Multiple sklerozisli hastalarda dolaşımdaki GAS6 protein düzeyi

Özet

Bu çalışmada multipl skleroz (MS) hastalarında GrowthArrest-Specific Protein 6 (GAS6) düzeylerinin inflamasyon (yüksek duyarlıklı C-reaktif protein, hsCRP), oksidatif stres (ileri oksidatif protein ürünleri, AOPP) ve total antioksidan kapasite (FRAP) ile ilişkisi araştırılmış; GAS6'nın özellikle hastalığın erken evresinde immün düzenleyici bir yanıt olup olmadığı değerlendirilmiştir.Çalışmaya yeni tanı almış MS hastaları (n=20), tedavi altında olan MS hastaları (n=30) ve sağlıklı kontroller (n=29) dahil edildi. Serum GAS6 düzeyleri ELISA, hsCRP, AOPP ve FRAP düzeyleri spektrofotometrik yöntemlerle ölçüldü. Multiple skleroz tanısı, 2017 revizyonlu McDonald kriterlerine göre konuldu. Hastalık şiddeti, Expanded Disability Status Scale (EDSS) ile belirlendi. MS grubunda GAS6, AOPP, FRAP düzeyleri kontrol grubuna kıyasla anlamlı yüksek bulundu (p<0,05). Yeni tanı alan grupta hsCRP ile GAS6 arasında orta düzeyde pozitif korelasyon (r=0,527*,(p=0,017)), GAS6 ile AOPP arasında zayıf derecede negatif korelasyon (r=-0,365*,(p=0,125)) saptandı. Yeni tanı alan hastalarda AOPP düzeyleri kontrol grubuna kıyasla anlamlı yüksek bulundu (p<0,05). Tedavi altındaki grupta FRAP düzeyleri kontrol grubuna kıyasla anlamlı düşük bulundu (p<0,05) ve FRAP ile AOPP arasında negatif korelasyon (r=-0,353*,(p=0,060)) tespit edildi.Tedavi altındaki grupta GAS6 ile FRAP arasında negatif korelasyon (r=-0,314, (p=0,111)), atak sayısı ile AOPP arasında pozitif korelasyon (r=0,355*,(p=0,054)), belirlendi. MS hastalarında erken evrede GAS6, inflamatuvar yük artışıyla birlikte oksidatif stresi sınırlayan kompansatuvar bir yanıt gösterebilirken; kronik evrede artan oksidatif yük ve azalan antioksidan rezervlerle birlikte progresyonun biyobelirteci haline gelebilir. In this study, therelationship between Growth Arrest-Specific Protein 6 (GAS6) levels and inflammation (high-sensitivity C-reactive protein, hsCRP), oxidative stress (advanced oxidation protein products, AOPP), and total antioxidant capacity (FRAP) was investigated in patients with multiple sclerosis (MS); and whether GAS6 represents an immunoregulatory response, particularly in the early stage of the disease, was evaluated. The study included newly diagnosed MS patients (n=20), MS patients under treatment (n=30), and healthy controls (n=29). Serum GAS6 levels were measured using ELISA, while hsCRP, AOPP, and FRAP levels were determined by spectrophotometric methods. The diagnosis of multiple sclerosis was established according to the 2017 revised McDonald criteria. Disease severity was assessed using the Expanded Disability Status Scale (EDSS). GAS6, AOPP, FRAP levels in the MS group were found to be significantly higher compared to the control group (p<0.05). In the newly diagnosed group, a mode rate positive correlation was found between hsCRP and GAS6 (r=0.527, p=0.017), and a weak negative correlation between GAS6 and AOPP (r=-0.365, p=0.125). AOPP levels in newly diagnosed patients were significantly higher than in the control group (p<0.05). In the treatment group, FRAP levels were significantly lower than in controls (p<0.05), and a negative correlation was observed between FRAP and AOPP (r=-0.353, p=0.060). In the treatment group, a negative correlation between GAS6 and FRAP (r=-0.314, p=0.111) and a positive correlation between the number of relapses and AOPP (r=0.355, p=0.054) were identified. In MS patients, GAS6 may represent a compensatory response in the early stage, limiting oxidative stress in paralel with the increase in inflammatory load; whereas in the chronic stage, it may become a biomarker of progression due to increased oxidative load and decreased antioxidant reserves.
