Çocukluk çağında antibiyotik ilişkili aşırı duyarlılık reaksiyonlarının değerlendirilmesi

| dc.description.abstract | Amaç: Antibiyotik ilişkili ilaç aşırı duyarlılık reaksiyonları (İADR) çocukluk yaş grubunda yüksek oranda bildirilmesine rağmen gerçek antibiyotik alerjisi sanılanın aksine oldukça nadirdir. Çalışmamızın amacı çocukluk çağında antibiyotik alerjisi sıklığını saptamak, antibiyotik alerjisine neden olan ilaç ve ilaç gruplarını belirlemek, bireysel ve ailesel risk faktörlerini ortaya koymak ve tanısal süreçte deri testleri ve ilaç provokasyon testlerinin önemini vurgulamaktır. Metod: 01 Eylül 2017-15 Ocak 2021 tarihleri arasında İstanbul Medeniyet Üniversitesi Göztepe Eğitim ve Araştırma Hastanesi çocuk alerji polikliniklerine antibiyotik alerjisi şüphesi ile başvuran 223 çocuk hastanın 286 antibiyotik ilişkili şüpheli İADR incelenmiştir. Hastanenin otomasyon yazılım hekim kayıtları incelenerek hastaların demografik özellikleri, reaksiyonların semiyolojisi, deri ve ilaç provokasyon testleri bilgileri edinilmiş ve her hasta için ''ilaç aşırı duyarlılık reaksiyonu anamnez formu'' (Ek 1) doldurulmuştur. Bulgular: 1 Eylül 2017 - 15 Ocak 2021 tarihleri arasında antibiyotik ilişkili ADR şüphesiyle 223 hasta 286 reaksiyon ile başvurmuş, 183 hasta [5(3-8)yaş, %50,2'si erkek] ve 241 reaksiyon tanısal yönden değerlendirilmiştir. Değerlendirilen ve değerlendirilemeyen hastalarda demografik özellikler, alerjik hastalıklar ve atopi yönünden farklılık saptanmamıştır. 30 hastada (%16.4), 38 reaksiyonda (%15.8) gerçek ADR saptanmıştır. Bu 30 hastadan 28'inde beta-laktam ilişkili ADR ve 2'sinde beta-laktam olmayan antibiyotik-ilişkili ADR vardır. En sık başvuru şüpheli amoksisilin-klavulonat ilişkili (%57) ADR nedeniyle olmasına rağmen en sık seftriakson (%54.2) alerjisi saptanmıştır. 3 reaksiyon antibiyotik ilişkili SJS/TEN, 25 reaksiyon tıbbi kayıtlarla dökümante edilmiş antibiyotik ilişkili anafilaksi, 3 reaksiyon DPT ile ve 7 reaksiyon İPT ile tanı almıştır. Gerçek antibiyotik ilişkili ADR riskini kronik hastalık varlığı (OR:3.21, %95GA:1.27-8.08, p=0.013), reaksiyonun ilaç alımını takiben ilk 1 saatte olması (OR:7.39, %95GA:1.17-46.51, p=0.033), gastrointestinal sistem bulgularının (OR:13.1, %95GA:2.27-76.12, p=0.004), solunum sistemi bulgularının (OR:15.48, %95GA:2.81-85.22, p=0.002) ve kardiyovasküler sistem bulgularının eşlik etmesi (OR:54.65, %95GA:2.38-1252.63, p=0.012), İADR-ilişkili hastanede yatış öyküsünün (OR:23.7, %95GA:5,03-11,70, p<0,001) arttırdığı bulunmuştur. Sonuçlar: Gerçek antibiyotik ilişkili ADR'nın, şüpheli reaksiyon sıklığına göre çok daha düşük bulunması bu hastalarda rasyonel tanısal değerlendirmenin önemini ortaya koymaktadır. Seftriakson, bir beta-laktam ilaç olarak en sık antibiyotik ilişkili gerçek ADR nedeni olarak tespit edilmiştir. Antibiyotik ilişkili ADR'ında hikayede sistemik tutulum, hastaneye yatış gereksinimi ve hastanın kronik hastalığı olması gerçek ADR riskini arttırmaktadır. | |
| dc.description.abstract | Objective: Although suspected antibiotic-associated drug hypersensitivity reactions (DHRs) are reported at a high rate during childhood, proven antibiotic allergy is quite rare. The aim of our study is to determine the frequency of actual antibiotic allergy in childhood, to identify culprit drugs and drug groups, to reveal individual and familial risk factors, and to emphasize the importance of skin tests and drug provocation tests in the diagnostic process. Method: Between September 01, 2017 and January 15, 2021, 286 suspected antibiotic-induced DHRs of 223 pediatric patients referred the Department of Pediatric Allergy, Istanbul Medeniyet University Göztepe Training and Research Hospital were included. By examining the hospital's software physician records, the demographic characteristics of the patients, the semiology of the reactions, the skin and drug provocation tests' results were obtained, and the "drug hypersensitivity reaction history form" (Appendix 1) was filled for each patient. Results: Between September 1, 2017 and January 15, 2021, 223 patients with 286 suspected antibiotic-induced DHRs were referred, 183 patients [5(3-8) years old, 50.2% male] and 241 reactions were evaluated diagnostically. There was no difference in terms of demographic characteristics, allergic diseases and atopy between the evaluated and unevaluated patients. Proven DHRs was detected in 30 patients (16.4%) and 38 reactions (15.8%). Of these, 28 patients had beta-lactam-associated DHRs and 2 had non-beta-lactam antibiotic-associated DHRs. Although the most common referral was due to amoxicillin-clavulanate (57%), the most common actual antibiotic allergy was due to ceftriaxone (54.2%). 3 reactions were diagnosed as antibiotic-associated SJS/TEN, 25 reactions were documented in medical records as antibiotic-associated anaphylaxis, 3 reactions were diagnosed with skin-prick tests (SPT), and 7 reactions were diagnosed with drug provocation tests (DPT). The risk factors for actual antibiotic-induced DHRs is the coexistent chronic disease (OR:3.21, 95%CI:1.27-8.08, p=0.013), the index reaction occurring during the first hour after drug intake (OR:7.39, 95%CI:1.17-46.51, p=0.033), gastrointestinal system findings (OR:13.1, 95%CI:2.27-76.12, p=0.004), respiratory system findings (OR:15.48, 95%CI:2.81-85.22, p=0.002) and cardiovascular system findings (OR:54.65, 95%CI:2.38-1252.63, p=0.012) during index reactions and history of DHR-related hospitalization (OR:23.7, 95%CI:5.03-11.70, p<0.001). Conclusions: The rate that the proven antibiotic-induced DHRs was much lower than the suspected reaction frequency that confirmed the importance of rational diagnostic evaluation in these patients. Ceftriaxone was identified as the most common cause of antibiotic-induced DHRs. The involvement of systemic findings, the need for hospitalization during index reaction, coexistent chronic disease increased the risk of actual antibiotic-induced DHR. | |
