Kronik viral Hepatit C hastalarında tedavi başarı oranı ve tedavi başarısına etki eden faktörler

AMAÇ: Kronik viral hepatit C tanısı ile takip edilmiş hastaların tedavi başarılarının ve tedavi başarısına etki eden faktörlerin incelenmesi. YÖNTEM: Bu retrospektif çalışma; ?İstanbul Medeniyet Üniversitesi Göztepe Eğitim ve Araştırma Hastanesi, Enfeksiyon Hastalıkları ve Gastroenteroloji ? polikliniklerinde kronik viral hepatit C tanısı ile takip edilmiş hastaların dosyaları taranarak yapıldı. Hastaların yaşı, cinsiyeti, hepatomegali ya da splenomegali varlığı, eşlik eden hastalıklar, komplikasyonlar, tedavi protokolleri ve tedavi yan etkileri incelendi. BULGULAR: Çalışmaya 418 hasta dahil edildi. Hastaların yaş ortalaması 48.4 (SD:11.3; min:27-max:76) yıl olup, %40.4'ü (n=169) erkek, %59.6'sı (n=249) kadın idi. Toplam 50 hastada (%12) hepatomegali, 45'inde (%10.8) splenomegali saptandı. Hastaların %13.4'ünde (n=56) diyabet, %5.3'ünde (n=22) tiroid fonksiyon bozukluğu, %1.4'ünde (n=6) diyabet ve tiroid fonksiyon bozukluğu mevcut iken, %79.9'unda (n=334) bu hastalıklar saptanmadı. Toplam 4 hastada (%1) hepatoselüler karsinom, 29 hastada (%6.9) siroz, 33'ünde (%7.9) ise ilaç yan etkisi gelişti. Hastaların 4'ü ribavirin, 32'si klasik interferon, 13'ü pegile interferon, 69'u klasik interferon ile ribavirin, 297'si pegile interferon ile ribavirin, 3'ü klasik interferon, ribavirin ve pegile interferon tedavisi aldı. Tedavilerin alan hastaların 12'sinde (%2.9) "tedavi altında alevlenme", 14'ünde (%3.3) "kısmi yanıt", 89'unda (%21.3) "nüks", 124'ünde (%29.7) "tam yanıtsızlık", 179'unda (%42.8) ise "kalıcı viral yanıt" gelişti. Tedaviye en iyi yanıt pegile interferon artı ribavirin ile alındı. Tekli istatistik kıyaslamalarda genç yaş, splenomegali olmaması ve siroz olmaması tedaviye yanıtı anlamlı olarak artırırken, çoklu analizde sadece kombine tedavi kalıcı viral yanıt alınmasında etkili bağımsız değişken olarak bulunmuştur. SONUÇ: Bu çalışma kombine tedavi almanın kalıcı viral yanıt açısından anlamlı olduğunu göstermektedir. En yüksek kalıcı viral yanıt pegile interferon ile ribavirin kombinasyonu alan hastalar arasında görülmüştür.
OBJECTIVE: To examine treatment success of patients with chronic viral hepatitis C and the factors that affect the success of treatment. METHODS: This retrospective study was made by scanning the files of the patients with chronic hepatitis C who has been followed up in Infectious Diseases and Gastroenterology outpatients clinics in Istanbul Medeniyet University, Göztepe Training and Research Hospital. The demographic and clinical characteristics (presence of hepatomegaly or splenomegaly, comorbidities treatment protocols and their side effects) were examined. RESULTS: A total of 418 patients were examined, the mean age of the patients was 48.4 (SD:11.3; min:27-max:76) years. Of the participants, 40.4% (n=169) were male and 59.6% (n=249) were female. A fifty patients (12%) had hepatomegaly and 45 (10.8%) had splenomegaly. Although 79.9% (n=334) of the patients had no comorbidites, 13.4% of patients (n=56) had diabetes, 5.3% (n=22) of the patients had thyroid dysfunction, 1.4% (n=6) of the patients had both diabetes and thyroid dysfunction. Hepatocellular carcinoma developed in 4 patients (1%) , cirrhosis developed in 29 patients (6.9%) and 33 patients (7.9%) had side effects. Moreover, 4 patients had received ribavirin, 32 patients had received conventional interferon, 13 patients had received pegylated interferon, 69 patients had received conventional interferon and ribavirin, 297 patients had received pegylated interferon and ribavirin, 3 patients had received classic interferon, ribavirin and pegylated interferon. Finally, 12 treatments (2.9%) resulted in "breakthrough", 14 (3.3%) treatments resulted in "partial response", 89 (21.3%) treatments resulted in "relapse", 124 (29,7%) treatments resulted in "null response" and 179 (42.8%) patients resulted in "sustained virological response". Overall, the most successful outcomes were achieved among patients treated with pegylated interferon plus ribavirin. In univariate comparisons, young age, absence of splenomegaly and absence of cirrhosis vi were significant among patients with sustained virologic response. In multivariate analysis treatment with combination regimens were found to be independently significant among patients with sustained virologic response. CONCLUSION: This study indicated that combined treatment protocols are significant to achieve sustained virologic response. The best outcome results were observed among patients those treated with pegylated interferon and ribavirin.
