Polyetheretherketone (PEEK) is an engineering polymer whose use has gained increasing attention in recent years due to its high resistance to thermal and chemical effects. Its radiolucency, biocompatibility, and resistance to sterilization have contributed to its widespread adoption in the medical field. The sterilization process is critical for the use of implants and implantable devices. Medical implants and implantable devices may be subjected to different numbers of cycles in a given sterilization modality. Repeated sterilization, however, may alter both mechanical properties and surface characteristics. This study aimed to investigate how the number of sterilization cycles, applied through two different modalities, influences the mechanical performance and surface roughness of PEEK. Test specimens prepared from PEEK in accordance with standards were sterilized 25, 50, 100, and 200 times, respectively, in an Autoclave (AC) and a Hydrogen Peroxide (HP) device. The changes produced by sterilization modality and cycle count in mechanical properties and surface roughness were evaluated. Within the same modality, differences across cycle counts were analyzed with analysis of variance (ANOVA), and post hoc Tukey tests were performed for significant results. Comparisons between sterilization modalities, independent of cycle count, were analyzed using the t-test. As a result of the statistical analyses, the comparisons of sterilization modalities and of cycle counts within the same modality for tensile strength were found to be not significant (p=0.247; AC p=0.618; HP p=0.627). For elongation at break, the comparison between sterilization modalities was found to be significant, whereas the comparison of cycle counts within the same modality was not significant in the AC modality but was significant in the xvii HP modality (p<0.001; AC p=0.406; HP p=0.009). For flexural strength, the comparisons of sterilization modalities and of cycle counts within the same modality were found to be not significant (p=0.465; AC p=0.498; HP p=0.132). For unnotched impact resistance, the comparison between sterilization modalities was found to be significant, whereas the comparison of cycle counts within the same modality was found to be not significant (p<0.001; AC p=0.921; HP p=0.850). For notched impact resistance, the comparison between sterilization modalities was found to be significant, whereas the comparison of cycle counts within the same modality was found to be not significant (p=0.022; AC p=0.088; HP p=0.809). For hardness, the comparison between sterilization modalities was found to be significant, whereas the comparison of cycle counts within the same modality was not significant in the AC modality but was significant in the HP modality (p<0.001; AC p=0.817; HP p=0.007). For surface roughness, the comparison between sterilization modalities was found to be not significant, whereas the comparison of cycle counts within the same modality was not significant in the AC modality but was significant in the HP modality (p=0.410; AC p=0.584; HP p=0.010). Repeated sterilization procedures did not adversely affect the biomechanical properties of PEEK material. These findings suggest that PEEK, due to its resilience against sterilization-induced alterations, is a reliable raw material suitable for safe use in reusable medical devices.
