olanların daha fazla yoğun bakım yatışı olduğu ve COVID-19 aşısı ile beraber pnömokok aşısı olanların, COVID-19 ile birlikte influenza aşısı olanlara ve her 3 aşısı da olanlara göre daha az yoğun bakıma yattığı görülmüştür. Benzer olarak erkeklerin, 85 yaş üstünde olanların ve kronik hastalığı olanların daha fazla entübe edildiği ve COVID-19 aşısı ile beraber pnömokok aşısı olanların, COVID-19 ile birlikte influenza aşısı olanlara ve her 3 aşısı da olanlara göre daha az entübe edildiği görülmüştür. Ayrıca COVID-19 ilişkili ölüm durumunu etkileyen faktörler incelendiğinde; erkek cinsiyet riskli, COVID-19 aşısı ile beraber pnömokok ve influenza aşısının mevcut olması koruyucu olarak bulunmuştur. SONUÇ: Sonuçlarımız COVID-19 aşısı varlığının ve dozunun tanı alma üzerinde; influenza ve pnömokok aşılarının varlığının COVID-19 hastalığının progresyonu üzerinde olumlu etkisinin olduğu yönündedir. Araştırmamız her 3 hastalık için de riskli olan bu grubun bağışıklama davranışını ve farkındalığını artırmaya yönelik teşvik edici olabilir. Ayrıca çalışmamız influenza ve pnömokok aşılarının COVID-19 aşısıyla beraber mevcut olduğunda hastalık üzerindeki etkisini incelemesi yönüyle literatüre katkı sağlayabilir.

Objective: Our aim in this study is to evaluate the effect of pneumococcal, influenza and COVID-19 vaccinations on the status of contracting COVID-19 infection, hospitalization and disease progression for people over 65 years old living in nursing homes. Method: This study was carried out in all nursing homes and elderly care centers in Üsküdar, Istanbul. The study was planned as a retrospective cohort. Between 01.12.2021 and 31.01.2022, some of the data were collected by face-to-face survey method; the remaining part was obtained through health information systems between 01.12.2021 and 31.03.2022. In the face- to-face part of the survey; age, gender, education level, sharing the same room, the most eating area, frequency of use of social space, chronic disease status and healthy living behaviors that may affect the immune status were questioned. With the data obtained from the records, the COVID-19 diagnosis and follow-up information of the individuals and their pneumococcal, influenza and COVID-19 vaccination status were accessed. Chi-square test was used to compare categorical data and Student's t test was used to compare numerical variables. The predictive effect of significant variables in univariate analyzes on the diagnosis of COVID-19 infection, hospitalization, admission to the intensive care unit, and the incidence of COVID-19-related death was evaluated with Cox regression analysis. Results: In 637 people included in the study, the frequency of diagnosis of COVID-19 was 49%, the frequency of hospitalization was 22.4%, the frequency of admission to the intensive care unit was 12.2%, the frequency of intubation was 10.4%, the frequency of mechanical ventilation 11.1%, the frequency of COVID-19-related death rate was determined as 9.7%. 40.2% of them received pneumococcal vaccine (The highest year was 32.5%, 2020) 63.7% of them received at least 1 dose of influenza vaccine (38.3% and 45.6% in 2020 and 2021) 85.7% of them received at least 1 dose of COVID-19 vaccine. According to the results of our research; male gender, being a secondary school graduate, staying in a double room, using social areas constantly were found to be risky for the diagnosis of COVID-19; increasing dose of COVID-19 vaccine found to be protective. When the factors affecting the hospitalization status are examined; male gender and chronic disease status were found to be risky, being vaccinated with 4 doses of COVID-19 vaccine was found to be protective. The protective effect of the presence of influenza vaccine together with COVID-19 vaccine at the time of diagnosis increased with the addition of pneumococcal vaccine. Among the people hospitalized with the diagnosis of COVID-19; men, people over the age of 85 and those with chronic diseases were more admitted to intensive care. Those who received the pneumococcal vaccine together with the COVID-19 vaccine were admitted to the intensive care unit less than those who received the influenza vaccine together with the COVID-19 vaccine and those who received all 3 vaccines. Similarly; men, people over the age of 85 and those with chronic diseases were more intubated. Those who received the pneumococcal vaccine together with the COVID-19 vaccine were intubated less than those who received the influenza vaccine together with the COVID-19 vaccine and those who received all 3 vaccines. In addition, when the factors affecting the COVID-19-related death situation are examined; male gender was found to be risky, co-existence of pneumococcal and flu vaccine with COVID-19 vaccine was found to be protective. Conclusion: Our results showed that the presence and dose of the COVID- 19 vaccine had positive effect on the diagnosis; the presence of influenza and pneumococcal vaccines had positive effect on the progression of COVID-19 disease. Our research can be an incentive to increase immunization behavior and awareness of this group at risk for all 3 diseases. In addition, our study may contribute to the literature in terms of examining the effects of influenza and pneumococcal vaccines together with the COVID-19 vaccine on the disease.
