dc.description.abstractÖZET Amaç: Testisin iniş mekanizmasını açıklamaya yönelik 200 yılı aşkın bir süredir çok sayıda teori öne sürülmesine karşın, tartışmalar halen sürmektedir. Testisin inişini engelleyen faktörler daha çok kanaliküler evrede etkilidir. Bu olası faktörlerden biride Calcitonin gene-related peptide (Kalsitonin gen-bağlantılı peptid) (CGRP)'dir. Bu çalışmada öncelikle insan kremaster kasında CGRP reseptörü varlığının değerlendirilmesi amaçlanmıştır. Ayrıca, farklı inguinal patolojilerde bu reseptörün yoğunluğunun araştırılması ve inmemiş testis etyopatogenezi ile reseptör dağılımının olası ilişkisinin değerlendirilmesi hedeflenmiştir. Gereç ve Yöntem: Bu klinik çalışma T.C. Medeniyet Üniversitesi Sağlık Bakanlığı Göztepe Eğitim ve Araştırma Hastanesi Çocuk Cerrahisi Kliniği'nde prospektif olarak gerçekleştirildi. Çalışma Aralık 2010 – Haziran 2011 tarihleri arasında 15'i inmemiş testis grubu, 15'i inguinal herni kız gurubu; 15'i de inguinal herni erkek grubu olmak üzere toplam 45 olgu ile yapılmıştır. Hastalardan alınan kremaster kası örneklendi. Hematoxylene-Eozin (H&E) kesitler incelendikten sonra immunhistokimyasal inceleme için uygun bloklar seçildi. İmmunhistokimyasal olarak CGRP sinonimi (Calcitonin ( SP17) antibody monoclonal, ready to use, GeneTex, Inc) ekspresyonu, ışık mikroskobunda kremaster kasında sitoplazmik boyanma dikkate alınarak yapıldı. Boyanma var ve yok olarak sınıflandırıldı. Bulgular: İnmemiş testis ve erkek inguinal herni gruplarında, kremaster kasında CGRP reseptörünün var olduğu gösterilmiştir. Kız inguinal herni grubunda ise reseptör bulunmamaktadır. İnmemiş testis grubunda reseptör görülme oranı erkek inguinal herni grubundan yüksektir ve sırasıyla, % 64,7 ve % 35,3 oranında saptanmıştır. İstatistiksel olarak, iki erkek grubu karşılaştırıldığında reseptör yoğunlukları arasında fark anlamlılık sınırındadır (p=0,05). İnmemiş testis grubunda; taraflar arasında reseptör yoğunluğuna göre istatistiksel olarak anlamlı farklılık saptanmamıştır (p>0,05). Sonuç: Bu sonuçlar CGRP'nin insanlarda, testisin inişinde rolünün olduğunu düşündürmüştür. Ayrıca, farklı inguinal patolojilerde CGRP reseptörünün yoğunluğunun farklılık gösterdiği tespit edilmiştir. Androjenler, GFS ve aynı zamanda CGRP'yi içerecek şekilde kombine çalışmalara ihtiyaç vardır.
dc.description.abstractABSTRACT Aim: Despite numerous theories proposed to explain the mechanism of descent of the testes over the past 200 years, discussions are ongoing. Factors that prevent the descent of the testes is are more common in canalicular stage. One of these potential factors is the Calcitonin gene-related peptide (CGRP). The primary aim of this study is to assess the presence of CGRP receptor in human cremaster muscle. Morover, investigation of the density of this receptor in various inguinal pathologies as well as evaluation of the possible contribution of its distribution in relation to the etiopathogenesis of undescended testes were aimed. Material and Methods: This clinical research was prospectively performed in T.C. Medeniyet University Sağlık Bakanlığı Göztepe Research and Training Hospital, Department of Pediatric Surgery between the months December 2010-June 2011. There were three patient groups consisiting of 15 boys with undescended testes, 15 girls with inguinal hernia, and 15 boys with inguinal hernia thus making a total of 45 patients. Cremaster muscle samples were obtained from the patients during the operation. Appropriate blocks were selected for immunohistochemical study after examining Hematoxylene-Eosin (H&E) cross-sections. Immunohistochemical evaluation was performed by expression of CGRP synonyme (Calcitonin (SP 17) antibody monoclonal, ready to use, Gene Tex, Inc) in the cremaster muscle cytoplasmic staining under light microscope. Staining was classified in terms of either presence or absence of the receptor Results: Presence of CGRP receptors in cremaster muscle was shown in undescended testes and male inguinal hernia groups. On the other hand, no receptor could be detected in girls with inguinal hernia. The presence of receptors was detected in a higher rate in undescended testes group than in boys with inguinal hernia with respective rates of 64,7% and 35,3%. Statistical analysis revealed that the difference in relation to the presence of receptor was statistically near significant between the two groups (p=0,05). There was no statistically significant difference in the density of receptor when the sides are compared in undescended testis group. Conclusion: These results suggest that the CGRP receptors play role in testicular descent mechanism. In addition, density of CGRP receptors differs in the different inguinal pathologies. Combined studies including androgens, GFN and CGRP receptors are needed for further clarification.
