Primer pterjium cerrahi tedavisinde konjonktival otogrefti ve primer eksizyonun karşılaştırılması

Abstract

PRİMER PTERJİUM CERRAHİ TEDAVİSİNDE KONJONKTİVAL OTOGREFT VE PRİMER EKSİZYONUN KARŞILAŞTIRILMASI?Pterjium, en çok karşılaşılan ve hakkında en çok araştırma yapılan oküler yüzey hastalıklarından biridir. Bu çalışmanın amacı, kliniğimizde primer Pterjium tanısı konulmuş hastalarda uygulanan konjonktival otogreft ve primer eksizyon cerrahi tekniklerinin postoperatif sonuçlarının nüks açısından değerlendirilmesidir. Bu amaçla, Ocak 2007- Ocak 2009 tarihleri arasında Göztepe Eğitim ve Araştırma Hastanesi Göz Hastalıkları kliniğinde pimer Pterjium tanısı almış, primer eksizyon cerrahisi ve serbest limbal konjonktival otogreft cerahisi yapılmış hastalar retrospektif olarak incelendi. Çalışmada 40'ı primer eksizyon uygulanmış, 54'ü konjonktival otogeft uygulanmış toplam 94 primer Pterjium olgusu incelendi. Primer eksizyon yapılan hastaların yaş ortalaması 46,75±9,62 otogreft yapılan hastaların yaş ortalaması 47,81±10,93 olarak saptandı. Primer Eksizyon grubunda 40 hastanın 9'unda (%22,5), otogreft grubundaki 54 hastadan 2'sinde (%3,7) nüks saptandı. PE grubundaki nüks varlığı otogreft grubundan istatistiksel olarak anlamlı derecede yüksek bulundu (p=0,005). Çalışmamızda ortaya çıkan sonuçlar doğrultusunda ve literartürde etkinliğinin kanıtlanmamış olması nedeniyle primer eksizyon tekniğini önermemekteyiz. Konjonktival otogreft tekniğinin Pterjium tedavisinde nüks riskini azaltmada etkili bir yöntem olduğunu düşünmekteyiz.

COMPARISON OF CONJUNCTIVAL AUTOGRAFT AND PRIMARY EXCISION FOR THE SURGICAL TREATMENT OF PRIMARY PTERYGIUM Pterygium is one of the most encountered and the most investigated ocular surface disorders. The purpose of this study is to evaluate the primary pterygium patients in our clinic who are operated with conjunctival autograft or primary excision technique in terms of recurrency. For this purpose, primary pterygium patients who were operated between January 2007 and January 2009 with conjunctival autograft or primary excision technique in Göztepe Training and Research Hospital Ophthalmology clinic were analyzed retrospectively. In this research 40 primary excision and 54 conjunctival autograft patients were analysed. The mean age of the primary excision group was 46, 75±9, 62 and the conjunctival autograft group's mean age was 47, 81±10, 93 respectively. The recurrence was found at 9 primary excision patients out of 40 (%22, 5) and 2 conjunctival autograft patients out of 54 (%3, 7). The recurrence rate of the PE group was higher than the conjunctival autograft group and this result was statistically significant (p=0,005). As a conclusion we do not suggest the primary excision technique for the treatment of primary ptergium. To decrese the recurrence rate we recommend the conjunctival autograft technique bacause of its lower reccurence rate.
