Antidepresan kullanan hastaların tanıları ve kaçıncı kez antidepresana başladıklarının araştırılması

Özet

Amaç: Uygun ya da yeterli olmayan antidepresan kullanımı ruhsal bozukluğun uzun sürmesine ya da tekrarlamasına yol açmaktadır. Bu çalışmada uzun süreli ya da rekürren olarak antidepresan kullanmakta olan hastalarda tedavi etkinliğinin araştırılması ve ruhsal bozukluk ve antidepresan kulllanım öyküleri ile çeşitli faktörlerin tedavi etkinliği üzerine etkisinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Hastanemiz psikiyatri kliniğinde depresyon tanısıyla takip edilen toplam 131 hastadan demografik bilgileri, psikiyatrik bozukluk ve antidepresan kullanım öyküleri ve mevcut antidepresan kullanımları ile ilgili toplam 12 soru içeren anket doldurmaları istendi. Bulgular: Katılımcıların ortalama yaşı 46,7±15 idi. Ankete katılanların 40'ı (%30,5) erkek, 91'i (%69,5) kadın idi. Önceki tanıların %40,3'ünü depresyon, yine %40,3'ünü anksiyete oluşturuyordu. Ek psikoterapi almış olanların oranı %11,5'ti. Katılımcıların %48,9'unda 24 ay veya daha uzun süre antidepresan kullanma öyküsü vardı. Katılımcıların %18,3'ünde kronik, %42,7'sinde rekürren antidepresan kullanma durumu bulunmaktaydı. Hekimin tanıyı söyleme ve/veya hastaların hangi hastalık kaynaklı ilaç kullandıklarını bilmeleri %55.1 bulundu. Çalışmaya alınan katılımcıların ortaokul mezunu olanlarda iki veya daha fazlakez antidepresan kullanmış olanların oranı diğer eğitim durumu gruplarına göre anlamlı yüksek bulundu (p=0,015). Şu anda evli olanlarda iki veya daha fazla kez antidepresan kullanmış olanların oranı bekar ya da ayrılmış olanlara göre anlamlı yüksek bulundu (p=0,017). Önceden 24 ay veya daha uzun süre antidepresn kullananlarda 24 ay veya daha uzun süredir antidepresan kullanıyor olanların oranı 24 aydan daha kısa süre antidepresan kullanmış olanlara göre anlamlı yüksekti (p<0,001). Üniversite mezunu olanlarda ilk kez antidepresan kullananların oranı daha alt düzeyde eğitim görmüş olanlara göre anlamlı yüksekti (p=0,003). Sonuç: Çalışmamızdan elde edilen bulgular eğitim durumu ve hekimin hastaya tanıyı söylemiş olma gibi faktörlerin rekürren antidepresan kullanımı ile ilişkili bulunduğunu, bu durumun da hastanın hastalığı hakkındaki farkındalığının tedavi sonucuna etkili olduğunu düşündürmektedir. Sonuç olarak ruhsal bozukluğu olan hastalarda büyük olasılıkla farkındalık ya da bilgi eksikliği nedeniyle hastanın tedaviye uyumsuzluğu ya da tedavi dozunun uygun şekilde yapılmamış olması gibi faktörler antidepresan tedavisi konusunda yetersizliğe yol açmaktadır. Bu yetersizliğin sonucu olarak maliyet kayıpları iş gücü kayıpları ve en önemlisi kişilerin işlevsellik ve üretkenlik kayıpları artmakta, relapslar, rekkürrensler ve sonuç olarak kronikleşmiş durumlar ortaya çıkmakta ve bu kronikleşmiş durumlar ko-morbid rahatsızlıkların artmasına sebep olmaktadır. Sonuç olarak tedavilerin gidişatı açısından sistemsel olarak bir konsensusa varılıp tüm antidepresan kullanan hastaların tanılarının netleştirilmesi ko-morbid rahatsızlıklarla birlikte risk faktörlerinin hesaplanıp rekkürens ve kronik hastaların ayrıştırılıp ortak bir algoritma eşliğinde tedavilerinin tekrardan revize edilmesi gerektiği görülmektedir.

Objective: The use of appropriate or insufficient antidepressants causes the mental disorder to last for a long time or to recur. In the present study, it was aimed to investigate the efficacy of treatment in patients using antidepressants for a long time or recurrently, and to evaluate the effects of mental disorders and antidepressant use histories and various factors on the treatment efficiency. Method: A total of 131 patients followed up in the psychiatry clinic of our hospital with a diagnosis of depression were asked to fill a questionnaire containing a total of 12 questions about their demographic information, history of psychiatric disorders and antidepressant use, and current antidepressant use. Results: The mean age of the participants was 46.7 ± 15 years. A total of the 40 (30.5%) of the participants were male, 91 (69.5%) were female. A total of 40.3% of the previous diagnoses were depression, and 40.3% were anxiety. The rate of those who received additional psychotherapy was 11.5%. A total of 48.9% of the participants had a history of using antidepressants for 24 months or longer. A total of 18.3% of the participants had chronic and 42.7% had recurrent use of antidepressants. It was found that 55.1% of the physicians did not tell the diagnosis and / or the patients knew which disease they used. Among the participants included in the study, among those who graduated from secondary school, the proportion of those who used two or more antidepressants was found to be significantly higher than the other education groups (p = 0.015). The rate of those who have used antidepressants two or more times was found to be significantly higher in those who are currently married than those who were single or separated (p=0.017). The rate of those who currently use antidepressants for 24 months or longer was significantly higher in those who had a history of antidepressant use for 24 months or longer than those Abstract viii who used antidepressants for shorter than 24 months (p<0.001). The rate of first-time use of antidepressants among university graduates was significantly higher than those with lower education (p=0.003). Conclusion: The findings of our study suggest that factors such as educational status and the physician's notification of the diagnosis to the patient are associated with the use of recurrent antidepressants, and this situation, in turn, suggests that the patient's awareness of the disease affects the outcome of the treatment. In conclusion, in patients with mental disorders, factors such as the patient's non-compliance with treatment or not administering the treatment dose appropriately due to lack of awareness or knowledge cause inadequate antidepressant treatment. As a result of this insufficiency, cost losses, loss of workforce and most importantly, loss of functionality and productivity of individuals increase, relapses, recurrence and consequently chronic conditions occur and these chronic conditions cause an increase in co-morbid disorders. As a result, it is seen that a systematic consensus should be reached in terms of the course of the treatments, clarifying the diagnoses of all patients using antidepressants, calculating risk factors with co-morbid diseases, separating recurrent and chronic patients, and revising their treatment with a common algorithm
