Akne vulgarisin farklı lezyonları arasında toll benzeri reseptör-2 (tlr-2),toll benzeri reseptör-4 (tlr-4) ve antimikrobiyal peptid düzeylerinin karşılaştırılması

ÖZET Giriş ve Amaç: Akne Vulgaris (AV) başlıca pilosebase üniteyi etkileyen kronik inflamatuvar bir hastalıktır. Son zamanlarda yapılan çalışmalar AV'de toll benzeri reseptörler (TLR) ve antimikrobiyal peptidlerin (AMP)'in patogenezde önemli rol oynadığını göstermiştir. Bu çalışmada; TLR-2, TLR-4, hBD-1 ve katelisidin ekspresyonunun derinin epidermis, dermis, deri eki ve inflamasyon bölgesi gibi farklı bölgelerinde ekspresyon düzeyleri değerlendirilmiş, ayrıca papül, püstül, nodül ve komedon gibi farklı akne lezyonlarında ve sağlıklı deri örneklerinde karşılaştırılmıştır. Materyal ve Metod: Çalışmamız her iki cinsiyetteki 80 AV hastası ve 20 sağlıklı kontrol grubundan oluşan toplam 100 olgu ile yapıldı. Daha önce tedavi almamış hastaların 20 papüler, 20 püstüler, 20 komedonal, 20 nodüler lezyonundan ve 20 sağlıklı gönüllünün sırt bölgesindeki lezyonlarından deri biyopsisi yapıldı. Her lezyonda 4 ayrı alanda (epidermis, dermis, inflamasyon bölgesi ve deri eki) TLR-2, TLR-4, hBD-1 ve katelisidin ekspresyon düzeyleri immünohistokimyasal yöntemle değerlendirildi. Daha sonra farklı lezyonlar arasında bu parametreler karşılaştırıldı. Bulgular: Çalışmamızda lezyon türlerinde epidermis, inflamasyon bölgesi, dermis ve deri eki bölgelerinde TLR-2, TLR-4, hBD-1 boyanma düzeyleri arasında istatistiksel olarak anlamlı farklılık bulundu (p<0,01). Lezyon türlerine göre katelisidin boyanma düzeyleri arasında ise sadece inflamasyon bölgesinde istatistiksel olarak anlamlı farklılık bulundu (p<0,01). Epidermis bölgesinde; nodül görülen olguların TLR-2 boyanma düzeyleri, papül ve komedon görülen olgulardan düşük bulundu (p<0,05). İnflamasyon ve dermis bölgesinde; papül görülen olguların TLR-2 boyanma düzeyleri, püstül görülen olgulardan yüksek bulundu (p<0,01). Dermis bölgesinde; komedon görülen olguların TLR-4 boyanma düzeyleri, papül görülen olgulardan düşük bulundu (p<0,05). Epidermis bölgesinde; komedon görülen olguların hBD-1 boyanma düzeyleri, nodül görülen olgulardan yüksek bulundu (p<0,05). İnflamasyon bölgesinde; komedon görülen olguların katelisidin boyanma düzeyleri papül ve püstül görülen olgulardan düşük bulundu (p<0,05). Kontrol grubunun TLR-2 boyanma düzeyleri dermis, deri eki ve inflamasyon bölgesinde; nodül, papül, komedon ve püstül görülen olgulardan anlamlı düzeyde düşüktü (p<0,01). Kontrol grubunun TLR-4 ve hBD-1 boyanma düzeyleri tüm bölgelerde; nodül, papül, komedon ve püstül görülen olgulardan anlamlı düzeyde düşüktü (p<0,01). Kontrol grubunun katelisidin boyanma düzeyleri sadece inflamasyon bölgesinde nodül, papül, komedon ve püstül görülen olgulardan anlamlı düzeyde düşüktü ( p<0,05). Tartışma ve Sonuç: Akne vulgarisin farklı klinik lezyonlarında derinin farklı bölgelerinde TLR-2, TLR-4, hBD-1 ekspresyon düzeyleri farklılık gösterdi. Toll benzeri reseptör-2, TLR-4, hBD-1'in artmış ekspresyonları bu faktörlerin akne immünopatogenezinde çok önemli yerleri olduğu görüşünü desteklemektedir. Çalışmamızda katelisidin ekspresyonunun ise sadece inflamasyon bölgesinde; tüm lezyon tiplerinde kontrol grubuna kıyasla daha yüksek olarak saptanması; katelisidinin özellikle inflamasyon bölgesinde eksprese edilen bir AMP olduğunu düşündürmektedir. Sonuç olarak AV patogenezinde ve farklı akne tiplerinin gelişmesinde TLR-2, TLR-4, hBD-1 ve katelisidinin çok önemli rol oynadığı düşünülmektedir. Toll benzeri reseptör-2, TLR-4, hBD-1 ve katelisidin salınımının regülasyonunu hedefleyen farklı tedavi seçenekleri ile AV tedavisinde çok daha yüz güldürücü sonuçlar alınabileceğini düşünmekteyiz.
ABSTRACT Introduction and Aim: Acne vulgaris is a chronic inflammatory disease of the pilosebaseous unit. Recent studies shown that toll like receptors (TLR) and antimicrobial peptides (hBD-1, cathelicidin) play a role in the pathogenesis of acne vulgaris. In the present study the expression of TLR-2, TLR-4, hBD-1 and cathelicidin in different regions of skin such as epidermis, dermis, skin appendages and inflammation regions were evaluated and also it was compared in different skin lesions such as papule, pustule, nodule and comedone and in healthy skin samples. Material and Methods: : This study was performed on a total of 100 cases, including 80 patients with AV and a control group of 20 healthy individuals. Skin biopsies were performed from 20 papular, 20 pustular, 20 comedonal and 20 nodular lesions of patients who had not received any prior treatment and 20 healthy volunteers. Expression levels of TLR-2, TLR-4, hBD-1 and cathelicidin in four separate areas (epidermis, dermis, inflammation region and skin appendages) were evaluated by immunohistochemical method. Further, these parameters were compared between different skin lesions. Results: A highly significant statistical difference was found between the levels of staining of TLR-2, TLR-4 and hBD-1 from the epidermis, inflammation region, dermis and skin appendages of the lesions in this study (p<0.01). Levels of cathelicidin staining were statistically significantly different in only the inflammation region among the four separate areas (p<0.01). The level of expression of TLR-2 in the epidermis areas of cases with nodules were significantly lower than the level of expression in cases with papules and comedons (p<0.05). Levels of staining of TLR-2 in the inflammation and dermis regions of the cases with papules were significantly higher compared to the cases with pustules (p<0.01). The levels of staining of TLR-4 in the dermis region of cases with comedons were significantly lower compared to the cases with papules (p<0.05). The level of staining of hBD-1 in the epidermis region of the cases with comedons were significantly higher compared to cases with nodules (p<0.05). The level of expression of cathelicidin in the inflammation region of cases with comedons were significantly lower compared to the cases with papules and pustules (p<0.05). The level of staining with TLR-2 in the regions of dermis, skin appendages and inflammation area in the control group were significantly lower compared to the cases with nodules, papules, comedons, and pustules (p<0.01). The level of staining of TLR-4 and hBD-1 in the control group was significantly lower compared to the cases with nodules, papules, comedons and pustules in all regions (p<0.01). The level of staining with cathelicidin in the control group was significantly lower in only the inflammation region compared to the cases with nodules, papules, comedons and pustules (p<0.05). Conclusion: The level of expressions of TLR-2, TLR-4, hBD-1 and cathelicidin in different regions of the skin in various clinical lesions of AV were found to be distinctive. The increased expression of TLR-2, TLR-4, hBD-1 supports the opinion that these factors have important roles in immunopathogenesis of acne. In the present study, the higher levels of cathelicidin expression in only inflammation regions in all skin types when compared with the control group suggests that cathelicidin is an AMP, expressed especially in inflammation regions. In conclusion; it is thought that TLR-2, TLR-4, hBD-1 and cathelicidin play important role in pathogenesis of AV and in development of different acne types. We think that, better results could be obtained in treatment of AV with different treatment options targetted in regulation of TLR-2, TLR-4, hBD-1 and cathelicidin release.
