Mikrosatellit instabilite varlığının sağ taraflı kolon kanserlerinde sağkalım üzerine etkisi.

Çalışmamızın birinci amacı sağ kolon kanserlerinde Mikrosatellit instabilite varlığının (MSİ-H) genel sağkalım ve hastalıksız sağkalım üzerine olan etkilerini ortaya koymaktır. İkincilamacımız ise; MSİ-H veMikrosatellit stabil MSS sağ kolon kanserlerinin demografik, laboratuvar ve patolojik özelliklerini karşılaştırmaktır. 1 ocak 2016 ile 1 ocak 2024 tarihleri arasında sağ kolon tümörü nedeniyle ameliyat edilen ve patoloji sonucu kolonik adenokarsinom olarak raporlanan hastalar retrospektif olarak incelendi. Hastalar MSİ-H ve MSS olarak iki gruba ayrıldı. Hastaların hastane sistemi üzerinden; ameliyat notları, pataloji ve laboratuvar bulguları, postoperatif 3. ay, 6. ay, 12. ay, 18.ay ve 24. ay takip bulguları kayıt altına alındı. Ayrıca tüm hasta grubunda hastalıksız ve genel sağkalıma etki eden faktörler araştırıldı. Toplam 100 hastanın66'sı (%66) MSS, 34'ü (%34) MSİ-Hgrubundaydı. Hastaların yaş, cinsiyet, VKİ, preoperatif CEA seviyeleri açısından anlamlı farklılık izlenmedi. MSİ-H grubundaki hastaların MSS grubuna göre daha büyük tümör çapına sahip olduğu, tümörlerin daha yüksek oranda müsinözdiferansiasyon gösterdiği ve daha fazla kötü diferensiye olduğu gözlendi. MSİ-H grubundaki tümörlerin daha ileri evrede olduğu görüldü. Lokal nüks ve yeni gelişen uzak metastaz açısından iki grup arasında anlamlı farklılık görülmedi. Tüm grupta ortalama 96aylık takipte, genel sağkalım süresi ortalama 37.5 (12; 60) ay, hastalıksız sağkalım süresi ise 36 (9; 60) ay olarak bulundu, ancak MSİ-H ve MSS grupları arasında anlamlı fark yoktu. MSİ-H varlığının sağ kolon kanserlerinde prognoza etkisi görülmemiştir.
The primary objective of our study is to elucidate the effects of microsatellite instability-high (MSI-H) on overall survival and disease-free survival in right-sided colon cancers. Our secondary objective is to compare the demographic, laboratory, and pathological characteristics of MSI-H and Microsatellite Stable (MSS) right colon tumors. Patients who underwent surgery for right colon tumors and were pathologically diagnosed with colonic adenocarcinoma between January 1, 2016, and January 1, 2024, were retrospectively reviewed. Patients were divided into two groups: MSI-H and MSS. Data including surgical notes, pathology and laboratory findings, and follow-up records at postoperative 3, 6, 12, 18, and 24 months were collected through the hospital system. Factors influencing disease-free and overall survival were also investigated in the entire patient cohort. Out of 100 patients, 66 (66%) were in the MSS group and 34 (34%) were in the MSI-H group. There were no significant differences observed in terms of age, gender, BMI, or preoperative CEA levels between the two groups. Patients in the MSI-H group had larger tumor sizes, a higher proportion of mucinous differentiation, and a greater degree of poor differentiation compared to the MSS group. Tumors in the MSI-H group were found to be at a more advanced stage. There was no significant difference observed between the two groups in terms of local recurrence and new distant metastases. The mean overall survival was 37.5 (12-60) months and the mean disease-free survival was 36 (9-60) months in the entire cohort over a mean follow-up period of 96 months, with no significant difference between the MSI-H and MSS groups. The presence of MSI did not impact the prognosis of right colon cancers.
