Özet

Beslenme, çocukların sağlıklı büyümesi ve gelişmesi için en önemli etkenlerden biridir. Gebelik ve çocukluk çağında kritik öneme sahiptir. Özellikle ilk 24 aya kadar devam eden süreçte beslenme çocuğun büyümesi, gelişimi ve yaşama sağlıklı başlaması için oldukça önemlidir. Çalışmamızda 0-24 ay arası sağlıklı süt çocuklarında beslenme durumu değerlendirildi. Tek merkezli prospektif olarak gerçekleştirilen bu çalışmada Şubat 2023 ve Ağustos 2023 tarihleri arasında Çocuk Sağlığı ve Hastalıkları polikliniğine başvuran 6-24 ay arası, 380/7 gestasyon haftası ve üzerinde doğan, kronik, metabolik ve sendromik hastalığı olmayan sağlıklı bebek ve süt çocuklarının ebeveynlerine anket çalışması yapıldı. 6 aylık süreçte polikliniklerimize rutin muayene amacıyla başvuran 500 sağlıklı süt çocuğunun ebeveynlerine yapılan anket çalışması sonucunda hastaların %50'si kız idi. Süt çocuklarının yaş ortalaması 16,39±5,44 ay, çalışmaya katılan annelerin yaş ortalaması 29,11±4,32 yıldı. Anketi dolduran annelerin %45'i lise, %34'ü ise üniversite ve üzeri eğitim düzeyine sahipti. Annelerin 133 (%26,6)'ü çalışıyordu. Çalışmaya katılan ebeveynlerin 407 (%81,4)'si çekirdek aile yapısına sahipti ve 245 (%49)'i bir çocuk sahibiydi. Üniversite mezunu annelerde tek çocuk sahibi olma oranı (n=99), ilkokul (n=5) ve ortaokul mezunu (n=17) annelere göre istatistiki açıdan anlamlı şekilde daha yüksekti (p<0.001). Altmış beş (%13) ailenin akraba evliliği yaptığı gözlendi. Üç yüz altmış dört (%72,8) anne çocuğuna kendisi bakıyordu. Annelerin 378 (%75,6)'i doğum öncesi emzirme ve anne sütü hakkında bilgi edinmişti ve 275 (%55)'i doğumdan sonra ilk 1 saat içinde bebeğini emzirmişti. Dört yüz seksen iki (%96,4) annenin doğumdan sonra bebeğine ilk anne sütü verdiği gözlendi. Bebeklerin sadece anne sütü alma süresi ortalama 5,14±1,55 ay iken, toplam anne sütü alma süresi 12,46±4,77 aydır. Bebeklerin ek gıdaya geçme zamanının ortalama 5,73±0,7 ay olduğu görüldü. En sık verilen ek gıda yoğurt (n=158, %31,6) idi. Ebeveynlerin 285 (%57)'inin beslenme sırasında çocuğun dikkatini çekmek için farklı yöntemler uyguladığı, en sık uygulanan yöntemin ise 'TV/telefon eşliğinde besleme' (n=119 ve%41,8) olduğu görüldü. Tamamlayıcı beslenmede günde en az dört besin grubu ile beslenebilen bebek sayısı 300 (%60) idi. Annenin eğitim düzeyi ile doğum şekli, doğum sonrası bebeğe ilk verilen besin, ilk emzirme zamanı, emzirme süresi, çocuk sayısı arasındaki ilişki analiz edildi. Okur-yazar olmayan annelerde normal doğum ile doğurma sıklığı (%85,7), üniversite mezunu annelere göre anlamlı olarak daha yüksekti (p<0.01). Doğum sonrası bebeğe verilen ilk besinin anne sütü olması lise (n=219, %97,3) ve üniversite mezunu (n=154, %98,7) annelerde, okur-yazar olmayan annelere göre istatistiki açıdan anlamlı şekilde daha yüksekti (p=0.002). Üniversite mezunu annelerin bebeklerini doğumdan sonra ilk 1 saat içinde emzirme oranları (%40,4) ilkokul (%78,6) ve lise (%58,7) mezunu annelere oranla anlamlı derecede düşüktü (p<0.001). Bebeğe 6 aydan önce ek gıda verilmesi eğilimine ilişkin dağılımlara bakıldığında ortaokul mezunu annelerin (%46,4), lise mezunu annelere (%20,9) göre bebeklerine 6 aydan önce ek gıda verme eğilimlerinin daha yüksek olduğu görülmekte idi (p=0.001). Lise mezunu annelerde ilk 6 ay sadece anne sütü verme oranı (%70,2) ortaokul mezunu olan annelere (%46,4) göre anlamlı olarak yüksekti (p=0.016). İlave olarak doğum öncesi emzirme ve anne sütü hakkında bilgi alan annelerin ilk 6 ay sadece anne sütü ile besleme oranı (%65,6) almayanlara oranla (%55,7) istatistiksel olarak daha yüksekti (p=0.049). Annenin yaş aralığına göre ise 26-30 yaş arası annelerin ilk bir saat içinde emzirme oranı (%58,7) >41 yaş annelere (%11,1) göre anlamlı olarak yüksekti (p=0.021). Çalışmamızda 0-24 ay arası sağlıklı süt çocuklarının beslenme durumları değerlendirilmiş olup annenin eğitim düzeyinin, doğum öncesi anne sütü ve emzirme konusunda bilgi edinmenin, tamamlayıcı beslenme hakkında bilgi edinmenin süt çocuğunun doğru beslenmesinde önemli faktörler olduğu görülmüştür. Ülkemizde tüm annelerin bebeklerini anne sütü ile emzirmesinin desteklenmesi ve emzirme sorunlarının giderilmesi için çok ciddi stratejilere gereksinim vardır. Tüm çocukların ilk altı ay sadece anne sütü ile beslenebilmesi için etkin adımlar atılması önem arz etmektedir.

Nutrition is one of the most important factors for the healthy growth and development of children. It is critical during pregnancy and childhood. Especially in the first 24 months, nutrition is very important for the child's growth, development and healthy start to life. In our study, the nutritional status of healthy infants aged 0-24 months was evaluated. In this single-center, prospective study, a survey was conducted on the parents of healthy infants and infants born at 380/7 weeks of gestation and born between 6-24 months of age, who applied to the Pediatrics Outpatient Clinic between February 2023 and August 2023, without chronic, metabolic and syndromic diseases. As a result of the survey conducted with the parents of 500 healthy infants who applied to our polyclinics for routine examination in a 6-month period, 50% of the patients were girls. The mean age of the infants was 16.39±5.44 months, and the mean age of the mothers participating in the study was 29.11±4.32 years. Of the mothers who filled out the questionnaire, 45% had a high school education and 34% had a university or higher education level. 133 (26.6%) of the mothers were working. 407 (81.4%) of the parents participating in the study had a nuclear family structure and 245 (49%) had a child. The rate of having a single child (n=99) among university graduate mothers was statistically significantly higher than primary school (n=5) and secondary school graduates (n=17) (p<0.001). It was observed that sixty-five (13%) families had consanguineous marriages. Three hundred and sixty-four (72.8%) mothers were taking care of their children by themselves. 378 (75.6%) of the mothers had learned about prenatal breastfeeding and breast milk, and 275 (55%) breastfed their baby within the first hour after birth. It was observed that four hundred and eighty-two (96.4%) mothers gave their baby first breast milk after birth. While the average duration of exclusive breastfeeding of babies was 5.14±1.55 months, the total duration of breastfeeding was 12.46±4.77 months. It was observed that the mean time for infants to switch to solid food was 5.73±0.7 months. The most frequently given supplementary food was yogurt (n=158, 31.6%). It was observed that 285 (57%) of the parents used different methods to attract the attention of the child during feeding, and the most frequently used method was 'feeding with TV/phone' (n=119 and 41.8%). The number of infants who could be fed with at least four food groups per day in complementary feeding was 300 (60%). The relationship between the education level of the mother and the mode of delivery, the first food given to the baby after birth, the first breastfeeding time, the duration of breastfeeding, and the number of children were analyzed. The frequency of delivery by normal delivery in illiterate mothers (85.7%) was significantly higher than in mothers with university degrees (p<0.01). Breast milk being the first food given to the baby after birth was statistically significantly higher in high school (n=219, 97.3%) and university graduate (n=154, 98.7%) mothers compared to illiterate mothers (p= 0.002). Breastfeeding rates in the first hour after birth of university graduate mothers (40.4%) were significantly lower compared to mothers who graduated from primary school (78.6%) and high school (58.7%) (p<0.001). Considering the distribution of the tendency to give additional food to the baby before 6 months of age, it was seen that mothers who graduated from secondary school (46.4%) had a higher tendency to give additional food to their babies before 6 months of age compared to mothers who graduated from high school (20.9%) (p=0.001). The rate of exclusive breastfeeding for the first 6 months in high school graduate mothers (70.2%) was significantly higher than in secondary school graduate mothers (46.4%) (p=0.016). In addition, the rate of exclusive breastfeeding in the first 6 months of mothers who received information about prenatal breastfeeding and breast milk was statistically higher (65.6%) than those who did not (55.7%) (p=0.049). According to the age range of the mother, the rate of breastfeeding within the first hour of the mothers aged 26-30 (58.7%) was significantly higher than the mothers aged >41 (11.1%) (p=0.021). In our study, the nutritional status of healthy infants aged 0-24 months was evaluated, and it was observed that the education level of the mother, getting information about prenatal breast milk and breastfeeding, and getting information about complementary feeding were important factors in the proper nutrition of the infant. In our country, there is a need for serious strategies to support all mothers to breastfeed their babies with breast milk and to eliminate breastfeeding problems. It is important to take effective steps so that all children can be fed only with breast milk for the first six months.
