Parkinson hastaları ve sağlıklı erişkinlerde hipokampüs volümlerinin MRG ile karşılaştırılması

Özet

GİRİŞ: İdiopatik Parkinson hastalığı (PH) temel olarak kardinal motor bulgularla karakterizedir, değişik derecelerde kognitif bozukluklar ve demans hastalığa eşlik edebilmektedir. Alzheimer hastalığı başta olmak üzere pek çok nörodejeneratif hastalıkta görülebilen bir bulgu olan hipokampal atrofi kognitif bozuklukla seyredebilen PH'de de görülebilir. Hipokampüs volümü Magnetik Rezonans Görüntüleme (MRG) ile değerlendirilebilmekte ve segmentasyon ile atlaslar oluşturulabilmektedir. Bu verilerin tam otomatik yazılım programları ile değerlendirilmesi nörodejeneratif hastalıklar açısından çalışmaya açık, ilgi çeken bir alan olmaya devam etmektedir. AMAÇ: Parkinson hastalarında hipokampüs segmental atrofisinin hastalığın kognitif prognozu açısından yol gösterici olabileceğini ve PH tanısı almış kişilerde kognitif bozukluk ve demans gelişimini öngörebilecek bir biyobelirteç olarak kullanılabileceğini düşündük. Bu amaçla çalışmamızda Parkinson hastaları ve kontrol grubunun hipokampüs total ve segmental volümlerini MRG ile karşılaştırdık. YÖNTEM VE GEREÇLER: Prospektif olarak yürütülen çalışmaya klinik olarak PH tanısı almış 46 Parkinson hastası ve bilinen nörolojik hastalığı ya da şikayeti bulunmayan 30 sağlıklı kontrol grubuyla başlanmıştır. MRG çekimlerini tolere edemeyen, MRG incelemesi artefaktlar nedeniyle diagnostik olmayan ya da volümetrik ölçümleri yapılamayan olgular çalışmadan dışlanmıştır. Sonuç olarak 27'si erkek (%64), 15'i kadın (%36) toplam 42 Parkinson hastası ve 17'si erkek (%63), 10'u kadın (%37) toplam 27 kontrol grubu olmak üzere toplam 69 olgu çalışmaya dahil edilmiştir. Çalışmaya katılan Parkinson hastaları normal kognitif durumda olanlar ve demans ile seyredenler olarak ikiye ayrılmıştır. Çalışmamızda hipokampüse yönelik MRG incelemede Fast Spoiled Gradient Recalled Echo (FSPGR) sekans görüntülerinden faydalanarak 'HIPpocampus subfield Segmentation' (HIPS) otomatik segmentasyon programı ile Winterburn (W) ve Kulaga – Yoskovitz (KY) protokolüne göre 5 segment ve 3 segment olarak hipokampüs segmentasyonu yapılmış ve volümleri hesaplanmıştır. İstatistiksel olarak hasta ve kontrol grubunun hipokampüs volümü karşılaştırılmasında normal dağılıma uymadıkları için Mann - Whitney U testi kullanıldı. Ayrıca hasta grubundaki total ve segmental hipokampüs volümüne demografik ve klinik özelliklerin etkisi Mann - Whitney U, Kruskal Walis ve Pearson korelasyon testi ile değerlendirildi. Tek yönlü analizde etkili olduğu bulunan faktörler için lineer regresyon metodu ile multivaryant analiz yapıldı. İstatistiksel anlamlılık düzeyi olarak p £ 0.05 alındı. BULGULAR: Normal kognitif durumda olan Parkinson hastaları (Ph) ile sağlıklı kontrol grubunun hipokampal volümleri karşılaştırıldığında; Parkinson hastalarında SR/SL/SM sağ, sol ve total volümleri ile subikulum sağ, sol ve total volümleri kontrol grubuna göre anlamlı olarak düşük bulunmuştur. Normal kognitif durumda ve demans ile seyreden Ph'nın beraber değerlendirildiği grup ile sağlıklı kontrol grubunun hipokampal volümleri karşılaştırıldığında; benzer şekilde Parkinson hastalarında SR/SL/SM sağ ve total volümleri ile subikulum sağ, sol ve total volümleri kontrol grubuna göre anlamlı olarak düşük bulunmuştur. SONUÇ: Ph ile kontrol grubu arasında kognitif durumdan bağımsız olarak subikulum ve SR/SL/SM volümleri farklı olup, belirgin radyolojik bulgusu olmayan PH'de tanıyı destekleyecek kantitatif değerler elde edilebilir. Ancak hipokampüsün segment volümleri PH'de kognitif duruma göre istatistiksel farklılık göstermemektedir. PH'de kognitif bozukluk olmadan da hipokampüs volümlerinde kontrol gruplarına göre fark olması nörodejeneratif zemine bağlanabilir. Sonuç olarak Parkinson hastalarında ilerleyen dönemlerde kognitif bozukluk - demans gelişimini öngörebilmek için takip volümetrik değerleri içeren daha geniş çalışmalara ihtiyaç duyulmaktadır.

BACKGROUND: Idiopathic Parkinson's disease (PD) is mainly characterized by cardinal motor motor symptoms, varying degrees of cognitive impairment and dementia may accompany the disease. Hippocampal atrophy, a finding that can be seen in many neurodegenerative diseases, especially Alzheimer's disease, can also be seen in PD that can progress with cognitive impairment. The hippocampus volume can be evaluated with Magnetic Resonance Imaging (MRI) and atlases can be created by segmentation. The evaluation of these data with fully automated software programs continues to be an open and interesting field in terms of neurodegenerative diseases. OBJECTIVE: We thought that segmental atrophy of the hippocampus in Parkinson's patients could guide the cognitive prognosis of the disease and could be used as a biomarker that could predict the development of cognitive impairment and dementia in people diagnosed with PD. For this purpose, we compared the total and segmental volumes of the hippocampus of Parkinson's patients and the control group with MRI in our study. METHODS: The prospective study was initiated with 46 PD patients with a clinical diagnosis of PD and 30 healthy controls without known neurological diseases or complaints. Patients who could not tolerate MRI scans, were not diagnosed due to MRI artifacts, or whose volumetric measurements could not be performed were excluded from the study. As a result, 27 of whom were male (64%), 15 of whom were female (36%) a total of 42 Parkinson's patients and 17 of whom were male (63%), 10 of whom were female (37%) a total of 27 control groups, a total of 69 patients were included in the study. Parkinson's patients participating in the study were divided into those with normal cognitive status and those with dementia. In our study, using the Fast Spoiled Gradient Recalled Echo (FSPGR) sequence images in the MRI examination of the hippocampus, hippocampus segmentation was performed in 5 segments and 3 segments according to Winterburn (W) and Kulaga - Yoskovitz (KY) protocol with the automatic segmentation program (HIPS) and their volumes were calculated. Mann-Whitney U test was used in the comparison of the hippocampus volume of the patient and control groups statistically because they did not comply with the normal distribution. In addition, the effects of demographic and clinical characteristics on the total and segmental hippocampus volume in the patient group were evaluated using the Mann - Whitney U, Kruskal Walis test and Pearson correlation test. For the factors found to be effective in one-way analysis, multivariate analysis was performed by linear regression method. p £ 0.05 was taken as the statistical significance level. RESULT: When the hippocampal volumes of Parkinson patients (Pp) with normal cognitive status and healthy control group were compared; SR / SL / SM right, left and total volumes and subiculum right, left and total volumes were found to be significantly lower in Parkinson's patients compared to the control group. When the hippocampal volumes of the group in which Pp with normal cognitive status and dementia is evaluated together and the healthy control group were compared; similarly, in Parkinson's patients, SR / SL / SM right and total volumes and subiculum right, left and total volumes were significantly lower than the control group. CONCLUSION: The subiculum and SR / SL / SM volumes differ between the Pp and the control group, regardless of cognitive status, quantitative values can be obtained to support the diagnosis in PD without obvious radiological findings. However, segment volumes of the hippocampus do not differ statistically in PD according to cognitive status. The difference in hippocampus volumes without cognitive impairment in PD compared to the control groups can be attributed to the neurodegenerative background. As a result, larger studies including follow-up volumetric values are needed to predict the development of cognitive impairment-dementia in the later stages of Parkinson's patients.
