ÖZET LAPAROSKOPİK KOLESİSTEKTOMİ CERRAHİSİNDE POSTOPERATİF AĞRI YÖNETİMİNDE EKSTERNAL OBLİK İNTERKOSTAL PLAN BLOKU, SUBKOSTAL TRANSVERSUS ABDOMİNİS PLAN BLOKU VE İKİSİNİN BERABER UYGULANMASININ ETKİLERİNİN KARŞILAŞTIRILMASI Laparoskopik kolesistektomi ameliyatından sonra hastalar orta-yüksek seviyede ağrı duyarlar. Postoperatif ağrı kontrolünde opioidler sıklıkla ve yüksek dozlarda kullanılmaktadır. Günümüzde ultrasonografinin kullanımın da yaygınlaşmasıyla rejyonel teknikler de multimodal analjezinin bir parçası olarak kullanılmaktadır. Çalışmamızda laparoskopik kolesistektomi ameliyatı geçiren hastalarda eksternal oblik interkostal plan (EOİKP) bloku, subkostal transversus abdominis plan (TAP) bloku ve ikisinin beraber uygulanmasında postoperatif analjezik etkinliklerini araştırmayı amaçladık. Çalışmaya dahil edilen 105 hasta EOİKP bloku uygulananlar (n=35), subkostal TAP bloku uygulananlar(n=35) ve iki blokun birlikte uygulandığı hastalar (n=35) olarak üç gruba ayrıldı. Ameliyat odasına alındıktan sonra tüm hastalara standart anestezi prosedürleri uygulandı. EOİKP bloku ve subkostal TAP bloku grubuna %0.25 bupivakain 30 ml ile işlem yapıldı. Her iki blokun yapıldığı gruba ise %0.25 bupivakain 15'er ml olarak ikiye bölünerek uygulandı. Operasyon bitimine yakın 1mg/kg tramadol intravenöz (i.v.) ve 1 gr parasetamol i.v. uygulandı. Postoperatif analjezi, hasta kontrollü analjezi (HKA) cihazı ile 5 mg/ml i.v. tramadol ile sağlandı. Numerik oranlama skalası (NRS)?4 olan hastalara kurtarıcı analjezik olarak 20 mg i.v. tenoksikam uygulandı. Çalışmaya dahil edilen hastaların demografik verileri, perioperatif hemodinamik parametreleri, remifentanil tüketimleri ve postoperatif ilk 24 saatlik NRS skorları, tramadol tüketimleri ve yan etkileri kaydedildi. Üç grubun tüm zaman aralıklarındaki NRS değerlerine bakıldığında sadece 30. dk istirahatteki NRS skoru subkostal TAP bloku grubunda EOİKP plan grubuna göre yüksek bulunmuştur (p=0.03). Diğer zaman aralıklarında benzerdi. Tüketilen 24 saatlik tramadol miktarlarına bakıldığında sadece 1. saatte subkostal TAP bloku grubunda EOİK plan bloku grubuna göre yüksek bulunmuştur (p=0.021). 24 saatlik toplam tüketilen opioid miktarları üç grupta da benzer bulundu. Postoperatif bulantı ve kusma varlığı açısından gruplar benzerdi. Laparoskopik kolesistektomi ameliyatı geçiren hastalarda postoperatif multimodal analjezinin bir parçası olarak EOİKP bloku ve iki blokun birlikte uygulanmasının, sadece subkostal TAP blokuna göre daha etkin ve güvenli bir yöntem olarak tercih edilebileceği kanaatindeyiz. ABSTRACT COMPARISON OF THE EFFECTS OF EXTERNAL OBLIQUE INTERCOSTAL PLANE BLOCK, SUBCOSTAL TRANSVERSUS ABDOMINIS PLANE BLOCK, AND THEIR COMBINED APPLICATION ON POSTOPERATIVE PAIN MANAGEMENT IN LAPAROSCOPIC CHOLECYSTECTOMY SURGERY Patients often experience moderate to severe pain following laparoscopic cholecystectomy. Opioids are frequently used in high doses for postoperative pain management. With the increasing use of ultrasonography, regional techniques have become a part of multimodal analgesia approaches.In our study, we compared the postoperative analgesic efficacy of the External Oblique Intercostal Plane Block (EOIPB), the subcostal Transversus Abdominis Plane (TAP) block, and the combination of both blocks in patients undergoing laparoscopic cholecystectomy. A total of 105 patients were included in the study and randomly divided into three groups; group one EOİP block (n=35),group two subcostal TAP block (n=35), and group three combination of both blocks (n=35).After being taken to the operating room, all patients underwent standard anesthesia procedures. In the EOIPB and subcostal TAP block groups, 30 mL of 0.25% bupivacaine was administered. In the group receiving both blocks, 15 mL of 0.25% bupivacaine was injected for each block, totaling 30 mL.Near the end of surgery, 1 mg/kg of tramadol and 1 gram of paracetamol were administered intravenously. Postoperative analgesia was provided via a patient-controlled analgesia (PCA) device with intravenous tramadol at a concentration of 2 mg/mL.For patients with an NRS (Numerical Rating Scale) ? 4, 20 mg of intravenous tenoxicam was administered as rescue analgesia.Demographic data, perioperative hemodynamic parameters, remifentanil consumption, postoperative NRS scores during the first 24 hours, tramadol consumption, and adverse effects were recorded for all patients included in the study. When evaluating the NRS across all time intervals for the three groups, only the 30th-minute resting NRS score was found to be significantly higher in the subcostal TAP block group compared to the EOIP block group (p = 0.03). NRS scores at all other time points were similar between the groups.Regarding the amount of tramadol consumed within the first 24 hours, only at the 1st hour, the subcostal TAP block group showed significantly higher tramadol consumption compared to the EOIP block group (p = 0.021). The total opioid consumption over 24 hours was similar across all three groups.There were no significant differences between the groups in terms of postoperative nausea and vomiting. We believe that the EOİP block alone and combined application of EOIP block with subcostal TAP block as part of postoperative multimodal analgesia in patients undergoing laparoscopic cholecystectomy can be performed as a safe and effective method compared to subcostal TAP block alone.
