GİRİŞ VE AMAÇ: Çalışmamızda mesane tümörünün manyetik rezonans görüntüleme (MRG)'de kas invazyonu açısından duvar temas uzunluğu ve Apparent Diffusion Coefficient (ADC) parametrelerinin önemi araştırmayı hedefledik. GEREÇ VE YÖNTEM: İstanbul Medeniyet Üniversitesi Göztepe Eğitim ve Araştırma Hastanesi'nde Ocak 2014-Aralık 2019 tarihleri arasında T2 ağırlıklı ve diffüzyon sekansını içeren alt batın MRG'leri olan, patolojik olarak mesane tümör tanısı doğrulanmış 53 olgunun 69 tümörü çalışmaya dahil edildi. İnvazyon, TNM sınıflandırma sistemine göre yapıldı. İki gözlemci MRG taramalarını bağımsız olarak analiz etti. Çalışmamızda aksiyal ve sagital T2 ağırlıklı MR sekanslarında mesane tümörünün duvardaki en büyük temas uzunluğu ve tümörün duvardan lümene doğru olan en büyük dik uzaklığı ölçüldü. Ayrıca ADC sekansında mesanenin normal duvarından, tümörün içinden ve duvarın tümör istilasına uğrayan kısmından ROI (Region of interest) değerleri ölçüldü. ADC ölçümleri üç kez tekrarlanmış ve ortalama ADC değerleri kaydedilmiştir. Musküller tabaka invazyonu bulunan 17 olgu ile invazyon bulunmayan 52 olgu arasında bu değerler arasındaki fark istatistiksel olarak hesaplanmıştır. İstatistiksel analiz SPSS 21.0 yazılımı ile yapılmış, normal dağılım gösteren sayısal verileri arasındaki fark bağımsız örnekler t testi, normal dağılım göstermeyen sayısal verilerin karşılaştırılmasında ise Man Whitney U testi kullanılmıştır. Gözlemciler arası uyumu değerlendirmek için sınıf içi korelasyon katsayısı (ICC) kullanıldı. Anlamlı sonuç veren sayısal verilerde optimal kestirim değerleri ve sensitivite, spesifite ve eğri altında kalan alan hesaplamalarında ROC analizi kullanılmıştır. BULGULAR: Çalışmaya 53 olguya ait toplam 69 lezyon dahil edilmiştir. Dahil edilen 53 olgudan 2'si kadın (%3,8), 51'si (%96,2) erkektir. Ortalama yaş 66.42 ±1.85, minimum yaş 53, maksimum yaş ise 89'tür. Lezyonların 50'si high grade (%72,5), 19'u (%27,5) low grade tümör olarak sınıflandırılmıştır. 17 lezyonda duvar invazyonu saptanmış iken (%24,6), 52 lezyonda (%75,4) invazyon saptanmamıştır. Lezyonun maksimum tümör-duvar temas uzunluğu ve lezyon derinliği, gözlemci 1 ve gözlemci 2 için invazyon bulunan ve bulunmayan olgular arasında istatistiksel olarak anlamlı bulunmuştur (sırası ile p<0,0001, p<0,0001 ve p=0,001, p<0,0001). İntratümöral ADC ortalaması, intratümöral ADC ortalamasının normal duvar ADC ortalamasına oranı, tümör izlenen mesane duvarı ADC ortalamasının normal mesane duvarı ADC ortalamasına oranı muskuler tabaka invazyonunun tespitinde istatistiksel olarak gözlemci 1 ve gözlemci 2 için anlamlı bulunmuştur (Sırasıyla; p=0,003, p<0,0001, p=0,001 ve p=0,001, p<0,0001, p=0,002 ). Tümör saptanan mesane duvarının ortalama ADC değeri muskuler tabaka invazyonu tespitinde istatistiksel olarak gözlemci 1 için anlamlı olup gözlemci 2 için anlamlı sonuç vermemiştir (Sırasıyla; p=0,011 ve p=0,072). Maksimum tümör-duvar temas uzunluğu ve lezyon derinliğinin değerlendirilmesinde gözlemciler arası ölçümlerin uyumunun mükemmel olduğu gözlenmiştir (sırasıyla, ICC = 0.985 and 0.988). Tümöre komşu mesane duvar ADC ortalaması ve tümöre komşu mesane duvarı ADC/normal mesane duvarı ADC oranında gözlemciler arasında iyi derecede uyum saptanmıştır (sırasıyla, ICC = 0.781 and 0.662). İntratümöral ADC ortalaması, normal mesane duvarı ADC ortalaması, intratümöral/normal mesane duvarı ADC oranlarında ise gözlemciler arasında orta derece uyum mevcuttur (sırasıyla, ICC=0.45, 0.443 and 0.516). SONUÇ: Çalışmamızda tümörün mesane duvar temas uzunluğunun ve ADC parametrelerinin MRG'de ölçümleri, kas invazyonunu öngörmede belirgin fayda sağlayabilir ancak daha büyük hasta gruplarında ve daha fazla çalışma ile doğrulanması gerekir.
INTRODUCTION: Tumor contact length (TCL) with the bladder wall and ADC (Apparent diffusion coefficient) parameters have been considered useful to predict muscle invasion of bladder carcinoma (BC) on 1.5 Tesla magnetic resonance imaging (MRI). PURPOSE: To investigate the importance of values of TCL and ADC parameters for prediction of invasion of BC on MRI. Materials and Methods: This study was included 53 patients with 69 pathologically confirmed tumor lesions. Invasion was performed according to TNM classification system. The aggressiveness of bladder cancer pathologically was classified as low and high-aggressiveness. Two observers independently analysed MRI scans. The longest TCL and the longest depth (D) of the tumor was measured on the axial and sagittal T2 weighted MRI. Additionally ADC values of the normal bladder wall, intratumoral and the tumor invaded bladder wall were measured with a proper ROI. The intra-class correlation coefficient (ICC) was used to determine the inter-observer agreement. RESULTS: The TCL and D of lesion were statistically significant between the patients with invasion and non-invasion groups for observer 1 and observer 2 (p <0.0001, p <0.0001 and p = 0.001, p <0, 0001 respectively). The mean value of intratumoral ADC, the ratio of the mean value of intratumoral ADC to the normal bladder wall ADC mean value, the ratio of the tumor-invaded bladder wall ADC mean value to the normal bladder wall ADC mean value were statistically significant for observer 1 and observer 2 in the determination of muscular invasion (Respectively; p = 0.003, p <0, 0001, p = 0.001 and p = 0.001, p <0.0001, p = 0.002). The mean ADC value of the tumor-invaded bladder wall was statistically significant for the observation of muscular invasion and was not statistically significant for observer 1 (p = 0.011 and p = 0.072, respectively). The inter-observer agreement was excellent in the tumor-wall contact length and lesion depth (ICC = 0.985 and 0.988, respectively). A good agreement was found between the observers at the mean ADC value of the tumor-invaded bladder and the ratio of the tumor-invaded bladder wall ADC mean to the normal bladder wall ADC (ICC = 0.781 and 0.662, respectively).A fair agreement was found between the observers in the mean value of intratumoral ADC, normal bladder wall ADC, and intratumoral/normal bladder ADC (ICC=0.45, 0.443 and 0.516, respectively). CONCLUSIONS: MRI measurements of ADC parameters and TCL have good potential to assess invasion but should be confirmed these findings in larger groups of patients.
