Özet

Acil servise başvurup sepsis tanısı alan hastalarda qSOFA, SIRS, CALLY skorlarının mortalite ve yoğun bakım yatışı üzerine etkisini karşılaştırmayı ve CALLY skorunun sepsis hastalarında kullanılabilirliğini değerlendirmeyi amaçladık. Çalışmamıza etik kurulu onayı alındıktan sonra 01.01.2021-31.12.2023 tarihleri arasında Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesi Acil Servisine başvurup sepsis tanısı konulan 65 yaş üstü hastalar dahil edilmiştir. Çalışmamıza dahil edilen hastaların qSOFA, SIRS, CALLY skorları hesaplanmış, mortalite ve yoğun bakım yatışı üzerine etkileri karşılaştırılmıştır. Acil servise başvurup sepsis tanısı alan 65 yaş üstü 150 hasta çalışmaya alınmıştır çalışmaya alınan hastaların yaş ortalaması 81.5'tur. 82 kadın 68 erkek hasta vardır. Yoğun bakım ünitesine yatış oranı %28 iken mortalite oranı %33,3'tür. qSOFA>1 , SIRS >1 , CALLY ?2 anlamlı kabul edilmiştir. Çalışmamıza göre mortalite olan grupta CALLY skoru daha düşük, qSOFA ve SIRS skoru daha yüksek bulunmuştur. CALLY skoru, qSOFA, SIRS mortaliteyi öngörmede anlamlıdır. Çok değişkenli modelde mortalite olan ve olmayan hastaların ayrımında CALLY skoru ve qSOFA skorunun birlikte kullanımı anlamlı bulunmuştur. Yoğun bakım ünitesine yatışı öngörmede sadece qSOFA anlamlı bulunmuştur. Çalışmamızda qSOFA, CALLY, SIRS skorunun mortaliteyi öngörmede anlamlı olduğunu tespit ettik. Acil servise başvuran sepsis tanılı hastalarda erken tanı koymak, hastalığın ciddiyetini değerlendirmek ve mortaliteyi öngörmek önemlidir. Acil servise başvuran sepsis tanılı 65 yaş üstü hastalarda mortaliteyi öngörmede CALLY skoru ve qSOFA skorunun birlikte kullanımı klinik olarak anlamlı olacaktır. This study aimed to compare the effects of qSOFA, SIRS and CALLY scores of mortality and intensive care unit (ICU) admission of the patients who were admitted to emergency department and were diagnosed with sepsis and to evaluate the usability of CALLY score in sepsis patients. Following the approval of the ethics committee, patients aged 65 years and older who were admitted to the Emergency Department of Göztepe Prof. Dr. Süleyman Yalçın City Hospital and diagnosed with sepsis between 01.01.2021 and 31.12.2023 were included in the study. For the patients included, qSOFA, SIRS, and CALLY scores were calculated, and their effects on mortality and ICU admission were compared. A total of 150 patients aged 65 years and older who were diagnosed with sepsis in the emergency department were included in the study. The mean age of the patients was determined to be 81.5 years. Among the patients, 82 were female, and 68 were male. The rate of hospitalization in the intensive care unit was 28% and mortality rate was 33.3%. qSOFA >1, SIRS >1, CALLY ?2 were considered significant. In the group with mortality, CALLY score was found to be lower, whereas qSOFA and SIRS scores were higher. CALLY score, qSOFA, and SIRS scores were all found to be significant predictors of mortality. In a multivariable model, the combined use of CALLY score and qSOFA score was found to be significant in distinguishing between patients with and without mortality. However, only qSOFA score was determined to be significant in predicting ICU admission. In this study, qSOFA, CALLY, and SIRS scores were identified as significant predictors of mortality. Early diagnosis, evaluation of disease severity, and prediction of mortality were emphasized as crucial in patients with sepsis admitted to the emergency department. The combined use of the CALLY score and the qSOFA score in predicting mortality in sepsis patients aged 65 years and older admitted to the emergency department is considered clinically significant.
