Yenidoğanın indirekt hiperbilirubinemisi tedavisinde aralıklı fototerapi ile devamlı fototerapinin karşılaştırılması

dc.description.abstractİndirekt hiperbilirubinemi yenidoğanlarda en sık hastaneye başvuru nedenlerinden biridir. İndirekt hiperbilirubinemi tedavisinde noninvaziv ve en yaygın kullanılan yöntem fototerapidir. Çalışmamızda yenidoğan indirekt hiperbilirubinemi tedavisinde rutin olarak uyguladığımız devamlı fototerapi modeli ile ışın süresini azaltan aralıklı fototerapi modelinin etkinlikleri karşılaştırıldı. Uygulanan fototerapi modellerinin yan etki görülme insidansı analiz edildi. Tek merkezli prospektif olarak gerçekleştirilen çalışmamızda, Temmuz 2022 ve Nisan 2023 tarihleri arasında yenidoğan yoğun bakım ünitemizde indirekt hiperbilirubinemi nedeniyle fototerapi tedavisi uygulanan 340/7 gestasyon haftasından büyük, ek hastalığı olmayan yenidoğanlar rastgele sayılar tablosuyla randomize edilerek aralıklı veya devamlı fototerapi verildi. Her iki modelde de fototerapi öncesinde ve 6 saatlik uygulama sonrası bilirubin düzeyine bakıldı. Hastaların fototerapi öncesinde ve sonlandırıldığında total serum bilirubin, iyonize kalsiyum, hematokrit değerleri kontrol edildi. Fototerapi sonlandırıldıktan 8 saat sonra rebound bilirubin değerlendirildi. Klinik yan etkiler açısından her iki grup karşılaştırıldı. 10 aylık süreçte ünitemizde 225 yenidoğana indirekt hiperbilirubinemi tanısı ile fototerapi uygulandı. 121 yenidoğan dahil etme kriterlerimize uymadığı için çalışmaya alınamadı. Çalışmaya dahil edilen toplam 104 yenidoğandan 52'sine devamlı fototerapi, 52'sine ise aralıklı fototerapi uygulandı. Çalışma gruplarının cinsiyet, doğum şekli, doğum ağırlığı, yatış ağırlığı, postnatal başvuru yaşı, beslenme şekli, taburcu ağırlığı ve kardeşte fototerapi öyküsü gibi demografik özellikleri iki arasında grup benzer bulundu (p>0,05). Hastaların %49'u erkekti. Annelerin gebelik sayısı ve kan grubu değişkenleri açısından her iki grup benzer özelliklerdeydi. İki çalışma grubunda da en sık etiyoloji ABO uygunsuzluğuydu. Fototerapi öncesinde bakılan iyonize kalsiyum, hematokrit ve pik bilirubin değerleri benzer özellikteydi. Yatış süresinin medyan değeri devamlı fototerapi uygulananlarda 3(2-4) gün, aralıklı fototerapi uygulananlarda 2(2-3) gün olarak istatistiksel açıdan anlamlı bulundu (p=0,032). Fototerapi uygulanma süresinin ortanca değeri devamlı fototerapi uygulananlarda 12(6-15) saat, aralıklı fototerapi uygulananlarda 4(2-4) saatti (p<0,001). Bilirubin düşüş hızı aralıklı fototerapi uygulananlarda ortalama 1,12±0,73 mg/dl/sa, devamlı fototerapi uygulananlarda ise 0,51±0,33 mg/dl/sa idi (p<0,001). Devamlı fototerapi uygulananlarda rebound yükselme hızının ortalama değeri 0,08±0,28 mg/dl/sa, aralıklı fototerapi uygulananlarda -0,01±0,17 mg/dl/sa idi (p=0,039). Bilirubin ve iCa değişim yüzdesinde, yatış ve taburculuk arası tartı değişim yüzdesinde iki grup arasında anlamlı farklılık görülmedi (p>0,05). Her iki çalışma grubunda deri döküntüsü, ishal, ilk ve 2. seans sonunda vücut ısı artışları karşılaştırıldığında aralıklı fototerapi uygulananlarda bu yan etkiler istatistiksel olarak daha az bulundu (p<0,001). Çalışmamızda aralıklı ve devamlı fototerapinin total serum bilirubinini düşürmede birbirlerine üstünlüğü olmadığı ve aralıklı fototerapi uygulanan hastaların daha kısa sürede taburcu oldukları gösterilmiştir. Aralıklı fototerapi grubunda vücut sıcaklığı, deri döküntüsü ve ishal yan etkilerinin daha az mevcut olduğu gösterilmiştir.
dc.description.abstractIndirect hyperbilirubinemia is one of the most common reasons for hospital admission in newborns. Phototherapy is the non-invasive and most widely used method in the treatment of indirect hyperbilirubinemia. In our study, the efficacy of the continuous phototherapy model, which we routinely use in the treatment of neonatal indirect hyperbilirubinemia, and the intermittent phototherapy model that reduces the irradiation duration were compared. The incidence of side effects of the applied phototherapy models was analyzed. In our single-center prospective study, newborns without any additional disease, older than 340/7 weeks of gestation who were treated with phototherapy due to indirect hyperbilirubinemia were randomized with a random number table and given intermittent and continuous phototherapy in our neonatal intensive care unit between July 2022 and April 2023. In both models, bilirubin levels were measured before phototherapy and after 6 hours of application. Total serum bilirubin, ionized calcium, and hematocrit values were checked before and after phototherapy. Rebound bilirubin was evaluated 8 hours after phototherapy was terminated. Both groups were compared in terms of clinical side effects. During the 10-month period, phototherapy was applied to 225 newborns with the diagnosis of indirect hyperbilirubinemia in our unit. 121 newborns were excluded because they did not meet our inclusion criteria. Continuous phototherapy was applied to 52 of the 104 newborns included in the study, and intermittent phototherapy was applied to 52 of them. Demographic characteristics of the study groups such as gender, mode of delivery, birth weight, hospitalization weight, weight at discharge, postnatal admission age, diet and phototherapy history in siblings were found to be similar between the two groups (p>0.05). 49% of the patients were male. Both groups were similar in terms of the number of pregnancies and blood group variables of the mothers. ABO incompatibility was the most common etiology in both study groups. Ionized calcium, hematocrit measured before phototherapy and peak bilirubin values were similar. The median length of hospital stay was statistically significant as 3(2-4) days in those who received continuous phototherapy and 2(2-3) days in those who received intermittent phototherapy (p=0.032). The median value of the duration of phototherapy was 12(6-15) hours in those who received continuous phototherapy and 4(2-4) hours in those who received intermittent phototherapy (p<0.001). The mean rate of bilirubin decline was 1.12±0.73 mg/dl/h in those receiving intermittent phototherapy and 0.51±0.33 mg/dl/h in those receiving continuous phototherapy (p<0.001). The mean value of the rebound rise rate was 0.08±0.28 mg/dl/h in those who received continuous phototherapy, and -0.01±0.17 mg/dl/h in those who received intermittent phototherapy (p=0.039). There was no significant difference between the two groups in the percentage of bilirubin and iCa change, and the percentage of weight change between hospitalization and discharge (p>0.05). When skin rash, diarrhea, and body temperature increases at the end of the first and second sessions were compared in both study groups, these side effects were found to be statistically less in those who received intermittent phototherapy (p<0.001). In our study, it was shown that intermittent and continuous phototherapy was not superior to each other in reducing total serum bilirubin and that patients who received intermittent phototherapy were discharged in a shorter time. Phototherapy side effects such as body temperature, skin rash and diarrhea were shown to be less present in the intermittent phototherapy group.
