Prematürenin metabolik kemik hastalığı tanısında yeni bir radyolojik değerlendirme

Perinatal ve neonatal bakımdaki iyileşmelere karşın prematürenin metabolik kemik hastalığı (MKH) hala önemini korumaktadır. Tanı ve izlem kriterlerine ilişkin bir uzlaşı olmadığı için gözden kaçabilmekte, erken ve geç dönemde önemli komplikasyonlara yol açabilmektedir. Bu yüzden de MKH gelişimi açısından riskli olan bebeklerin taranması gerekmektedir. Prematüre bebeklerde erken dönemde kemik sağlığını gösterecek ve taramada kullanılabilecek bir radyolojik yöntem bulunmamaktadır. Bu çalışma erken dönemde kemik yoğunluğu hakkında fikir vererek biyokimyasal belirteçlerle birlikte MKH taramasında kullanılabilecek, bebeğe ve üniteye ek yük ve risk getirmeyen yeni ve basit bir radyolojik yöntemin etkinliğini araştırmak amacıyla kurgulanmıştır. Bu amaçla postnatal 4-8. haftalar arasında çekilmiş akciğer grafileri kullanılarak "fırsatçı radyolojik tarama" yönteminin, teknik olarak da "kemik yoğunluğu ölçümü" nün uygulanması planlanmıştır. Çalışmamıza 01.01.2018 ile 31.12.2022 tarihleri arasında Yenidoğan Yoğun Bakım Ünitemize kabul edilen gestasyon yaşı <32 hafta olan, en az 28 gün yatırılan ve postnatal 4-8. haftalar arasında çekilmiş akciğer grafileri bulunan 101 prematüre bebek alınmıştır. Bebeklerin doğumdaki demografik özellikleri; maternal demografik bilgiler, kronik hastalıklar, gestasyonel patolojiler; beslenmeye ait parametreler ve majör komorbiditeler ile 4-8. haftalar arasındaki fosfor (P), alkalen fosfataz (ALP) düzeylerine ilişkin veriler elektronik kayıt sistemi (Nucleus) yolu ile, gerektiğinde yazılı dosyalara başvurarak toplanmıştır. Çalışma grubunda MKH tanısı biyokimyasal belirteçlere dayanarak iki farklı ekole göre konmuştur: Figueras-Aloy ekolüne göre postnatal 4–8. haftadaki serum ALP değerlerinin >500 IU/l olması MKH; ALP değerinin >500 IU/l olması ile birlikte serum P düzeyinin >4,5 mg/dl olması hafif, <4,5 mg/dl olması şiddetli MKH olarak tanımlanmıştır. Backström'e göre değerlendirildiğinde ise ALP değerinin >900 IU/l olması MKH olarak tanımlanmıştır. Bu bebeklerde postnatal 4-8. haftalar arasında çekilmiş akciğer grafileri hastanemiz PACS sisteminden Horos sistemine aktarılmış ve bu grafiler bebeklerin kliniğine ve laboratuvar bulgularına kör olan deneyimli 2 radyolog tarafından birbirinden bağımsız olarak değerlendirilmiştir. Sağ ve sol 2,4,6,8. kostalar ve torakal vertebralarda, sağ ve sol humerusta toplam 14 ölçüm bölgesi (ROI) belirlenmiş ve bu ROI'ler içindeki piksel yoğunluklar sayısal verilere dönüşmüş; MKH olan ve olmayan bebeklerde ortalama kemik yoğunluğunu ifade eden bu değerler karşılaştırılmıştır. Figueras-Aloy'a göre MKH tanısı alan bebeklerde yoğunluk ortalamaları, istisnasız her ROI'de MKH olmayan bebeklere göre daha düşük bulunmuş, ancak bu fark hiçbir ROI için istatistiksel bir anlam göstermemiştir (p>0,05). Tüm ROI'leri içerecek şekilde genel ortalama kemik yoğunluğu, kosta, torakal vertebra ve humerus ortalamaları; MKH olmayan vs MKH olan gruplarda sırasıyla; 53,4 ± 40,1 vs 47,7 ± 32.7; 48,9±37,3 vs 43,7±29,7; 69,1±53,4 vs 61.4±44.7 ve 39.1±27.1 vs 36.4±24.7 olarak hesaplanmıştır. Bu farklar da anlamlı bulunmamıştır (sırasıyla p=0,87, 0,94, 0,52 ve 0,76). MKH varlığı Backström'e göre değerlendirildiğinde de benzer sonuçlar elde edilmiştir. Kemik yoğunluk ortalamaları istisnasız her ROI'de MKH tanılı bebeklerde daha düşük olmasına karşın, farklar istatistiksel olarak anlamlı değildir (p>0,05). Çalışmamızda farklı kemik bölgelerinde MKH açısından yol gösterici olabilecek güvenilir cut-off değerleri olup olmadığı da araştırılmıştır. Ancak AUC (area under curve – eğri altında kalan alan) değerlerinin 0.46-0.52 aralığında olması saptanan cut-off değerlerinin MKH tanısında düşük bir ayırt edici güce sahip olduğunu göstermiştir. Bildiğimiz kadarıyla araştırmamız prematüre bebeklerde MKH taraması açısından bir "fırsatçı radyolojik değerlendirme yöntemi" kullanan ilk çalışmadır. Ayrıca manuel kemik yoğunluğu ölçümü tekniği de literatürde ilk defa kullanılmıştır. Kemik yoğunluk değerleri istisnasız her ROI'de MKH olan grupta daha düşük saptandığı halde istatistiksel bir anlam saptanmamıştır. Bu yöntemin MKH taramasındaki etkinliğinin net olarak belirlenmesi için daha geniş çalışmaların gerekli olduğunu düşünüyoruz.
Despite advances in the perinatal and neonatal care, the metabolic bone disease (MBD) of prematuriy is still an important concern. A universal consensus is lacking with regard to the diagnostic criteria and disease management; that is why MBD may go unnoticed resulting in serious complications at the short- and long-term. An appropriate screening in the high risk group of infants is essential. There isn't any radiological method which can be used as an early screening method and evaluating bone health for preterm infants. This study was designed to evaluate the appropriateness of a new and simple radiological method which could be used to screen MBD in the early period along with the biochemical indicators without causing any burden or risk to the babies and to the NICU. It was planned to perform the opportunistic radiological screening based on the evaluation of bone density measurements using chest X-ray's taken between postnatal 4-8 weeks. This study included the data collection of preterm infants with a gestational age ?32 weeks who were under care in our NICU at least for 28 days and who had a chest X-ray at a time between postnatal 4-8 weeks during a 5 year period starting from the beginning of 2018. The data was collected through the electronic record system (Nucleus), written records were used when needed. Demographic characteristics of the infants at birth, maternal demographic data, pregestational and gestational diseases, gestational adversities, major comorbidities, some key nutritional data, and serum phosphorus (P), alkaline phosphatase (ALP) levels at 4-8 weeks were recorded. MBD was defined according to 2 different approaches: (i) Figueras-Aloy approach defined MBD as a serum ALP level >500 IU/l at postnatal 4-8th week, severe MBD was defined as a serum ALP level >500 IU/l plus a serum P<4,5 mg/dl. (ii) Backström approach defined MBD as a serum ALP level >900 IU/l. The chest X-rays belonging to anytime between postnatal 4-8 weeks were transferred from the hospital PACS system to the Horos system and evaluated by 2 independent radiologists blinded to clinical and laboratory findings of the infants. A total of 14 ROIs (Region of Interest) were extracted from the left and right costal (2,4,6,8), thoracal vertebral (2,4,6,8), and humeral sites, the picsel densities within the ROIs were converted to the numerical values and these values were compared between the infants with and without MBD as an indicator of bone density. Although the mean bone density measurements at all ROIs were found to be lower in the infants with MBD according both to Figueras-Aloy and Backström approaches, the differences did not reach a statistical significance (p>0,05). The mean overall, costal, thoracal and humeral bone density values in the group with MBD vs without MBD were; 53,4 ± 40,1 vs 47,7 ± 32.7; 48,9±37,3 vs 43,7±29,7; 69,1±53,4 vs 61.4±44.7 ve 39.1±27.1 vs 36.4±24.7, respectively in the Figueras-Aloy approach (p =0,87, 0,94, 0,52 ve 0,76, respectively). The cut-off bone density values with a potential to predict MBD development were shown to have an AUC (area under curve) value between 0.46-0.52, suggesting that they do not seem to have a strong predictive value in this regard. To our knowledge, this study is the first one attempting to use the "opportunistic radiological screening" method evaluating its appropriateness as a means for screening MBD in preterm infants. Moreover, the manual bone density measurement technique was used for the first time in a study. Although lower bone density measurements were attained in the MBD group at all ROIs without exception, none of the differences was statistically significant. We believe that larger scaled studies are required to determine precisely the efficacy of this method for the MBD screening in preterm infants.
