Genu varum hastalarında iki farklı plak ile tespit edilen proksimal tibia açık kama osteotomilerinin klinik ve radyolojik sonuçları

Abstract

Çalışma iki farklı plak ile tespit edilmiş proksimal tibia medial açık kama osteotomisi yapılan medial artrozu gelişmiş genu varum hastalarının klinik ve radyolojik sonuçlarının irdelenmesini amaçlamaktadır. Ocak 2018 ve Nisan 2024 tarihleri arasında varus osteoartrozu nedenli proksimal tibia açık kama osteotomisi uygulanan hastalar retrospektif olarak değerlendirildi. Öncesinde bağ rekonstrüksiyonu veya kırık nedeniyle cerrahi geçirmeyen, monoplanar osteotomi yapılan, kilitli tibia osteotomi plağı veya OWO (open wedge osteotomy) plak ile tespit yapılmış, değerlendirmeye uygun radyolojik verileri olan, en az 6 ay takip süresi olan ve tedaviye uyumlu hastalar çalışmaya dahil edildi. Mevcut iki plak dışında tespit materyali kullanılan, düzenli poliklinik takibi olmayan, 6 aydan daha az takibi olan ve kendi rızasıyla çalışmaya katılmak istemeyen hastalar çalışma dışı bırakıldı. İntraoperatif veriler ve poliklinik raporları kaydedilmiştir. Tespit materyaline göre hastalar; Grup A (Kilitli tibia osteotomi plağı) ve Grup B (OWO plak) olarak iki gruba ayrılmıştır. Deformitenin tayini ve hedeflenen düzeltme miktarıyla birlikte patellanın konumu ve tibial eğim preoperatif görüntüler ile kaydedildi. Postoperatif radyografik ölçümler tekrarlanmış ve klinik olarak WOMAC, MSS, VAS skorlamalarıyla birlikte kaynama durumu, eklem hareket açıklığı ve komplikasyonlar kaydedildi. The study aims to examine the clinical and radiological results of genu varum patients with medial arthrosis who underwent proximal tibial medial open wedge osteotomy fixed with two different plates. Patients who underwent proximal tibial open wedge osteotomy due to varus osteoarthritis between January 2018 and April 2024 were evaluated retrospectively. Patients who had no previous surgery due to ligament reconstruction or fracture, who underwent monoplanar osteotomy, who were fixed with a locking tibial osteotomy plate or OWO (Open Wedge Osteotomy) plate, who had radiological data suitable for evaluation, who had a minimum of 6 months of follow-up, and who were compliant with treatment were included in the study. Patients who used fixation materials other than the existing two plates, who did not have regular outpatient clinic follow-up, who had a follow-up of less than 6 months, and who did not want to participate in the study with their own consent were excluded from the study. Intraoperative data and outpatient clinic reports were recorded. According to the fixation material, patients; It was divided into two groups as Group A (Locked tibial osteotomy plate) and Group B (OWO plate). The determination of the deformity and the targeted correction amount along with the position of the patella and the tibial slope were recorded with preoperative images. Postoperative radiographic measurements were repeated and clinically WOMAC, MSS, VAS scores were recorded along with union status, joint range of motion and complications. The level of statistical significance in the study was accepted as p<0.05.
