Obsesif kompulsif bozukluk otojen ve reaktif alt tiplerinde bağlamsal benlik ve benlik korkusu kavramlarının incelenmesi

Özet
Bu çalışmada, Obsesif Kompulsif Bozukluk (OKB) için yeni bir alt gruplandırma fırsatı sunan otojen ve reaktif OKB hastalarında bağlamsal benlik ve benlik korkusu etkilerinin incelenmesi, böylelikle bilişsel kuram bağlamında yapılmış olan bu alt tiplendirmenin benlik açısından farklılaşıp farklılaşmadığının incelenmesi amaçlanmıştır. Çalışmaya Temmuz 2023-Aralık 2023 arasında İstanbul Medeniyet Üniversitesi Tıp Fakültesi Göztepe Prof Dr Süleyman Yalçın Şehir Hastanesi Psikiyatri Bölümü Obsesif Kompulsif Bozukluk Polikliniğine başvuran, DSM-5 tanı ölçütlerine göre OKB tanısı alan hastalardan çalışmaya dahil edilme ölçütlerini karşılayan ve çalışmaya katılmayı kabul eden 60 hasta alınmıştır. Bu hastalar primer obsesyonlarına göre 30 otojen ve 30 reaktif OKB olarak gruplara ayrılmıştır. Örnekleme, bu hastalarla yaş, cinsiyet ve eğitim düzeyi açısından eşleştirilmiş sağlıklı kontrol grubu olarak 30 hastane çalışanı alınmıştır. Tüm katılımcılara bilgilendirilmiş onam formu, sosyodemografik veri formu, DSM-5 için Yapılandırılmış Klinik Görüşme, Vancouver Obsesif Kompulsif Envanteri (VOKE), Boyutsal Obsesyon Kompulsiyon Ölçeği (BOKÖ), Bağlamsal Benlik Ölçeği (BBÖ), Benlik Korkusu Anketi-Kısa Form (BKA-KF), Dünya Sağlık Örgütü Yaşam Kalitesi Ölçeği-Kısa Form (WHOQOL-BREF) uygulanmıştır. Etik kurul onayı alınmıştır. Hasta grubu için 30 otojen ve 30 reaktif obsesyonlara sahip katılımcı ile sağlıklı kontrol grubu için 30 katılımcı olmak üzere toplam 90 katılımcının verileri değerlendirilmiştir. Çalışmamızda üç gruptan her biri yaş (p=0,500), cinsiyet (p=0,349), eğitim düzeyi (p=0,668), medeni durum (p=0,110) gibi özellikler açısından birbiri ile benzer bulundu. Otojen OKB, reaktif OKB ve sağlıklı kontrol grupları arasında BBÖ (p=0,001) ve BKA-KF (p<0,001) puan ortalamalarının istatistiksel açıdan anlamlı derecede farklılık gösterdiği bulundu. Sağlıklı kontrol grubu vakalarının otojen ve reaktif OKB grubu vakalarına göre BBÖ ortalamalarının istatistiksel açıdan anlamlı seviyede daha yüksek olduğu (p<0,005); otojen OKB grubu vakalarının reaktif OKB grubu vakalarına göre, reaktif OKB grubu vakaların ise sağlıklı kontrol grubu vakalarına göre BKA-KF puan ortalamalarının istatistiksel açıdan anlamlı seviyede daha yüksek olduğu (p<0,005) bulundu. BKA-KF skorlarındaki artışın istatistiksel açıdan anlamlı seviyede otojen OKB tanı riskini 1,120 (p=0,004) arttırdığı bulundu. VOKE toplam skorlarını açıklamada otojen OKB grubu incelendiğinde sadece BKA-KF (p=0,031) skorlarının; reaktif OKB grubuna bakıldığında sadece BBÖ (p=0,042) skorlarının istatistiksel açıdan anlamlı seviyede etkili olduğu bulundu. OKB şiddetinde benlik korkusu ve bağlamsal benlik etkilerinin otojen ve reaktif alt tiplerde farklı olduğu, bu alt tiplere sahip OKB hasta gruplarında tedavi sürecinde potansiyel olarak faydalı olabilecek benlik müdahalelerin farklı kavramlar üzerinden olabileceği ve hastaların iyileşmelerine katkı sağlayabileceği düşünülmüştür.
In the current study, it was aimed to investigate the effects of self-as-context and fear of self in OCD patients with autogenous and reactive features that provide a new subgrouping for OCD. Hence, it is aimed to investigate whether this subtyping, which has been done within the context of cognitive theory, differs in terms of self. From the patients who applied to Istanbul Medeniyet University Faculty of Medicine Goztepe Prof Dr Suleyman Yalcin City Hospital Psychiatry Department Obsessive Compulsive Disorder Outpatient Clinic between July 2023 and December 2023, 60 patients who accepted to participate in the study and met the inclusion criteria among the patients diagnosed with OCD according to DSM-5 diagnostic criteria were included in the study. These patients were divided into groups according to the content of their primary obsessions: 30 patients with autogenous obsessions and 30 patients with reactive obsessions. The sample also included 30 hospital staff as a healthy control group who matched with these patients in terms of gender, age and education level. Informed consent form, sociodemographic data form, Structured Clinical Interview for DSM-5 (SCID-5), Vancouver Obsessive Compulsive Inventory (VOCI), Dimensional Obsessive Compulsive Scale (DOCS), Self-as-Context Scale (SACS), Fear of Self Questionnaire-short form (FSQ-8), World Health Organization Quality of Life Scale-short form (WHOQOL-BREF) were applied to all participants. Ethics committee approval was obtained for the study. The data of a total of 90 participants were evaluated. In our study, each of the three groups was found to be similar to each other in terms of characteristics such as age (p=0,500), gender (p=0,349), education level (p=0,668), and marital status (p=0,110). It was found that the mean scores of SACS (p=0,001) and FSQ-8 (p<0,001) differed statistically significantly between the autogenous OCD, reactive OCD and healthy control groups. The SACS mean scores of the healthy control group cases were statistically significantly higher than the autogenous and reactive OCD group cases (p<0,005). It was found that the FSQ-8 mean scores of the autogenous OCD group cases were statistically significantly higher than the reactive OCD group cases, and the reactive OCD group cases were statistically significantly higher than the healthy control group cases (p<0.005). It was found that an increase in FSQ-8 scores increased the risk of autogenous OCD diagnosis at a statistically significant level by 1,120 (p=0,004). When the autogenous OCD group was examined in explaining the VOCI total scores, only FSQ-8 (p=0,031) scores; when the reactive OCD group was examined, only SACS (p=0,042) scores were found to be statistically significant. It was thought that the effects of fear of self and self-as-context in OCD severity are different in autogenous and reactive subtypes, and that self-interventions that may be potentially useful in the treatment process in OCD patient groups with these subtypes may be based on different concepts and may contribute to the recovery of patients.
