Sıçanlarda normobarik hiperoksinin tekrarlayan anestezi sonrası gelişen postoperatif kognitif disfonksiyon üzerine önkoşullayıcı etkisi

Özet

Amaç: Postoperatif kognitif disfonksiyon, tekrarlayan anestezi maruziyeti sonrası sık görülen bir komplikasyondur. Volatil anesteziklere maruziyet sonrası gelişen postoperatif kognitif disfonksiyonun muhtemel mekanizmalarından biri de nöroapoptozistir. Bu çalışmanın amacı, iskemik toleransı artırıcı etkisi olduğu kanıtlanan normobarik hiperoksinin tekrarlayan anestezi sonrası gelişen postoperatif kognitif disfonksiyon üzerine önkoşullayıcı etkisini araştırmaktır. Yöntem: Ağırlıkları 300-320 gram arasında değişen 24 adet Sprague-Dawley sıçan randomize olarak 4 gruba (n=6) ayrıldı: Kontrol grubu (C), normobarik hiperoksi grubu (O), tekrarlayan sevofluran anestezisi grubu (S), normobarik hiperoksi ardından tekrarlayan sevofluran anestezisi grubu (OS). C grubuna normobarik hiperoksi veya tekrarlayan anestezi uygulanmadı. O grubuna 1., 3., 5., 7. ve 9. günlerde her gün üçer saat olmak üzere toplam 5 gün yalnızca normobarik hiperoksi uygulandı. S grubuna 1., 3., 5., 7. ve 9. günlerde her gün ikişer saat olmak üzere toplamda 5 gün yalnızca sevofluran anestezisi uygulandı. OS grubuna 1., 3., 5., 7. ve 9. günlerde her gün üçer saat normobarik hiperoksi önkoşullandırması ardından ikişer saat sevofluran anestezisi toplamda 5 gün olmak üzere uygulandı. Birinci gün anestezi uygulamalarından sonra anestezinin solunumsal ve metabolik etkilerini incelemek için Grup S ve Grup OS'deki sıçanlardan arteriyel kan örnekleri alındı. Yapılan uygulamalardan sonra çalışmanın 10. gününde sıçanlar Morris Su Labirenti (Morris Water Maze, MSLT) testine alındı. Sıçanlara davranış testleri uygulandı. Sonrasında sıçanlar genel anestezi altında dekapite edildi ve beyin dokuları Hematoksilen & Eozin (H&E) ile boyanarak histopatolojik olarak incelendi. Bulgular: Birinci gündeki anestezi uygulamalarından sonra Grup S ve Grup OS'deki sıçanların kan gazı analizlerinde anestezinin herhangi bir solunumsal yahut metabolik bozukluğa yol açmadığı görüldü. MSLT'nin öğrenme fazının sonucunda Grup S'nin platformu bulma süresinin diğer gruplara göre daha uzun olduğu izlendi. Grup içi karşılaştırmalarda Grup S hariç diğer gruplarda 1. gün ve 4. gün arasında istatiksel açıdan anlamlı düşüş saptandı. Platformun bulunduğu kadranda geçirilen süre oranındaki değişimlerde gruplar arasında istatistiksel olarak anlamlı fark bulunmadı. Platformu bulmak için katedilen mesafelerde her grubun 1. ve 4. günleri arasında istatistiksel olarak anlamlı düşüş görüldü. Bellek fazında ise platformu bulma sayısı Grup S'de, Grup C ve Grup O'ya göre istatistiksel olarak anlamlı düzeyde düşüktü. Platformu bulma sayısı Grup OS'de, Grup S'ye göre daha fazla olsa da bu sonuç istatistiksel olarak anlamlı bulunmadı. Histopatolojik incelemelerde hipokampüsteki kırmızı hücre dejenerasyonu Grup S'de diğer gruplara göre yüksek saptandı. Sonuç: Tekrarlayan sevofluran maruziyeti, sıçanlarda öğrenme ve bellek fonksiyonlarında bozulmaya yol açar. Normobarik hiperoksinin önkoşullayıcı etkisi ile tekrarlayan sevofluran maruziyetine bağlı kognitif fonksiyonlardaki bozukluğun ve hipokampüsteki nörodejenerasyonun önüne geçilebilir.

Aim: Postoperative cognitive dysfunction is a common complication after repeated exposure to anesthesia. One of the possible mechanisms of postoperative cognitive dysfunction after exposure to volatile anesthetics is neuroinflammation and apoptosis. The aim of this study is to investigate the preconditioning effect of normobaric hyperoxia, which has been proven to increase ischemic tolerance, on postoperative cognitive dysfunction after repeated anesthesia. Method: Twenty-four Sprague-Dawley rats weighing between 300-320 gr were randomly divided into 4 groups (n=6): Control group (C), normobaric hyperoxia group (O), recurrent sevoflurane anesthesia group (S), normobaric hyperoxia followed by recurrent sevoflurane anesthesia group. (OS). Normobaric hyperoxia or repetitive anesthesia was not administered to group C. In the O group, only normobaric hyperoxia was administered for a total of 5 days, three hours a day, on the 1st, 3rd, 5th, 7th and 9th days. In the S group, only sevoflurane anesthesia was administered for 5 days, for two hours each day on the 1st, 3rd, 5th, 7th and 9th days. In the OS group, 3 hours of normobaric hyperoxia preconditioning followed by 2 hours of sevoflurane anesthesia was administered for a total of 5 days, on the 1st, 3rd, 5th, 7th and 9th days. After the first day of anesthesia, arterial blood samples were taken from rats in Group S and Group OS to examine the respiratory and metabolic effects of anesthesia. After the administrations, on the 10th day of the study, the rats were taken to the Morris Water Maze test. Behavioral tests were applied to the rats. Afterwards, the rats were decapitated under general anesthesia and their brain tissues were stained with Hematoxylin & Eosin (H&E) and examined histopathologically. Results: In the blood gas analyzes of the rats in Group S and Group OS after the first day of anesthesia, it was observed that anesthesia did not cause any respiratory or metabolic disorders As a result of the learning phase of MSLT, it was observed that Group S had a longer time to find the platform than the other groups. In intragroup comparisons, a statistically significant decrease was found between the 1st and 4th days in the other groups, except for Group S. There was no statistically significant difference between the groups in the changes in the rate of time spent in the quadrant where the platform is located. There was a statistically significant decrease in the distances traveled to find the platform between the 1st and 4th days in each group. In the memory phase, the number of platform findings was statistically significantly lower in Group S compared to Group C and Group O. Although the number of platform findings was higher in Group OS than in Group S, this result was not statistically significant. In histopathological examinations, red cell degeneration in the hippocampus was found to be higher in Group S compared to other groups. Conclusion: Repeated exposure to sevoflurane leads to impaired learning and memory functions in rats. With the preconditioning effect of normobaric hyperoxia, cognitive dysfunction and hippocampal neurodegeneration due to repeated sevoflurane exposure can be prevented.
