Obez kadınlarda metabolik sendrom ve komponentlerinin cinsel işlev bozukluğu üzerine etkisinin değerlendirilmesi

AMAÇ: Metabolik sendrom tüm dünyada yaygınlığı gittikçe artan bir halk sağlığı sorunudur. Metabolik sendromun diyabet, hiperlipidemi, hipertansiyon ve aterosklerotik kalp hastalığı gibi hastalıkların yanısıra kadın cinsel i?lev bozukluğuna yol açtığına dair çalimalar yapılmitır. Ancak obezitenin ve metabolik sendromun kadın cinsel i?levi üzerine etkisi farklı sonuçları içeren çalimalar olmasından dolayı net olarak anla?ılamamitır. Kadın cinsel i?lev bozukluğu yaygınlığı yüksek olan buna rağmen önemsenmeyen bir sağlık problemidir. Çalimamızda obezite polikliniğine ba?vuran obez kadınlarla MetS tanısı olan obez kadınlar arasında cinsel i?lev bozukluğu açısından fark olup olmadığını tespit etmeyi amaçladık. YÖNTEM: Tek merkezli, prospektif kesitsel nitelikteki bu çalima Ocak 2022 - Nisan 2022 tarihleri arasında ?stanbul Medeniyet Üniversitesi Göztepe Prof. Dr. Süleyman Yalçın ?ehir Hastanesi Endokrinoloji ve Metabolizma Hastalıkları Kliniği, Obezite Polikliniği'ne ba?vuran 21-51 ya? aralığındaki 114 premenopozal dönemdeki katılımcı ile yapılmitır. Polikliniğe ba?vuran ve çalima kriterlerine uyan hastalara sosyodemografik veri formu, kadın cinsel i?lev bozukluğu anketi (Female Sexual Disfunction Index, FSFI) ve Beck Depresyon Envanteri anketleri hekim tarafından uygulanmitır. Hastaların ba?vuru esnasında hekim tarafından antropometrik ölçümleri, kan basıncı ölçümleri yapılmi, kan tetkikleri not edilmi?tir. Veriler IBM SPSS Statistics 18 © Copyright SPSS Inc. 1989, 2010 yazılımı kullanılarak analiz edilmi?tir. Çalimada istatistiksel anlamlılık düzeyi 0,05 olarak kabul edilmi?tir. BULGULAR: Ara?tırmaya katılan 114 katılımcının 73'ü (%64) obez, 41'i (%36) obez ve metabolik sendrom hastasıydı. Katılımcıların ya? ortalaması Özet vi 38,99±7,23 yıl (min:21 max:51)'dı. Demografik veriler, klinik özellikler, Beck depresyon skorları arasında fark izlenmeyen iki grup arasında FSFI total skoru değerleri arasında anlamlı fark izlenmemi?tir. FSFI alt parametrlerinden olan lubrikasyon metabolik sendromlu grupta anlamlı dü?ük saptanmitır (p=0,028), diğer alt parametlerde fark izlenmemi?tir. Cinsel i?lev bozukluğu olan grup ile olmayan grup kıyaslandığından vücut yağ yüzdeleri, vücut yağ toplam kg'ı, vücut gövdesel yağ kg'ı ve antropometrik ölçümler arasında anlamlı fark izlenmemi?tir. SONUÇ: Metabolik sendrom ve obezitenin kadın cinsel i?levi üzerine etkisi net olarak belirtilmemi? olsa da metabolik sendromun kadın cinsel i?levinin en az bir alt parametresini etkilediği görü?ü bir çok çalimada desteklenmi?tir. Bu çalimada diğer çalimalarda bildirilen obezite, MetS ve KCFB arasındaki ili?kiyi doğrulayamadık. Bununla birlikte, bir uyarılma sorununun i?areti olarak lubrikasyonun, MetS'den etkilendiğini gösterdik. Ek olarak, MetS olan hastaların aynı ya?taki obez hastalara göre daha depresif bir ruh hali içinde oldukları bulundu. MetS, obezite, hipertansiyon, diyabet gibi KCFB açısından risk faktörü bulunduran hastaları cinsel i?lev açısından değerlendirerek bütüncül yakla?mak KCFB'nin genel sağlık durumuna olası etkilerini önleyebilmek adına önem ta?ımaktadır.
A?M: Metabolic syndrome is a public health problem that is increasing in prevalence all over the world and causes many health problems. Metabolic syndrome leads to diseases such as diabetes, hyperlipidemia, hypertension and atherosclerotic heart disease. In addition, there are studies showing that it causes female sexual dysfunction. However, the effect of obesity and metabolic syndrome on female sexual function has not been clearly understood because there are studies with different results. Although the prevalence of female sexual dysfunction is high, it is a neglected health problem. In our study, we aimed to investigate the effects of metabolic syndrome components on famele sexual dysfunction. METHOD: This study was carried out between January 2022 and April 2022. This is a single-center, prospective cross-sectional study, conducted with 114 participants between the ages of 21-51. The study was carried out among patients who applied to the Istanbul Medeniyet University Göztepe Prof. Dr. Süleyman Yalçın City Hospital, Department of Endocrinology and Metabolism Defects, Obesity Polyclinic. Sociodemographic data form, female sexual dysfunction questionnaire (Famele sexual dysfunction index, FSFI) and Beck depression inventory questionnaires were administered by the physician to the patients who met the study criteria. Anthropometric measurements, blood pressure measurements were made by the physician at the time of admission, and blood test results were noted. Data IBM SPSS Statistics 18 © Copyright SPSS Inc. It was analyzed using the 1989, 2010 software. The statistical significance level was accepted as 0.05 in the study. RESULTS: Of the 114 participants in the study, 73 (64%) were obese, 41 (36%) were obese and had metabolic syndrome. The mean age of the Abstract viii participants was 38.99±7.23 years (min.21, max:51). There was no significant difference between the two groups in terms of demographic data,clinical features, beck depression scores and fsfi total score. Lubrication, which is one of the sub-parameters of FSFI, was found to be significantly lower in the group with metabolic syndrome (p=0,028). When the group with sexual dysfunction and the group without sexual dysfunction were compared, no significant difference was observed between body fat percentages, body fat total kg and anthropometric measurements. CONCLUSION: Although the difference between metabolic syndrome, obesity and female sexual function has not been clearly stated, the view that metabolic syndrome affects at least one sub-parameter of female sexual function has been supported by many studies. In this study, we could not confirm the association between obesity, MetS and FSD reported in other studies. We show that lubrication as a sign of an arousal problem is affected by MetS. Patients with MetS were found to be in a more depressed mood than obese patients of the same age. It is important to evaluate patients with risk factors for FSD, such as MetS, obesity, hypertension, diabetes, in terms of sexual function, and to approach them holistically in order to prevent the possible effects of FSD on general health.
