PLATELET-LYMPHOCYTE RATIO AS AN INFLAMMATORY MARKER MAY BE SUPERIOR TO C-REACTIVE PROTEIN FOR PREDICTING THE OCCURRENCE OF ACUTE CHOLECYSTITIS

Introduction: Platelet-lymphocyte ratio (PLR), neutrophil-lymphocyte ratio (NLR), mean platelet volume (MPV) and red-cell distribution width (RDW) have recently been evaluated as the new and simple inflammatory markers in systemic inflammation. In this study, we aimed to investigate whether PLR, NLR, MPV, RDW, and C-Reactive Protein (CRP) levels can help to diagnose the acute, subacute and chronic cholecystitis. Case presentation: A total of 96 patients who underwent cholecystectomy (20 acute, 39 subacute, and 37 chronic cholecystitis) and 30 healthy volunteers have enrolled into the study. The patients were selected retrospectively from the clinic medical records, and the controls were selected prospectively among the patients' escorts who had similar age and gender. All the patients' laboratory findings were obtained from the medical records. All statistical analyses were performed using SPSS for Windows, version 17.0. PLR was found to be higher in patients with acute cholecystitis as compared to controls (p<0.005), and NLR was found to be higher in acute group and chronic group than the control group (p<0.00.5). CRP wasn't significantly higher in patients compared to control group. Using the cut-off value of 130 for PLR, there was sensitivity at 82% and specificity at 63% for predicting acute cholecystitis. Conclusions: PLR may be a better predictor than CRP and NLR for diagnosis of acute cholecystitis, and it is also positively correlated with other inflammatory markers such as CRP and the NLR.
