| dc.description.abstract | Polikistik Over Sendromu (PKOS), üreme çağındaki kadınlarda yaygın görülen; oligo-anovülasyon, hiperandrojenizm ve polikistik over morfolojisi gibi bulgularla tanımlanan kompleks bir endokrin hastalıktır. Son yıllarda yapılan araştırmalar, PKOS'lu bireylerde tiroid fonksiyon bozuklukları ve otoimmün tiroid hastalıklarının (OİT) daha sık görülebileceğini ortaya koymuştur. Bu ilişki, hormonal dengesizlikler ve immün sistemin etkilenmesiyle açıklanmaktadır. Özellikle progesteron eksikliğine bağlı olarak gelişen inflamatuar süreçlerin, otoimmün yanıtı tetikleyebileceği ve tiroid otoantikor düzeylerinin yükselmesine neden olabileceği düşünülmektedir. Bu çalışma, Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesi'nde Ocak 2021 - Ocak 2025 tarihleri arasında retrospektif ve kesitsel bir tasarımla gerçekleştirilmiştir. Amaç, PKOS tanısı almış kadınlarda otoimmün tiroid hastalıkları ve hipotiroidi sıklığını, PKOS fenotiplerine göre değerlendirmektir. Çalışmaya Rotterdam kriterlerine göre PKOS tanısı konmuş, 18-40 yaş aralığında ve yeterli klinik ve laboratuvar verisine sahip 262 kadın dahil edilmiştir. Katılımcılar dört fenotipe ayrılmıştır: Fenotip A (HA+OA+PKO), Fenotip B (HA+OA), Fenotip C (HA+PKO) ve Fenotip D (OA+PKO). Katılımcıların tiroid fonksiyon testleri (TSH, serbest T3, serbest T4) ve otoimmün belirteçleri (Anti-TPO, Anti-TG) ile birlikte, metabolik göstergeleri (glukoz, insülin, HOMA-IR, lipid profili), androjen parametreleri ve klinik bulguları SPSS programı ile analiz edilmiştir. Hipotiroidi sıklığı Fenotip D grubunda en yüksek oranda gözlenmiş olmasına rağmen bu fark istatistiksel açıdan anlamlı bulunmamıştır. TSH ve FT3 düzeylerinde fenotipler arasında anlamlı bir fark saptanmamıştır. Fenotip C grubunda FT4 düzeyleri Fenotip D'ye göre yüksektir. Anti-TG pozitiflik oranı Fenotip D grubunda anlamlı şekilde daha yüksek bulunmuştur. Anti-TPO pozitifliği sıklığı fenotipler arasında farklılık göstermemiştir. Bu sonuçlar, normoandrojenik PKOS fenotiplerinin tiroid fonksiyon bozuklukları ve otoimmünite açısından daha dikkatli izlenmesi gerektiğini göstermektedir. Çalışmanın bulguları, PKOS'un fenotipik çeşitliliğinin sadece üreme sistemi değil, tiroid ekseni üzerinden de farklı klinik yansımalar gösterebileceğini ortaya koymaktadır. Bu nedenle, PKOS'lu bireylerde tiroid fonksiyonlarının düzenli olarak değerlendirilmesi hem erken müdahale hem de uzun dönemli komplikasyonların önlenmesi açısından önem taşımaktadır. | dc.description.abstract | Polycystic ovary syndrome (PCOS) is a complex endocrine disorder commonly seen in women of reproductive age, characterized by findings such as oligo-anovulation, hyperandrogenism, and polycystic ovary morphology. Recent studies have shown that thyroid dysfunction and autoimmune thyroid diseases (AITD) may be more common in individuals with PCOS. This relationship is explained by hormonal imbalances and immune system involvement. In particular, it is thought that inflammatory processes associated with progesterone deficiency may trigger an autoimmune response and lead to elevated thyroid autoantibody levels. This study was conducted at Göztepe Prof. Dr. Süleyman Yalçın City Hospital between January 2021 and January 2025 using a retrospective and cross-sectional design. The aim was to evaluate the prevalence of autoimmune thyroid diseases and hypothyroidism in women diagnosed with PCOS according to PCOS phenotypes. The study included 262 women aged 18–40 years who had been diagnosed with PCOS according to the Rotterdam criteria and had sufficient clinical and laboratory data. Participants were divided into four phenotypes: Phenotype A (HA+OA+PKO), Phenotype B (HA+OA), Phenotype C (HA+PKO), and Phenotype D (OA+PKO). Participants' thyroid function tests (TSH, free T3, free T4) and autoimmune markers (Anti-TPO, Anti-TG), along with metabolic indicators (glucose, insulin, HOMA-IR, lipid profile), androgen parameters, and clinical findings were analyzed using SPSS software. Although the prevalence of hypothyroidism was highest in the Phenotype D group, this difference was not statistically significant. No significant differences were found in TSH and FT3 levels between phenotypes. FT4 levels were higher in the Phenotype C group compared to Phenotype D. The prevalence of Anti-TG positivity was significantly higher in the Phenotype D group. The frequency of anti-TPO positivity did not differ between phenotypes. These results indicate that normoandrojenik PCOS phenotypes should be monitored more closely for thyroid dysfunction and autoimmunity. The findings of this study reveal that the phenotypic diversity of PCOS may have different clinical manifestations not only in the reproductive system but also in the thyroid axis. Therefore, regular assessment of thyroid function in individuals with PCOS is important for both early intervention and prevention of long-term complications.
