Neoadjuvan tedavi almış meme kanserli olguların tedaviye yanıtının radyolojik ve histopatolojik olarak değerlendirilmesi

Özet
Amaç: Neoadjuvan kemoterapi, lokal ileri meme kanserlerinde meme koruyucu cerrahi şansını arttırmaktadır. İnoperabl kabul edilen hastalarda cerrahi şansını arttırdığı gibi, aynı zamanda onkolojik tedavi protokolüne yanıt varlığının öngörülmesinde de faydalıdır. Neoadjuvan tedaviye yanıtın değerlendirilmesinde çeşitli görüntüleme yöntemleri kullanılmaktadır. Bu çalışmanın amacı, Manyetik Rezonans Görüntüleme (MRI) ve Pozitron Emisyon Tomografisi (PET-CT) görüntülemelerinin neoadjuvan kemoterapi yanıtındaki öngörücülüğünü değerlendirmektir. Gereç ve Yöntem: Ocak 2018-Mayıs 2020 arasında meme kanseri nedeniyle neoadjuvan kemoterapi sonrası opere edilmiş 88 hasta çalışmaya dahil edildi. Hastaların dosyaları ve görüntülemeleri İstanbul Medeniyet Üniversitesi Tıp Fakültesi Etik Kurulu'ndan onay alındıktan sonra retrospektif olarak incelendi. Neoadjuvan tedavi öncesi ve sonrası uygulanan görüntüleme yöntemleri ile tedaviye yanıt değerlendirildi. Operasyon sonrası patoloji raporları analiz edildi. MRI'de yanıt değerlendirilmesi Response Evaluation Criteria in Solid Tumors (RECIST) ve kontrastlanma paterni değişikliklerine göre sınıflandırılarak değerlendirildi. PET-CT'deki yanıt memedeki primer tümör ve aksilla için ayrı ayrı olmak üzere PET Response Criteria in Solid Tumors (PERCIST) sınıflamasına göre değerlendirildi. Aynı şekilde patolojik yanıt varlığı da, Sataloff sınıflamasına göre primer tümör ve aksilla için değerlendirildi. MRI'deki yanıt skorları ve PET-CT yanıt skorları patolojik yanıt skorlarıyla istatistiksel olarak karşılaştırıldı. İstatistiki veriler SPSS (Statistical Package for the Social Sciences) versiyon 20 (SPSS Inc.; Chicago, IL, USA) kullanılarak analiz edildi. Bulgular: Ortalama yaşı 53.1(±15.6) olan88 kadın hasta değerlendirildi. Yapılan analizlerde neoadjuvan kemoterapi öncesi ve sonrası tümör çaplarının (MRI, Ultrasonografi, PET-CT) ve PET-CT'de Standart Uptake Değeri (SUDmax) değerlerinin istatistiksel açıdan anlamlı olacak şekilde azaldığı saptandı (p<0.001). ER pozitifliği ve PR pozitifliği ile kemoterapiye patolojik tam yanıt varlığı arasında negatif korelasyon görülürken (sırasıyla p:0.047 ve p:0.02), HER-2 pozitifliği ile kemoterapiye tam yanıt arasında istatistiki olarak anlamlı pozitif korelasyon bulundu (p:0.01). Moleküler alt gruplar arasında primer tümörün patolojik tam yanıtının en sık HER-2 overeksprese alt grubunda olduğu saptandı (p:0.02). MRI RECIST'e göre değerlendirildiğinde tam yanıt öngörüsü açısından patolojik tam yanıt varlığı ile orta düzeyde uyum gösterirken (?:0.46), kontrast tutulumundaki regresyon skoruna göre değerlendirildiğinde patolojik tam yanıtla önemsiz derecede uyum gösterdiği saptandı (?:0.17). PET-CT ile yanıt değerlendirildiğinde ise neoadjuvan tedaviye tam yanıt açısından patolojik tam yanıt varlığı ile iyi düzeyde uyumlu olduğu saptandı (?:0.61). Neoadjuvan tedaviye tam yanıtın tespitinde, MRI seçiciliği %80.7, duyarlılığı %65.2, pozitif prediktif değeri %75 ve negatif prediktif değeri %72.4 bulundu. Neoadjuvan tedaviye tam yanıtın tespitinde PET-CT seçiciliği %75.7, duyarlılığı %100, pozitif prediktif değeri %57.8, negatif prediktif değeri ise %100 bulundu. MRI ve patolojik tam yanıt uyumluluğu, ER boyanma yoğunluğu zayıf ve orta olan hastalarda ve e-cadherin ekspresyon kaybı olan hastalarda istatistiki olarak daha fazla görüldü (p:0.03). PET-CT ve patolojik tam yanıt uyumluluğu ise, HER-2 overeksprese ve Lüminal B alt grupları ve Ki-67 pozitif olan hastalarda daha fazla tespit edildi (sırasıyla; p:0.01 ve p:0.01). Sonuç: MRI ve PET-CT meme kanserinde neoadjuvan kemoterapiye tam yanıtın değerlendirilmesinde belli oranlarda duyarlı yöntemlerdir. Neoadjuvan tedavi sonrası, ER boyanma yoğunluğu zayıf ve orta olan ve e-cadherin ile ekspresyon kaybı izlenen hastalarda MRI'nin, HER-2 overeksprese, Lüminal B ve Ki-67 pozitif olgularda ise PET-CT'nin tam yanıt varlığının değerlendirilmesinde daha duyarlı olduğu ortaya konmuştur.
Objective: Neoadjuvant chemotherapy increases feasibility of breast-conserving surgery in locally advanced breast cancers. As it increases possibility of surgery in patients considered inoperable, it is also useful in predicting the presence of response to the oncological treatment protocol. Various imaging methods are used to evaluate response to neoadjuvant therapy. The aim of this study is to evaluate the predictability of Magnetic Resonance Imaging (MRI) and Positron Emission Tomography (PET-CT) imaging in neoadjuvant chemotherapy response. Methods:Eighty-eight patients who were operated after neoadjuvant chemotherapy for breast cancer between January 2018 and May 2020 were included in the study. The files and imaging of the patients were analyzed retrospectively after approval from the Ethics Committee of Istanbul Medeniyet University Faculty of Medicine. Response to treatment was evaluated with imaging methods applied before and after neoadjuvant therapy. Post-operative pathology reports were analyzed. Response evaluation in MRI was evaluated by classification according to Response Evaluation Criteria in Solid Tumors (RECIST) and another claassification according to contrast pattern changes. The response in PET-CT was evaluated separately for the primary tumor in the breast and axilla according to the PET Response Criteria in Solid Tumors (PERCIST) classification. Likewise, presence of pathological response was evaluated for primary tumor and axilla according to Sataloff classification. MRI response scores and PET-CT response scores were compared statistically with pathological response scores. Statistical data were analyzed using SPSS (Statistical Package for the Social Sciences) version 20 (SPSS Inc .; Chicago, IL, USA). Results:Eighty-eight female patients with a mean age of 53.1 (±15.6) were evaluated. In the analyzes, it was found that tumor diameters (ultrasonography, MRI and PET-CT) and PET-CT Standard Uptake Values (SUVmax) decreased statistically after neoadjuvant chemotherapy compared to same values before neoadjuvant chemotherapy(p <0.001). While there was a negative correlation between ER positivity and PR positivity and the presence of complete pathological response to chemotherapy (p: 0.047 and p: 0.02, respectively), a statistically significant positive correlation was found between HER-2 positivity and pathological complete response to chemotherapy (p: 0.01). Among the molecular subgroups, the pathological complete response of the primary tumor was most frequently found in the HER-2 overexpressed subgroup (p: 0.02). When MRI was evaluated according to RECIST, it was found to be moderately compatible with the presence of a complete response (?: 0.46), when it was evaluated according to the regression score in contrast enhancement, it was found to be slightly compatible with the pathological complete response (?: 0.17) in terms of complete response prediction. When the response was evaluated with PET-CT, it was found that it was substantially compatible with the presence of a complete pathological response in terms of complete response to neoadjuvant therapy (?: 0.61). In determining the complete response to neoadjuvant therapy, the selectivity of MRI was 80.7%, sensitivity was 65.2%, positive predictive value was 75%, and negative predictive value was 72.4%. In determining the complete response to neoadjuvant therapy, the selectivity of PET-CT was 75.7%, sensitivity was 100%, positive predictive value was 57.8%, and negative predictive value was 100%. MRI and pathological complete response compatibility were statistically more common in patients with weak and moderate ER staining intensity and in patients with loss of e-cadherin expression (p: 0.03). PET-CT and pathological complete response compatibility were common in patients with HER-2 overexpressed and Luminal B subgroups and in patients with Ki-67 positivity (p: 0.01 and p: 0.01, respectively). Conclusion: MRI and PET-CT are sensitive methods at certain rates in evaluating the complete response to neoadjuvant chemotherapy in breast cancer. After neoadjuvant therapy, it has been revealed that MRI was more sensitive in patients with weak and moderate ER staining intensity and with loss of expression with e-cadherin, and PET-CT was more sensitive in evaluating the presence of complete response in HER-2 overexpressed, Luminal B and Ki-67 positive cases.
