Kalça protezi ameliyatlarında farklı volümlerde uygulanan perikapsüler sinir grubu (PENG) blokunun postoperatif ağrı üzerine etkisi

Çalışmada kalça protezi ameliyatı sonrası farklı volümlerde uygulanan perikapsüler sinir grubu bloklarının postoperatif ağrı, motor fonksiyon, yan etkiler ve hasta memnuniyeti üzerine etkilerinin araştırılması amaçlandı. Hastalar kalça protezi cerrahisi sonrasında 50 hastaya perikapsüler sinir grubu blok %0.25 bupivakain grup I (n=25) 20 ml ve grup II (n=25) 10ml olacak şekilde ultrason yardımı ile uygulandı. Postoperatif ağrı tedavisi için morfin ile hasta kontrollğ analjezi ile sağlandı ve hastaların ağrıları aktif ve pasif olarak 15.dk, 30.dk, 1.sa, 2.sa, 4.sa, 8.sa, 16.sa 24.sa. zamanlarında NRS ile değerlendirildi ve kullanılan morfin miktarı kayıt edildi. NRS değeri 4 ve üzeri olduğunda kurtarma analjezi olarak tenoksikam 20 mg iv. uygulandı. Postoperatif bulantı ve kusması 15.dk, 30.dk, 1.sa, 2.sa, 4.sa, 8.sa, 16.sa 24.sa. zamanlarında 'Bulantı kusma tanımlayıcı skala' (0=Yok, 1=Hafif bulantı, 2=Bulantı, 3=Bir kez kusma ve 4=Birden fazla kusma) ile değerlendirilip kayıt edilirken sakalası 2 ve üzeri olan hastalara ondansetron 0.15 mg/kg intravenöz olarak uygulanmıştır. Hastaların kas gücü değerlendirildi. Ameliyattan 24 saat sonra hastaların memnuniyeti Likert Memnuniyet Ölçeği (1-5) ile değerlendirildi ve kayıt edildi. Opioid kullanımına bağlı olduğu düşünülen yan etkiler (Solunum depresyonu, sedasyon, mide bulantısı, kaşıntı ve konstipasyon gibi) kayıt edildi. Ayrıca PENG blokuna bağlı gelişen komplikasyonlar olursa kayıt edilmesi planlandı. Aynı zamanlarda heriki grup arasında NRS skorları, morfin kullanımı, ek analjezi gereksinimi açısından fark yoktu. Ayrıca hasta memnuniyeti ve bulantı-kusma bakımındanda grupların sonuçları benzerdi. Sonuç olarak çalışmamızda yakın zamanda tanımlanmış periferik blok olan ve kalça cerrahilerinde uygulanan PENG blokunun etkin ve güvenli volümünün araştırıldığı çalışmamızda 10 ml volümde uygulanan volümün 20 ml ile uygulanan blok kadar etkin bir şekilde postoperatif ağrı yönetimi gerçekleştirdiği sonucuna varılmıştır.
The aim of this study was to explore the effects of pericapsular nerve group blocks administered in different volumes on postoperative pain, motor function, side effects, and patient satisfaction after hip prosthesis surgery. Following hip prosthesis surgery, pericapsular nerve group blocks were administered to 50 patients, with Group I (n=25) receiving 20 ml of 0.25% bupivacaine and Group II (n=25) receiving 10 ml, guided by ultrasound. Postoperative pain management was achieved through patient-controlled analgesia with morphine, and patients' pain levels were assessed actively and passively at 15 min, 30 min, 1 h, 2 h, 4 h, 8 h, 16 h, and 24 h time points using the Numeric Rating Scale (NRS), with the recorded amount of morphine usage. Tenoxicam 20 mg iv. was administered as rescue analgesia when the NRS value was 4 or higher. Postoperative nausea and vomiting were evaluated and recorded using the 'Nausea Vomiting Descriptive Scale' (0=None, 1=Mild nausea, 2=Nausea, 3=One episode of vomiting, and 4=More than one episode of vomiting) at 15 min, 30 min, 1 h, 2 h, 4 h, 8 h, 16 h, and 24 h time points, with ondansetron 0.15 mg/kg intravenously administered to patients with a score of 2 or higher. Muscle strength of patients was assessed. Patient satisfaction 24 hours after surgery was evaluated and recorded using the Likert Satisfaction Scale (1-5). Side effects believed to be related to opioid use, such as respiratory depression, sedation, nausea, itching, and constipation, were recorded. Additionally, any complications related to the PENG block were planned to be recorded. There were no significant differences between the two groups in terms of NRS scores, morphine usage, and additional analgesic requirements at the specified time points. Furthermore, patient satisfaction and results regarding nausea-vomiting were similar between the groups. In conclusion, our study investigated the efficacy and safety of the recently described peripheral block, the PENG block, commonly used in hip surgeries. Our findings suggest that the application of a 10 ml volume was as effective as a 20 ml volume in postoperative pain management.
