| dc.description.abstract | Post operatif akut ağrı kontrolünün iyi sağlanması, günümüzde anestezide en çok önem verilen konulardan biri olmuştur. Bu sayede hastanın erken taburculuğu, ERAS (Enhanced Recovery After Surgery) protokolüne uygunluk, kronik ağrı oluşumunun engellenmesi gibi getiriler örnek gösterilebilir. Akut ağrının daha iyi kontrol edilmesi için meme cerrahisi grubunda toraks duvarı sinir blokları tanımlanmıştır. Bu bloklar daha önce tanımlanmış yöntemlere göre, daha güvenli daha kolay uygulanabilir durumdadır. Bu çalışmada, meme koruyucu cerrahi ameliyatı planlanan hastalarda, serratus anterior plan blokunda (SAPB), tek seferlik enjeksiyon ile bu plana katater yerleştirilmesi ve devamlı infüzyon sağlanması arasındaki analjezik etkilerin araştırılması amaçlanmıştır. Çalışmamız İstanbul Medeniyet Üniversitesi Göztepe Eğitim ve Araştırma Hastanesi Genel Cerrahi Kliniğinde planlı meme koruyucu cerrahi endikasyonu konulan, yazılı ve sözlü bilgilendirilmiş onamları alınmış, yaşları 18 ve 80 arası olan, ASA (American Society of Anesthesia) I-III gruplarından 50 hastada yapılmıştır. Çalışmaya alınan hastalar 2 gruba ayrıldı. Opere olacak hastalardan Grup I (n=25) olgulara tek seferlik SAPB uygulandı ve Grup II (n=25) olgulara tek seferlik SAPB peşine serratus anterior planına periferik sinir katateri yerleştirildi ve ameliyat sonrası infüzyon şeklinde 24 saat analjezisini alacak şekilde hazırlık yapıldı. Bloklar sonrası sensoryal dermatomları kontrol edilen hastalar sonrasında ameliyat odasına alındı ve cerrahi işlem uygulandı. Peri-operatif hastaların hemodinamik değerleri, remifentanil tüketimleri, cerrahi süreleri not edildi. Hastaların servis takiplerinde ise 1,4,8,12 ve 24. saatlerde Nümerik oranlama skorları (NRS), bulantı ve kusma atak sayısı, ilk kurtarma analjezisine kadar geçen süre, yan etki gözlenip gözlenmediği not edildi. Çalışmaya dahil edilen hastaların demografik verileri, ASA skorları, vücut kitle indeksleri, ağırlıkları, anestezi süresi, cerrahi süresi ve peri-operatif remifentanil tüketim miktarları her iki grupta da benzerdi. Servis takiplerinde yapılan NRS değerlendirmesinde 1. saatte iki grup arasında bir farklılık yoktu (p>0,05). Fakat 4, 8, 12 ve 24. saatlerde tek seferlik SAPB grubu, devamlı infüzyon SAPB grubuna göre daha yüksek ağrı skoruna sahipti ve anlamlı fark bulundu (p<0,05). Ayrıca ilk kurtarma analjezisinin de tek seferlik SAPB grubunda daha erken kullanıldığı ve anlamlı sonuç olarak bulundu (p<0,05). Meme koruyucu cerrahi planlanan hastalarda, akut post-operatif ağrının önlenmesinde, serratus anterior planına periferik katater aracılığı ile devamlı infüzyon sağlanmasının, tek seferlik SAPB'undan üstünlüğü gösterilmiştir. Bu sayede, opioid tüketiminde azalma, erken mobilizasyon ve taburculuk, akut ağrının kronik ağrıya dönüşümünün azalması sağlanabilir. | dc.description.abstract | Achieving effective postoperative pain control has become one of the most important aspects in anesthesia. This contributes to early patient discharge, compliance with Enhanced Recovery After Surgery (ERAS) protocols, prevention of chronic pain development and improved psychological well-being. Thoracic wall nerve blocks have been identified in the breast surgery field as a means to better control acute pain. Compared to traditional methods, these blocks are safer and easier to administer. This study aims to investigate the analgesic effects of single-shot serratus anterior plane block (SAPB) versus continuous infusion through catheter placement in the serratus anterior plane in patients undergoing breast-conserving surgery. The study was conducted on 50 patients between the ages of 18 and 80, who were scheduled for breast-conserving surgery at Istanbul Medeniyet University Göztepe Training and Research Hospital General Surgery Clinic. Written and verbal informed consent was obtained from the patients, who were classified into ASA I-III groups. The patients were divided into two groups: Group I (n=25) received a single-shot SAPB, and Group II (n=25) received a single-shot SAPB followed by the placement of a peripheral nerve catheter in the serratus anterior plane for continuous postoperative infusion analgesia for 24 hours. After performing the blocks and confirming sensory dermatomes, the patients were taken to the operating room for the surgical procedure. Perioperative hemodynamic parameters, remifentanil consumption and surgical duration were recorded. During the postoperative follow-up, 1, 4, 8, 12 and 24 hours Numeric Rating Scale (NRS) scores, episodes of nausea and vomiting, time to first rescue analgesia and occurrence of side effects were noted. The demographic data of the patients, ASA scores, body mass indexes, weights, anesthesia duration, surgical duration, and perioperative remifentanil consumption were similar in both groups. There was no significant difference between the two groups in terms of NRS assessment at 1 hour (p>0.05). However, at 4, 8, 12, and 24 hours, the single-shot SAPB group had higher pain scores compared to the continuous infusion SAPB group and a significant difference was found (p<0.05). Additionally, the single-shot SAPB group required rescue analgesia earlier, which was also a significant finding (p<0.05). In breast-conserving surgery patients, continuous infusion through a peripheral catheter in the serratus anterior plane was shown to be superior to single-shot SAPB in preventing acute postoperative pain. This approach can lead to reduced opioid consumption, early mobilization and discharge and decreased conversion of acute pain to chronic pain. We believe that the application of a peripheral block catheter in patients undergoing breast-conserving surgery can yield better results. | |
