Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesinde çalışan hekimlerin temel ve ileri yaşam desteği ile ilgili güncel bilgi düzeylerinin değerlendirilmesi

Abstract

AMAÇ: Araştırmada İstanbul Medeniyet Üniversitesi Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesi'nde Haziran 2021–Temmuz 2021 tarihlerinde görev yapmakta olan asistan hekim, uzman hekim ve öğretim görevlisi hekimlerin temel ve ileri yaşam desteği ile ilgili bilgi düzeyleri ve bunu etkileyen faktörlerin incelenmesi amaçlanmıştır. YÖNTEM: Tanımlayıcı tipteki bu çalışmamızda temel ve ileri yaşam desteği ile ilgili AHA 2020 ve ERC 2021 kılavuzları kaynak alınarak hazırlanan 22 soruluk anket formu, Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesinde aktif olarak görev yapmakta olan 27 farklı bölümden 334 hekime uygulandı. Anket yüz yüze anket yöntemiyle yapıldı. Ankete başlamadan önce onayları alındı. Daha sonra yaş, cinsiyet, unvan, çalışılan bölüm ve hekimlik yaptığı süre durumunu içeren soruları yanıtlamaları istendi. Bu sorulardan sonra bilgi düzeyini ölçen 22 sorudan oluşan bilgi anketine cevap vermeleri istendi. Her iki bölüm yanıtları 100 puan üzerinden değerlendirildi. Bulgular sayı ve yüzde şeklinde belirtildi. İstatistiksel hesaplamalarda Kruskal-Wallis testi, istatistiksel karşılaştırmada ise student-t testi kullanıldı. p<0,05 anlamlı kabul edildi. BULGULAR: Araştırmada katılımcıların %53,6'sı kadın (n:179) ve %82'si 26-35 yaş aralığındadır. Çalışmada hekimlerin unvanlarına ve hekimlik sürelerine göre bilgi düzeyleri karşılaştırıldığında anlamlı fark saptanmamıştır. Son bir yıl içinde uyguladığı resüsitasyon sayısı 10 kereden fazla olanların bilgi düzeyleri diğerlerine göre anlamlı ölçüde yüksek saptanmıştır. Çalıştığı bölümde yoğun bakım hastası riskli hasta bakan hekimlerin diğer hekimlere göre bilgi düzeyi anlamlı derecede yüksek saptanmıştır. SONUÇ: Araştırma sonucunda hekimlerin bilgi düzeyleri çalıştığı serviste yoğun bakım hastası bakmanın, çalıştığı servisin dahili servis olması, çalıştığı bölümde sık KPR uygulamanın ve güncel kılavuza uygun teorik ve pratik eğim alma durumu yüksek Temel Yaşam Desteği ve İleri Kardiyak Yaşam Desteği puanıyla ilişkili saptanmıştır. Hekimlik süresi ve unvanlarıyla TYD ve İKYD puanları arasında kuvvetli bir ilişki saptanamamıştır.

AIM: In this study, it was aimed to examine the knowledge levels of resident physicians, specialist physicians and lecturer physicians who performed a duty at İstanbul Medeniyet University Göztepe Prof. Dr. Süleyman Yalçın Şehir Hospital from June 2021 to July 2021 about basic and advanced life support and factors affecting it. MATERIAL AND METHOD: In this our descriptive study, 22-question survey which was prepared based on AHA 2020 and ERC 2021 guidelines related to basic and advanced life support was applied to 334 physicians from 27 different clinics who were actively on duty. The survey was conducted by face-to-face survey method. Before starting the survey, the consent of the doctors was obtained. Then, they were asked to answer questions including age, gender,title, department worked and the status of the time spent as a physician. After these questions, they were asked to answer the knowledge questionnaire consisting of 22 questions measuring the level of knowledge. Responses to both sections were evaluated out of 100 points. Findings were expressed as numbers and percentages. Kruskal-Wallis test was used for statistical calculations and student-t test was used for statistical comparison. p<0.05 was considered significant. RESULTS: In the study, 53.6% of the participants were female (n:179) and 82% were in the age range of 26-35. In the study, a significant difference wasn't detected when the knowledge levels were compared according to doctors' titles and their medical duration. The knowledge level of those who have performed resuscitation more than 10 times in the recent year, was detected significantly higher compared to others. The knowledge levels of physicians who examined risky intensive care patients were detected significantly higher than other physicians. CONCLUSION: As a result of research , knowledge levels of physician caring for intensive care patients in the service where he/she works, the service that he/she works with is an internal service, to apply frequent CPR in the department where he /she works and in accordance with the current guide, having high theoretical and practical training situation were detected with high BLS and ACLS scores. A strong relationship wasn't found between the duration and titles of physicians and BLS and ACLS scores.
