Nazal septum perforasyonunda trombositten zengin fibrinin (TZF) ve hyalüronik asidin (HA) etkinliğinin araştırılması (deneysel çalışma)

Özet
Amaç: Nazal septum perforasyonlarının (NSP) tedavisi için konservatif yaklaşım veya cerrahi yöntemler denenmiş ancak tatmin edici bir yöntem henüz tanımlanamamıştır. Trombositten zengin fibrinde (TZF) bulunan trombositlerin ?-granüllerinde bol miktarda büyüme faktörleri bulunmaktadır. Kondrosit proliferasyonu üzerine büyüme hormonlarının etkilerini araştıran çalışmalar vardır ancak NSP iyileşmesi ile ilgili çalışma bulunmamaktadır. Hyalüronik asit (HA) karakteristik yapısal özellikleri sayesinde eklendiği solüsyonlara yüksek viskoelastikiyet ve su tutma kapasitesi sağlamakta ve doku rejenerasyonunda yaygın olarak kullanılmaktadır. Timpanik membran perforasyonlarının onarımında başarılı olduğu tespit edilen HA'nın NSP üzerine etkinliğinin gösterildiği bir çalışma yoktur. Çalışmamızda TZF'nin ve HA'nın tavşan septumunda deneysel olarak oluşturulan septum perforasyonlarının onarımındaki etkisinin araştırılması amaçlanmıştır. Yöntem: Çalışmaya toplam 24 adet beyaz Yeni Zelanda tavşanı dahil edildi. Tavşanlar 4 eşit gruba ayrıldı. Her tavşan anestezi altında transkolumellar yaklaşım uygulanarak opere edildi. Nazal septumlarında mukoperikondriumlar ve septal kartilajı da içerecek şekilde 0,7 cm x 0,7'lik perforasyonlar oluşturuldu. 1. grup olan kontrol grubuna ek müdahale yapılmadı. 2. 3. ve 4. gruba NSP bölgesine sırası ile HA, TZF ve HA+TZF kullanıldı ve transkolumellar insizyonlar kapatıldı. TZF, tavşanlardan alınan 8 cc kan 2000 devirde 15 dk santrifüj edilerek elde edildi. HA olarak ise %100 saf HA içeren MeroGel® kullanıldı. 40 gün boyunca takip edilen tavşanların nazal septumları çıkartıldı. Makroskopik ve histopatolojik olarak (perforasyon çapı, yabancı cisim reaksiyonu, inflamasyon derecesi, granülasyon, epitelizasyon ve fibrozis) değerlendirildi. Bulgular: Tavşanlarda herhangi bir komplikasyon gelişmedi. 1. grupta hiçbir tavşanın NSP'nda kapanma gözlenmedi. İkinci grupta 3 tavşanın (%50) NSP'nda tam kapanma izlendi. Üçüncü grupta 3 tavşanın (%50) NSP'u tamamen kapandı. Dördüncü grupta ise 4 tavşanın (%66,6) NSP'nda tam kapanma izlendi. Gruplar arasında perforasyonların tamamen kapanmasına yönelik yapılan istatistiksel değerlendirme anlamsız bulunmakla birlikte 2., 3. ve 4. gruplarda NSP'nda izlenen perforasyon alanlarında %50'den fazla küçülme bulguları istatiksel olarak anlamlı bulundu (p<0.05). Histolojik olarak yapılan incelemelerde yabancı cisim reaksiyonu ve fibrosiz açısından kontrol grubuna göre diğer tüm gruplarda istatiksel olarak anlamlı fark izlendi (p<0.05). Granülasyon dokusu, epitelizasyon ve inflamasyon derecesi açısından ise gruplar arasından anlamlı fark izlenmedi (p> 0.05). Sonuç: NSP oluşturulan tavşanlarda TZF ve/veya HA ile makroskopik anlamlı kapanma sağlanmış ve bu materyallerin kullanıldığı tüm septum perforasyonlarında, perforasyon alanında %50'den fazla, en az yarısında ise tam kapanma izlenmiştir. Perforasyonların tamamen kapanması sonuçlarının istatistiksel anlamlı bulunmaması, yerel etik kurulun izin verdiği denek sayısı nedeniyle gruplardaki tavşan sayısının yetersizliğine bağlanabilir. Daha fazla denek sayısı ile yapılacak çalışmalar konuya açıklık getirecektir. Kolay elde edilebilir ve basitçe uygulanabilir olan bu materyaller septum perforasyonlarının kapanmasında veya küçültülmesinde yararlı olabilir.
Objectives: Conservative approach or surgical methods have been tried for the treatment of nasal septum perforations (NSP), but a satisfactory method has not been defined yet. Platelets in the platelet-rich fibrin (PRF) have abundant growth factors in the ?-granules. Although there are many studies describing the effects of growth hormones on chondrocyte proliferation, there are no studies on NSP recovery. Hyaluronic acid (HA) provides high viscoelasticity and water retention capacity to the solutions it is added thanks to its characteristic structural properties and is widely used in tissue regeneration. There is no study showing the effectiveness of HA, which was found to be successful in repairing tympanic membrane perforations, on NSP. In this study, it was aimed to investigate the effect of PRF and HA on the repair of septal perforations which were experimentally formed in the rabbit septum. Methods: A total of 24 white New Zealand rabbits were included in the study. Rabbits were divided into 4 equal groups. Each rabbit was operated under general anesthesia with a transcolumellar approach. 0,7 cm perforations including mucoperichondrial and septal cartilage were created in the nasal septum. No additional intervention was made to the control group, which was the first group. Septal perforations were closed to groups 3 and 4 using HA, PRF and HA + PRF, respectively. PRF was obtained by centrifuging 8 cc of blood taken from rabbits at 2000 rpm for 15 minutes. MeroGel ® containing 100% pure HA was used as HA. Nasal septum were removed from the rabbits followed for 40 days. Macroscopic closure rate and histopathologically foreign body reaction, degree of inflammation, granulation, epithelization and fibrosis were evaluated. Results: No complications developed in rabbits in the 40-day period. In the 1st group, no rabbits were closed in NSP. In the second group, complete closure was observed in NSP of 3 rabbits (50%). In the third group, NSP of 3 rabbits (50%) was completely closed. In the fourth group, complete closure was observed in NSP of 4 rabbits (66.6%). The statistical evaluation for the complete closure of perforations between the groups was found to be insignificant, but the findings of shrinkage of more than 50% in the perforated areas observed in the NSP in the 2nd, 3rd and 4th groups were found statistically significant (p <0.05). In histological examinations, a statistically significant difference was observed in all other groups in terms of foreign body reaction, epithelialization and fibrosis, compared to the control group (p <0.05). There was no significant difference between the groups in terms of granulation tissue and degree of inflammation (p> 0.05). Conclusion: PRF and HA macroscopic closure was achieved in rabbits with NSP and more than 50% closure was observed in the perforation area in all septum perforations using these materials. While the complete closure of perforations was detected in at least half of the rabbits in all study groups, it could be attributed to the insufficient number of rabbits in the groups due to the number of subjects allowed by the local ethics committee. Studies with more number of animals will clarify the issue. These easily available and simply applicable materials can be useful in closing or shrinking septum perforations.
