Pediatrik popülasyonda TSH ve ST4'ün yaşa ve cinsiyete özgü sürekli referans aralıklarının indirekt tespiti

Abstract

AMAÇ: Pediatrik popülasyonda tiroid stimulan hormon (TSH) ve serbest T4'ün (sT4) yaşa ve cinsiyete özgü sürekli referans aralıklarını (RI) indirekt yöntemle belirlemeyi amaçladık. Olası patolojik verileri dışlayıp bu verilerin hesapladığımız RI'lara etkisini de inceledik. Belirlediğimiz RI'ları, direkt yöntemle belirlenen RI'larla karşılaştırıp kullandığımız yöntemin güvenilirliğini de test ettik. YÖNTEM: Laboratuvar veri tabanından aldığımız TSH ve sT4 verilerinden doğumdan erişkinliğe kadarki yaşa özgü sürekli RI'larını indirekt yöntemle belirledik. Verileri yaşlara göre gruplara ayırdık ve her grubun referans limitlerini belirledik. Her grubun referans limitlerini birleştirip sürekli RI'lar elde ettik. Belirlediğimiz RI'ları, direkt yöntemle belirlenen RI'larla kıyasladık. Böylece indirekt yöntemle belirlediğimiz RI'ların güvenilirliğini inceledik. BULGULAR: Belirlediğimiz referans aralıklara, olası patolojik verilerin önemli bir etkisinin olmadığını gördük. Direkt yöntemlerle hesaplanan RI'larla yüksek uyum bulduk. Ayrıca direkt yöntemle yapılan çalışmalar az bireyle ve geniş yaş aralığını temel alarak yapıldığından bu çalışmaların özellikle yaşamın ilk aylarında görülen dinamik değişim sürecini temsilde yetersiz kaldığını ve yüksek yalancı pozitifliğe neden olduğunu gördük. SONUÇ: Belirlediğimiz RI'ların, direkt yöntemle belirlenen RI'larla karşılaştırılabilir ve güvenilir olduğunu gördük. Bu indirekt yaklaşım, geleneksel yaklaşımların pek mümkün olmadığı durumlarda özellikle pediatrik popülasyonda diğer parametrelere de uygulanabilir. OOBJECTIVE: We aimed to determine the age and sex-specific continuous reference intervals (RI) of the thyroid stimulating hormone (TSH) and free T4 (fT4) in the pediatric population by indirect method. We excluded possible pathological data and examined the effect of these on the continuous RIs. We compared the continuous RIs with the RIs determined by direct method and tested the reliability of the method we use. METHOD: We determined the age-specific continuous RIs of TSH and fT4 from birth to adulthood by the indirect method using the data obtained from the laboratory database. We divided the data into age groups and determined the reference limits for each group. We combined the reference limits of each group and obtained continuous RIs. We compared the continious RIs with the RIs determined by direct method. Thus, we examined the reliability of the continious RIs. RESULTS: We have seen that the possible pathological data does not have a significant effect on the continious reference intervals. We found a high level of agreement with RIs determined by direct methods. In addition, since the studies used with direct method were based on few individuals and wide age range, we found that these studies were inadequate in representing the dynamic change process seen in the first months of life and caused high false positivity. CONCLUSION: We have found that the continious RIs we have determined are comparable and reliable to the RIs determined by direct method. This indirect approach can also be applied to other parameters, especially in the pediatric population which traditional approaches are not possible.
