Kronik böbrek yetmezliği hastalarına verilen kabul kararlılık terapisi temelli psikoeğitimin hastalık uyumu, psikolojik esneklik ve yaşam doyumları üzerine etkisi

Abstract

Bu tez çalışması kronik böbrek yetmezliği (KBY) olan hastalara verilen kabul ve kararlılık terapisi temelli psikoeğitimin, hastalık uyumu, psikolojik esneklik ve yaşam doyumları üzerindeki etkisinin belirlenmesi amacıyla yapıldı. Çalışma, öntest, sontest, izlem testli müdahale ve kontrol gruplu randomize kontrollü deneysel bir araştırma olarak gerçekleştirildi. Çalışma Haziran 2023 – Kasım 2023 tarihleri arasında Rize'de nefroloji polikliniğinde takip edilen 16 müdahale, 16 kontrol grubu olarak toplam 32 hasta birey ile gerçekleştirildi. Müdahale grubuna 8 hafta süre ile kabul ve kararlılık temelli psikoeğitim uygulanmış olup kontrol grubuna herhangi bir girişim uygulanmamıştır. Veriler, Kronik Hastalıklara Uyum Ölçeği, Kabul ve Eylem Formu II ve Yaşam Doyumu ölçekleri ile toplandı. Girişim öncesi, sonrası ve izlem ölçümleri yapıldı. Verilerin analizinde; t-testi, Mann-Whitney U testi, One-way ANOVA, Kruskal-Wallis H, Wilcoxon, ve Ki-kare testleri kullanıldı. Müdahale ve kontrol grubunun son testte KHUÖ (Kronik Hastalıklara Uyum Ölçeği), KEF-II (Kabul ve Eylem Formu II) ile Yaşam Doyumu ölçeği puanları arasında anlamlı bir fark bulunmuştur. Müdahale grubunda son testte kabul ve eylem formu II ölçeği toplam puanlarındaki düşüş, kronik hastalıklara uyum ölçeği alt boyut ve toplam puanındaki ve yaşam doyumu puanlarındaki artış anlamlı bulunmuştur (p<0,05). Sonuç olarak, Kabul ve Kararlılık Terapisi (ACT) temelli psikoeğitimin KBY hastalarında, hastalık uyumu, psikolojik esneklik ve yaşam doyumu üzerinde olumlu etkiler meydana getirdiği tespit edilmiştir. Çalışma, ACT'in kronik böbrek yetmezliğinde sağaltımı destekleyici olacağını göstermektedir.This thesis study was conducted to determine the effect of acceptance and commitment therapy-based psychoeducation on illness adjustment, psychological resilience and life satisfaction of patients with chronic renal failure (CRF). The study was conducted as a randomised controlled experimental research with pretest, posttest, intervention with follow-up test and control group. The study was registered at https://clinicaltrials.gov/ (ClinicalTrials.gov Identifier: NCT06358534). The study was carried out between June 2023 and November 2023 with a total of 32 patients (16 intervention and 16 control groups) who were followed up in the nephrology outpatient clinic in Rize between June 2023 and November 2023. Acceptance and commitment-based psychoeducation was applied to the intervention group for 8 weeks and no intervention was applied to the control group. Data were collected with the Adaptation to Chronic Diseases Scale, Acceptance and Action Form II and Satisfaction with Life scales. Pre-intervention, post-intervention and follow-up measurements were made. In the analysis of the data; t-test, Mann-Whitney U test, One-way ANOVA, Kruskal-Wallis H, Wilcoxon, and Chi-square tests were used. A significant difference was found between the post-test scores of the intervention and control groups on the Chronic Disease Adaptation Scale (CHAS), Acceptance and Action Questionnaire II (ACQ-II) and Satisfaction with Life Scale. In the intervention group, the decrease in the total scores of the acceptance and action form II scale, the increase in the sub-dimension and total scores of the adaptation to chronic diseases scale and the increase in life satisfaction scores were found to be significant (p<0.05). In conclusion, it was found that Acceptance and Commitment Therapy (ACT)-based psychoeducation had positive effects on illness adaptation, psychological flexibility and life satisfaction in CRF patients. The study suggests that ACT can be supportive of treatment in chronic renal failure.
