THE EFFECT OF CORE STABILIZATION TRAINING ON PAIN, FUNCTIONAL STATUS, FATIGUE, AND QUALITY OF LIFE IN PATIENTS WITH JUVENILE SPONDYLOARTHRITIS

dc.description.abstractBackground: In Juvenile Spondyloarthritis (SpA), physiotherapy interventions play a crucial role in treatment due to spinal involvement and limited mobility. Core stabilization exercises, with their protective effect on the axial spine, may be effective in reducing pain and improving aerobic capacity and muscle strength in children with juvenile spondyloarthritis. Aim: The aim of this study is to compare the effectiveness of core stabilization training versus conventional physiotherapy methods on pain, functional status, fatigue, and quality of life in children with spondyloarthritis. Method: A total of 20 children with spondyloarthritis were included in this randomized controlled study, consisting of an intervention group (n=10; 14.60±2.41 years) and a control group (n=10; 15.80±2.53 years). Muscle strength was assessed using manual muscle testing and pain level was evaluated with the Visual Analog Scale (VAS). The Pediatric Quality of Life Inventory (PedsQL) was used to assess quality of life, and the Multidimensional Fatigue Scale (PedsQL-MFS) was employed to measure fatigue levels. Functional status was evaluated using the 6-Minute Walk Test (6MWT) and the Timed Up and Down Stairs Test (TUDS). Over a 12week period, the intervention group received core stabilization exercises, while the control group was provided only with a daily physical activity program. Results: In the intervention group, significant improvements were observed in pain scores (pre: 5.60 ± 1.71; post: 2.00 ± 1.15), 6MWT distance (pre: 364.00 ± 84.32 meters; post: 433.00 ± 81.79 meters), and TUDS time (pre: 21.00 ± 4.50 seconds; post: 16.10 ± 4.23 seconds) (p<0.05). Post-treatment muscle strength improvement was only noted in the intervention group (p<0.01). Additionally, the intervention group demonstrated significant enhancements in PedsQL-Fatigue scores (pre: 28.40 ± 17.82; post: 19.70 ± 15.78) and PedsQL-Quality of Life scores (pre: 25.10 ± 13.92; post: 15.10 ± 14.62) (p<0.05). Conclusion: The core stabilization exercise program reduces pain and fatigue levels, improves functional status, and enhances quality of life in patients with juvenile spondyloarthritis.
dc.description.abstractGiriş: Juvenil Spondiloartrit (SpA)’te omurga tutulumu ve hareket kısıtlılığı nedeniyle fizyoterapi uygulamaları tedavide büyük önem taşır. Core stabilizasyon egzersizleri, aksiyal omurgayı koruyucu etkisi nedeniyle, juvenil spondiloartritli çocuklarda ağrıyı azaltma, aerobik kapasiteyi ve kas gücünü arttırmaya yönelik olarak etkili olabilir. Amaç: Bu çalışmanın amacı, spondiloartritli çocuklarda, core stabilizasyon eğitiminin konvansiyonel fizyoterapi yöntemlerine göre ağrı, fonksiyonel durum, yorgunluk ve yaşam kalitesi üzerine etkinliğinin karşılaştırılmasıdır. Metod: Randomize kontrollü çalışmaya çalışma grubu (n=10; 14.60±2,41 yıl) ve kontrol grubu (n=10; 15,80±2,53 yıl) olmak üzere toplam 20 spondiloartritli çocuk dahil edildi. Kas kuvveti, manuel kas testi ile, ağrı seviyesi Vizuel Analog Skala (VAS) ile değerlendirildi. Yaşam kalitesi için Pediatrik Yaşam Kalitesi Envanteri (PEDSQL), yorgunluk düzeyi için ise Çok Boyutlu Yorgunluk Ölçeği (PEDSQL-MFS) kullanıldı. Fonksiyonel durum değerlendirmesi için 6 dakika yürüme testi (6DYT) ve Zamanlı Merdiven İnme-Çıkma Testi (TUDS) kullanıldı. 12 hafta boyunca çalışma grubuna core stabilizasyon egzersizleri uygulanırken, kontrol grubuna ise sadece günlük fiziksel aktivite programı oluşturuldu. Bulgular: Çalışma grubunda ağrı skorlarında azalma (pre: 5.60 ± 1.71; post: 2.00 ± 1.15), 6DYT mesafesinde artış (pre: 364.00 ± 84,32 metre; post: 433.00 ± 81,79 metre) ve TUDS süresinde iyileşme (pre: 21,00 ± 4,50 saniye; post:16,10 ± 4,23 saniye) kaydedildi (p<0.05). Tedavi sonrasında yalnızca çalışma grubunda kas kuvvetinde artış görüldü (p<0.01). Ayrıca, çalışma grubunda PedsQL-Yorgunluk (pre: 28.40 ± 17.82; post: 19.70 ± 15.78) ve PedsQL-Yaşam Kalitesi skorlarında (pre: 25.10 ± 13.92; post: 15.10 ± 14.62) iyileşmeler saptandı (p<0.05). Sonuç: Core stabilizasyon egzersiz programı, juvenil spondiloartritli hastalarda ağrı ve yorgunluk düzeyini azaltır, fonksiyonel durumu iyileştirir ve yaşam kalitesini arttırır.
