Acil serviste senkop ile başvuran 65 yaş ve üzeri hastalarda oesil sanfrancisco ve rose skorlarının mortalite ve morbidite açısından karşılaştırılması

Abstract

AMAÇ: Acil servise bilinç kaybı şikayeti ile başvuran senkop tanısı almış 65 yaş ve üzerindeki hastalarda; ROSE, OESIL, San Francisco risk skorlarının morbidite ve mortalite açısından karşılaştırılması ve bu senkop skorlarının geriatrik popülasyon için uygunluğunun ve kullanılabilirliğinin değerlendirilmesi amaçlanmıştır. YÖNTEM: Prospektif, gözlemsel ve tek merkezli bir çalışmadır.15 Ekim 2018 - 15 Ekim 2019 tarihleri arasında acil servise bilinç kaybı şikâyeti ile başvuran 866 hastadan 65 yaş ve üzerinde olan ve gerekli kıstasları sağlayan 101 erkek, 111 kadın olmak üzere toplam 212 hasta çalışmaya dâhil edilmiştir. Çalışmaya dâhil edilmiş hastalar veya yakınları ile 7. gün ve 30. günde telefon ile iletişime geçilerek hastalar takibe alınmıştır. Mortalite ve/veya belirlediğimiz morbidite kriterlerinden birinin gelişip gelişmediği sorgulanmıştır. BULGULAR: SFSR skorlaması mortalite ve morbiditeyi öngörme açısından sensitivite ve spesivitesi ROSE ve OESIL skorlamalarından daha yüksektir. ROSE ve OESIL skorlamaları birbirlerine yakın olmakla beraber, ROSE skorlamasının 1. haftada mortalite ve morbidite açısından OESIL skorlamasından daha yüksek sensitivite ve spesifiteye sahip olduğu görülmüştür. SONUÇ: Yaşlı hasta grubu için standart bir skorlama sistemi yoktur. Mevcut skorlama sistemleri her ne kadar kullanılabilir olsa da, geriatrik hastalarda gözlenen anatomik ve fizyolojik değişiklikler sebebiyle bu hasta gurubu için spesifik bir skorlama sistemine ve yeni çalışmalar yapılmasına ihtiyaç vardır.

OBJECTIVE: It aimed to compare the risk scores of ROSE, OESIL, San Francisco in terms of morbidity and mortality in patients aged 65 and over who admitted to the emergency department with a loss of consciousness and diagnosed with syncope to evaluate the usability of these syncope scores for the geriatric population (age 65 and over). METHOD: It is a prospective, observational and single-center study. A total of 212 patients, aged 65 and over, 101 men and 111 women, who applied to the emergency department with the complaint of loss of consiousness between 15 October 2018 and 15 October 2019, were included in the study. Patient follow-up was performed by contacting by the phone with the patients or relatives included in the study, on the 7th and 30th days. It was investigated whether one of the criteria of mortality and / or the morbidity we determined was developed. RESULTS: SFSR scoring is higher than ROSE and OESIL scores in terms of sensitivity and specificity while predicting mortality and morbidity. Although ROSE and OESIL scores were comparable to each other, it was observed that ROSE scoring had higher sensitivity and specificity in terms of mortality and morbidity at week 1 than OESIL score. CONCLUSION: As far as we know; there is no standardized scoring system for the elderly patient group. Although existing scoring systems can be used due to the anatomical and physiological changes observed in geriatric patients, a specific scoring system and new studies are needed for this group of patients.
