Özet
Bilişsel Kopma Sendromu (BKS) zihinsel bulanıklık, hayallere dalma, yavaş hareket etme gibi belirtilerle tanımlanan ve Dikkat Eksikliği ve Hiperaktivite Bozukluğu'ndan (DEHB) bağımsız bir bozukluk olarak değerlendirilen bir dikkat sorunudur. Kendine Zarar Verme Davranışları (KZVD) ise intihar amacı olmaksızın kişinin kasıtlı olarak kendine zarar verdiği tekrarlayıcı davranışlar olarak tanımlanır ve özellikle ergenlik döneminde sık görülür. Çalışmamızda, psikiyatri yatış öyküsü bulunan 12-18 yaş arası ergen kızlarda BKS belirtileri ile KZVD arasındaki ilişkinin incelenmesi amaçlanmıştır. Bu doğrultuda, BKS belirtilerinin şiddetinin belirli türde KZVD ile olan bağlantısı, BKS'nin KZVD'nin içsel ve dışsal düzenleyici işlevlerine etkisi, psikiyatri yatış süresinin bu ilişki üzerindeki rolü değerlendirilmiş ve özbildirim ile ebeveyn raporları arasındaki farklar araştırılmıştır. Araştırma örneklemi, 30.08.2024-30.11.2024 tarihleri arasında Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesi Çocuk ve Ergen Ruh Sağlığı yataklı servisinde tedavi görmüş kız ergenler ve ebeveynlerinden oluşmaktadır. Veri toplama araçları arasında yarı yapılandırılmış tanısal klinik görüşme, sosyodemografik veri formu, Yavaş Bilişsel Tempo Ölçeği (ebeveyn ve özbildirim), Kendine Zarar Verme Davranışları Değerlendirme Envanteri ve diğer geçerli psikometrik ölçüm araçları yer almaktadır. Araştırmaya yaşları 12 ile 18 arasında değişen (Ortalama: 15,94 ± 1,35 yıl) toplam 145 kız hasta dahil edilmiştir. Çalışmada, hastalar arasında en sık görülen kendine zarar verme davranışı kesme olup, bu davranış tüm hastalarda (%100) gözlemlenmiştir. Yavaş Bilişsel Tempo Özbildirim Ölçeği (YBTÖÖ) toplam puanı ile KZVD'nin kesme (p<.01) ve saç koparma (p<.05) türleri arasında pozitif yönde anlamlı bir ilişki bulunmuştur. KZVD kesme sayısı ile YBTÖÖ toplam puanı (p<.01), GGA akran sorunları alt ölçek puanı (p<.01) ve GGA toplam güçlük puanı (p<.05) arasında da pozitif yönde anlamlı bir ilişki tespit edilmiştir. YBTÖÖ toplam puanının KZVDDE kesme sayılarını öngörüp görmediğini belirlemek için yapılan hiyerarşik regresyon analizi, iki aşamada gerçekleştirilmiştir. GGA ergen formu alt ölçek puanları kontrol değişkeni olarak eklenmiş ve model anlamlı bulunmuştur (F (5,136) = 2.887, p = .016, ?R² = .063); burada KZVDDE kesme sayılarını anlamlı şekilde öngören tek değişken GGA akran sorunları alt ölçek puanı olmuştur (p = .009). 2.aşamada YBTÖÖ toplam puanı modele eklenmiş ve modelin anlamlılığı artmıştır (F (6,135) = 3.161, p = .006, ?R² = .084). Ayrıca, YBTÖÖ toplam puanı KZVD kesme sayılarını anlamlı olarak öngörmüştür (p = .043). YBTÖÖ toplam puanı ile KZVD otonom işlevlerden affekt regülasyonu (p<.1), özkıyımı önleme (p<.05), sıkıntıyı etiketleme (p<.01), kendini cezalandırma (p<.01), çözülmeyi önleme (p<.001), sosyal işlevlerden dayanıklılık (p<.05), özerklik (p<.01) ve kendiyle ilgilenme (p<.001) alt ölçek puanları arasında pozitif yönde anlamlı bir ilişki bulunmuştur. Çalışmamız, psikiyatrik yatışı olan ergenlerde Bilişsel Kopma Sendromu (BKS) belirtilerinin Kendine Zarar Verme Davranışları (KZVD) ile anlamlı bir ilişki gösterdiğini ortaya koymuştur. BKS belirtilerinin erken tespit ve yönetiminin, KZVD'nin ve dolayısıyla intihar riskinin önlenmesinde kritik bir öneme sahip olabileceği düşünülmektedir. İlişkinin yönünü anlamak ve önleyici tedaviler geliştirmek için ileri araştırmalara ihtiyaç vardır.
Cognitive Disengagement Syndrome (CDS) is an attention-related condition characterized by symptoms such as mental fogginess, daydreaming, and slow movement. It is considered a distinct disorder, independent of Attention Deficit Hyperactivity Disorder (ADHD). Non-Suicidal Self-Injury (NSSI) refers to repetitive behaviors where individuals deliberately harm themselves without suicidal intent and is particularly prevalent during adolescence. This study investigates the relationship between CDS symptoms and NSSI in adolescent girls aged 12-18 with a history of psychiatric inpatient treatment. Specifically, the study evaluates the association between the severity of CDS symptoms and specific types of NSSI, the impact of CDS on the internal and external regulatory functions of NSSI, the role of psychiatric hospitalization duration in this relationship, and differences between self-reports and parent reports. The study sample consists of adolescent girls and their parents who received inpatient treatment at the Child and Adolescent Psychiatry Unit of Goztepe Prof. Dr. Suleyman Yalcın City Hospital between August 30, 2024, and November 30, 2024. Data collection tools included a semi-structured diagnostic clinical interview, a sociodemographic data form, the Sluggish Cognitive Tempo Scale (parent and self-report versions), the Non-Suicidal Self-Injury Assessment Inventory, and other validated psychometric measurement tools. A total of 145 female patients aged between 12 and 18 years (Mean: 15.94 ± 1.35 years) participated in the study. The most common type of non-suicidal self-injury (NSSI) observed among the patients was cutting, which was reported in all participants (100%). A positive and significant relationship was found between the total score of the Sluggish Cognitive Tempo Self-Report Scale (SCT-SR) and cutting (p <.01) and hair-pulling behaviors (p <.05). Additionally, the frequency of cutting behaviors was positively associated with the total SCT-SR score (p <.01), the peer problems subscale score of the Strengths and Difficulties Questionnaire (SDQ) (p <.01), and the total difficulties score of the SDQ (p <.05). A hierarchical regression analysis was conducted to determine whether the total SCT-SR score predicted the frequency of cutting behaviors assessed in the NSSI inventory. In the first step, SDQ adolescent form subscale scores were included as control variables, and the model was found to be significant (F (5,136) = 2.887, p = .016, ?R² = .063). Among the predictors, only the peer problems subscale score significantly predicted cutting frequency (p = .009). In the second step, the total SCT-SR score was added to the model, which improved the model's significance (F (6,135) = 3.161, p = .006, ?R² = .084). The total SCT-SR score was also found to significantly predict the frequency of cutting behaviors (p = .043). Moreover, a positive and significant relationship was observed between the total SCT-SR score and the following subscales of NSSI functions: affect regulation (p <.1), preventing suicide (p <.05), labeling distress (p <.01), self-punishment (p <.01), and preventing dissociation (p <.001). In terms of social functions, significant positive relationships were found with resilience (p <.05), autonomy (p <.01), and self-care (p<.001). Our study has demonstrated a significant relationship between Cognitive Disengagement Syndrome (CDS) symptom levels and Non-Suicidal Self-Injury (NSSI) in adolescent girls with a history of psychiatric inpatient treatment. It is suggested that the early detection and management of CDS symptoms could play a critical role in preventing NSSI and, consequently, reducing the risk of suicide. Further research is needed to better understand the direction of this relationship and to develop preventive interventions.
