Obezite cerrahisi sonrası osteoporoz sıklığının değerlendirilmesi

Özet

Ciddi obezite ve eşlik eden hastalıklar dünya çapında önemli bir halk sağlığı sorunudur. Beslenme desteği ve geleneksel tıbbi tedaviler aşırı kilo ve ilgili hastalıkları tedavi edebilir ancak morbid obezite için bu tedaviler yeterli olmayabilir. Bu durumda obezite tedavisi için bariatrik cerrahiye başvurulmaktadır. Bariatrik cerrahi çeşitli komplikasyonlara yol açmaktadır. Bu komplikasyonlardan biri de iskelet sağlığını olumsuz yönde etkilemesidir. Bu çalışma ile bariatrik cerrahi uygulanan hastalar osteoporoz açısından değerlendirilerek bariatrik cerrahinin iskelet sistemi üzerine olan etkilerinin ortaya çıkarılması amaçlanmıştır. Bu çalışmaya obezite nedeniyle bariatrik cerrahi uygulanan ve ameliyat sonrası en az 1 yılını tamamlayan 61 hasta dahil edildi. Hastaların ameliyat öncesi kilo ve boy bilgileri, kan değerleri hastane kayıtlarından alındı. Kontrollerde boy ve kilo ölçümleri yapıldı. Vücut vücut ağırlığı (kg), vücut kitle indeksi (kilo/boy(m²) olarak hesaplandı. DEXA ile değerlendirilen osteoporoz tanımı, T skoru ??2,5 ve Z skoru ??2 idi ve osteopeni tanımı, T skoru ??1 ve >?2,5 olarak kabul edildi. Cerrahi öncesi ve sonrası elde edilen tüm veriler istatistiksel olarak analiz edildi. Çalışmaya dahil edilen hastaların yaş ortalaması 38.61 yıl, %88.5'i kadındı. VKİ ortalamasının cerrahi öncesi 45.9 kg/m², cerrahi sonrası 29.38 kg/m² olduğu tespit edildi. Cerrahi sonrası D vitamini, Mg, P ve Na değerlerinin cerrahi öncesi döneme göre istatistiksel olarak anlamlı derecede artış gösterirken, PTH ve Ca değerleri istatistiksel olarak anlamlı derecede düşüş gösterdi. Ameliyattan sonraki geçen süre ile postop VKİ, preop PTH, postop PTH arasında anlamlı pozitif, Beta-CTx anlamlı negatif korelasyon gözlendi. Cerrahi sonrası birinci yılda hasta takibinin iyi yapılması ile D vitamini ve Ca eksikliklerinin önüne geçildiği ve bu sürede osteoporoz gelişmediği tespit edildi. Bariatrik cerrahi sonrası hasta takibinin önemli olduğu, kemik mineral yoğunluğunun korunması için hasta takibinin iyi yapılması gerektiği sonucuna ulaşıldı. Bariatrik cerrahinin osteoporoz oluşturma risklerinin araştırılması için daha uzun süreli takip çalışmalarına ihtiyaç vardır.

Severe obesity and its accompanying diseases are an important public health problem worldwide. Nutritional support and conventional medical treatments can treat overweight and related diseases, but these treatments may not be sufficient for morbid obesity.In this case, bariatric surgery is used for the treatment of obesity. However, bariatric surgery causes various complications. One of these complications is considered to be a negative impact on skeletal health. In this study, it was aimed to evaluate the patients who underwent bariatric surgery in terms of osteoporosis and to reveal the effects of bariatric surgery on the skeletal system. 61 patients who underwent bariatric surgery for obesity and completed at least 1 year after surgery were included in this study.Preoperative weight and height information of the patients; blood values were obtained from hospital records. Height and weight measurements were taken upon arrival. Body mass index was calculated as body weight (kg)/height (m)2. The definition of osteoporosis assessed by DEXA was T score ??2.5 and Z score ??2, and osteopenia definition was accepted as T score ??1 and >?2.5. All data obtained before and after surgery were statistically analyzed. The mean age of the patients included in the study was 38.61, and 88.5% of the patients were female. It was determined that the mean BMI of the patients before surgery was 45.9, and the mean BMI after surgery was 29.38.While vitamin D, Mg, P and Na values of the patients increased significantly after surgery compared to the preoperative period, PTH and Ca values showed a statistically significant decrease. There was a significant positive correlation and a negative Beta-CTx correlation between the time after surgery and postoperative BMI, preoperative PTH, and postoperative PTH. It was determined that vitamin D and Ca deficiencies were prevented by good follow-up in the first year after surgery and osteoporosis did not develop during this period.It was concluded that patient follow-up is important after bariatric surgery, and patient follow-up should be done well in order to preserve bone mineral density. Longer-term follow-up studies are needed to investigate the risks of bariatric surgery causing osteoporosis.
