HIV ile yaşayan insanlarda intiharın damgalanma, aidiyet ve diğer psikolojik faktörlerle ilişkisi

dc.description.abstractAmaç: HIV ile yaşayan insanlarda (HİYİ) önlenebilir ölüm nedenlerinden olan intihar, genel popülasyona kıyasla belirgin olarak fazladır. İntihar davranışını anlamakla ilgili çeşitli teoriler öne sürülmüştür. Bunlardan biri kişiler arası intihar teorisi ve bu teoriye göre engellenmiş aidiyet ve başkalarına yük olma algısı artmış intihar riskiyle ilişkilidir. Bu çalışmada HIV ile yaşayan insanlarda depresyon, anksiyete, aidiyet, reddedilme ve damgalanmanın intihar ile ilişkisi araştırılmıştır. Yöntem: Çalışma kesitsel ve tanımlayıcı özellikte tasarlanmıştır. 85 HIV ve 75 Hepatit B enfeksiyonu ile yaşayan insanın katılımı ile gerçekleştirilmiştir. Çalışma grubunu HİYİ'ler, kontrol grubunu ise Hepatit B ile yaşayan insanlar oluşturmaktadır. Katılımcıların HİYİ grubunu oluşturan kişilere sivil toplum kuruluşu sosyal medya hesaplarından; Hepatit B grubuna ise gastroenteroloji kliniğinden ulaşılmıştır. Katılımcılara sosyodemografik form, I?ntihar Davranış O?lçeg?i, Hastane Anksiyete ve Depresyon O?lçeg?i, I?çselleştirilmiş AIDS I?lişkili Damgalama O?lçeg?i, Kronik Hastalıklarda Beklenen Stigma O?lçeg?i, Kişilerarası I?htiyaçlar Anketi, Genel Aidiyet O?lçeg?i, Modifiye Morisky O?lçeg?i uygulanmıştır. Bulgular: HİYİ'lerin yaş ortalamasının 33,63 ± 9,81 olduğu ve büyük çoğunluğunun erkek katılımcılardan (%79,8) oluştuğu gözlenmiştir. HI?YI?'lerin kontrol grubuna go?re, damgalanma, başkalarına yük olma algısı, engellenmiş aidiyet duygusu, reddedilme, anksiyete, depresyon, intihar davranışının ciddiyeti, tedaviye yo?nelik motivasyon ve bilgi düzeylerinin daha yüksek oldug?u (sırasıyla, p<0,001, p<0,001, p=0,002, p<0,001,p=0,014, p=0,009, p<0,001, p<0,001, p<0,001); kabul edilme ve genel aidiyet duygusunun daha düşük oldug?u go?zlenmiştir (sırasıyla, p=0,008, p<0,001). HİYİ'lerde intihar davranışının ciddiyetinin damgalanma, reddedilme, başkalarına yük olma algısı, engellenmiş aidiyet, anksiyete ve depresyon artışı ile arttığı (sırasıyla, p=0,003, p<0,001, p<0,001, p<0,001, p<0,001, p<0,001); kabul edilme ve genel aidiyet duygusunda artış ile azaldığı saptanmıştır (sırasıyla, p<0,001, p<0,001). HİYİ'lerde damgalanma miktarının artması ile depresyonun, anksiyetenin, intihar davranışının ciddiyetinin, başkalarına yük olma algısının, engellenmiş aidiyetin ve reddedilmenin arttıg?ı (sırasıyla, p=0,001, p=0,004, p=0,003, p<0,001, p<0,001, p<0,001); kabul edilme ve genel aidiyet duygusunun azaldıg?ı go?zlenmiştir (sırasıyla, p<0,001, p<0,001). Enfekte olma süreleri incelendig?inde, HI?YI?'lerde enfekte olma süresi azaldıkça intihar davranışının ciddiyetinde artış go?rülmüştür (p= 0,046). Son olarak, reddedilmenin HI?YI?'lerde damgalanma ve intihar davranışının ciddiyetini yordadıg?ı bulunmuştur (sırasıyla, p=0,007, p=0,01). Sonuç: HİYİ'lerin artmış olan depresyon, anksiyete ve intihar riski hastalığın her evresininde göz önünde bulundurulmalı ve multidisipliner bir yaklaşımla değerlendirilmelidir. İntihar ve damgalanma ile ilişkili bulunan reddedilmeyi azaltmak için, hasta ile temastaki tüm sağlık çalışanlarının eğitilmesi; sağlık politikaları, yasalar oluşturulması; çeşitli damgalayıcı ve marjinalleştirici konularda toplumun bilgilendirilmesi önerilir.
dc.description.abstractObjective: Suicide, a preventable cause of death in people living with HIV (PLWH), is significantly higher than general population. Various theories of suicidal behavior have been proposed in the literature. Interpersonal-psychological theory of suicide suggests that increased suicidality is related to thwarted belongingness, and perceived burdensomeness. In this study, the relationship between depression, anxiety, belongingness, rejection, stigma and suicide was assessed in PLWH. Methods: The study was designed as cross-sectional and descriptive. It was carried out with 85 PLWH and 75 people living with Hepatitis B. The study group consists of PLWH, and the control group consists of people living with Hepatitis B. PLWH group was invited to be participated in the study by the social media accounts of a non-governmental organization; The hepatitis B group was reached from Istanbul Medeniyet University Gastroenterology Outpatient Clinic. The survey consists of Sociodemographic Form, Suicidal Behaviors Questionnaire, Hospital Anxiety and Depression Scale, Internalized AIDS-Related Stigma Scale, Chronic Illness Anticipated Stigma Scale, Interpersonal Needs Questionnaire, General Belongingness Scale, Modified Morisky Scale were applied to the participants. Result: Participants' mean age was 33,63 ± 9,81 and the majority was male (79,8%). PLWH had higher levels of stigma, perceived burdensomeness, thwarted belongingness, rejection, anxiety, depression, motivation and knowledge for treatment and higher severity of suicidal behavior compared to the control group (p<0,001, p<0,001, p=0,002, p<0,001,p=0,014, p=0,009, p<0,001, p<0,001, p<0,001, respectively). However, acceptance and general belongingness was lower in PLWH (p=0,008, p<0,001, respectively). The severity of suicidal behavior was associated with increased levels of stigma, rejection, perceived burdensomeness, thwarted belongingness, anxiety and depression (p=0,003, p<0,001, p<0,001, p<0,001, p<0,001, p<0,001, respectively), and decreased levels of acceptance and general belongingness in PLWH (p<0,001, p<0,001, respectively). Increased level of stigma was positively correlated with depression, anxiety, perceived burdensomeness, thwarted belongingness and rejection levels and the severity of suicidal behavior (p=0,001, p=0,004, p=0,003, p<0,001, p<0,001, p<0,001, respectively), whereas was negatively correlated with the acceptance and general belongingness (p<0,001, p<0,001, respectively). In terms of the duration of infection, PLWH had higher severity of suicidal behavior during the first years after diagnosis (p= 0.046). Finally, rejection was found to predict stigma and the severity of suicidal behavior in PLWH (p=0.007, p=0.01, respectively). Conclusion: The increased risk of depression, anxiety and suicide of PLWH should be considered at all stages of the disease and should be evaluated with a multidisciplinary approach. Educating healthcare workers in contact with PLWH, establishing healthcare policies; and informing the society on various stigmatized and marginalized issues related to HIV are highly warranted to reduce rejection which is associated with stigma and suicidal behavior.
