İdiyopatik Parkinson hastalarında bazal ganglion ve talamus volümlerinin Manyetik Rezons Görüntüleme ile hesaplanarak klinik bulgularla karşılaştırılması

| dc.description.abstract | İdiyopatik Parkinson Hastalığı (İPH) substantia nigra'nın dopaminerjik nöronlarındaki değişikliklerin neden olduğu nörodejeneratif bir hastalıktır. Motor ve nonmotor semptomların oluşmasında bazal ganglion ve talamusların rolü tartışılagelmiştir. Bazal ganglion ve talamus, Manyetik Rezonans Görüntüleme (MRG) ile değerlendirilip volBrain gibi tam otomatik, çevrimiçi yöntemlerle segmente edilerek volümetrik hesaplamaları yapılabilir. İlk amacımız İPH hastalarında motor ve non-motor semptomlara göre bazal ganglion ve talamus volümlerini değerlendirmekti. Bir diğer amacımız ise subkortikal gri madde anatomik yapılarındaki atrofi ile klinik değerlendirme ölçekleri arasındaki korelasyon varlığını değerlendirmekti. Hareket bozukluğu konusunda deneyimli nörolog tarafından değerlendirilmiş ve İPH tanısı almış 20'si kadın, 40'ı erkek 60 hasta ve 26'sı kadın, 25'i erkek 51 sağlıklı kontrol grubu çalışmaya dahil edilmiştir. Çalışmaya katılan İPH hastaları demansı olanlar ve normal kognitif durumda olanlar olarak gruplandırılmıştır. Ayrıca ilk klinik semptomu tremor olanlar ve bradikinezi olanlar olarak gruplandırılmıştır. Bazal ganglionların ve talamusun 3D T1A Spoiled Gradient Recalled Eko (SPGR) sekans MRG görüntülerinden faydalanarak vol2Brain aplikasyonu ile otomatik segmentasyon yapılmış ve volümleri hesaplanmıştır. İPH hastaları ile kontrol grubunun bazal ganglion ve talamus volümleri karşılaştırıldığında İPH hastalarında sağ putamen (p=0,001; p<0,01), sol putamen (p=0,017; p<0,05), total putamen (p=0,017; p<0,05) volüm ölçümleri istatistiksel olarak anlamlı farklılık göstermekte olup İPH hastalarında daha düşüktür. Demanslı İPH hasta grubu ile normal kognitif durumdaki İPH hastaları kıyaslandığında ise sağ talamus, (p=0,001; p<0,01), sol talamus (p=0,001; p<0,01), total talamus (p=0,001; p<0,01) volüm ölçümleri istatistiksel olarak anlamlı farklılık göstermekt. Demans bulgusu olanlarda talamus volümü daha düşük bulunmuştur. İPH'li hasta grubunun Birleşik Parkinson Hastalığı Dereceleme Ölçeği (UPDRS) skorlarına göre bazal ganglion ve talamus volümleri karşılaştırıldığında UPDRS-I skorları ile sağ, sol ve total talamus volüm ölçümleri arasında negatif yönlü istatistiksel olarak anlamlı bir ilişki bulunmuştur (p<0,05). UPDRS-III skorları ile sağ, sol ve total putamen volüm ölçümleri arasında negatif yönlü istatistiksel olarak anlamlı bir ilişki bulunmuştur (p<0,05). UPDRS-Total skorları ile sağ, sol ve total putamen volüm ölçümleri arasında negatif yönlü istatistiksel olarak anlamlı bir ilişki bulunmuştur (p<0,05). UPDRSTotal skorları ile sol ve total talamus volümleri arasında negatif yönlü anlamlı bir ilişki bulunmuştur (p<0,05). İPH hastalığının motor ve non-motor semptomları arasındaki ilişkiyi daha iyi anlamak ve klinik yönetimde daha etkili stratejiler geliştirmek adına yapılan bu çalışma, putamen ve talamus atrofisinin klinik seyir ve prognoz tahmininde potansiyel bir MRG belirteci olarak kullanılmasının önemine işaret etmektedir. | |
| dc.description.abstract | Idiopathic Parkinson's Disease (IPD) is a neurodegenerative condition caused by changes in the dopaminergic neurons of the substantia nigra. The role of the basal ganglia and thalamus in the development of motor and non-motor symptoms has been extensively discussed. These regions can be evaluated using Magnetic Resonance Imaging (MRI) and subjected to volumetric calculations through fully automated, online methods such as volBrain. The goal was to assess basal ganglion and thalamus volumes in IPD patients based on motor and non-motor symptoms. Another objective was to evaluate the correlation between atrophy in subcortical gray matter anatomical structures and clinical assessment scales. A total of 60 patients, 20 females and 40 males, diagnosed with IPD by a neurologist experienced in movement disorders, and 51 healthy control subjects, including 26 females and 25 males, were included in the study. IPD patients participating in the study were categorized based on the presence or absence of dementia, as well as the first clinical symptom being tremor or bradykinesia. Automatic segmentation of the basal ganglia and thalamus was performed using 3D T1A Spoiled Gradient Recalled Echo (SPGR) sequence MRI images with the vol2Brain application, and their volumes were calculated. When comparing the basal ganglia and thalamus volumes between IPD patients and the control group, statistically significant differences were observed in the volumes of the right putamen (p=0.001; p<0.01), left putamen (p=0.017; p<0.05), and total putamen (p=0.017; p<0.05) in IPD patients, indicating lower volumes in IPD patients. When comparing IPD patients with dementia to those with normal cognitive status, statistically significant differences were found in the volumes of the right thalamus (p=0.001; p<0.01), left thalamus (p=0.001; p<0.01), and total thalamus (p=0.001; p<0.01). The thalamus volume was lower in those with dementia. When comparing basal ganglia and thalamus volumes with the Unified Parkinson's Disease Rating Scale (UPDRS) scores in the IPD patient group, a statistically significant negative correlation was found between UPDRS-I scores and volumes of the right, left, and total thalamus (p<0.05). Similarly, UPDRS-III scores showed a statistically significant negative correlation with volumes of the right, left, and total putamen (p<0.05). Negative correlations were also observed between UPDRS-Total scores and volumes of the right, left, and total putamen (p<0.05), as well as between UPDRS-Total scores and volumes of the left and total thalamus (p<0.05). This study, conducted to better understand the relationship between the motor and non-motor symptoms of IPD and to develop more effective strategies in clinical management, highlights the potential use of putamen and thalamus atrophy as an MRI marker for predicting the clinical course and prognosis of the disease. | |
