Down sendromlu çocuklarda aurikular antropometrik büyüme çalışması

Abstract

Amaç: Down sendromu toplum içinde sıklıkla görülebilen bir genetik bozukluktur. Bu hastalar birçok anatomik malformasyon ve ek hastalıklara sahiptir. Sıklıkla tıbbi müdahale gerekliliği olabilen bu hastaların özellikli yaklaşıma gereksinimi vardır. Kulak patolojilerinin görülme sıklığı da normal bireylere göre fazladır. Biz bu çalışmamızda Down sendromlu hastaların aurikula boyutlarının ölçülmesi ve kulak gelişimlerinin değerlendirilmesini amaçladık. Yöntem: Yenidoğan döneminden 18 yaşa kadar her yaştan ve 18 yaş ve üstü erişkin grubu olmak üzere toplam 19 grup oluşturuldu. Her grupta 20, toplamda 380 hasta çalışmaya dahil edildi. Tüm hastalar ve/veya yakınları bilgilendirildikten sonra sağ kulaklarından altı düzlemde ölçümler yapıldı. Bu ölçümler supraheliksten infralobule, heliksten tragusa, antiheliksten tragusa, konka yapışma noktasından tragusa (konkal derinlik), supraheliksten mastoide ve tragus seviyesinde heliksten mastoide olarak planlandı. Ölçümler alındıktan sonra tüm gruplar kadın erkek olarak ayrıldı ve istatistiksel analizler yapıldı. Bulgular: Supraheliksten infralobule aurikula yüksekliğinin erkeklerde ve kızlarda 13 yaşta erişkin yaşa ulaştığı gözlendi. Heliksten tragusa aurikula genişliği erkek ve kızlarda 5 yaşta erişkin yaşa ulaşırken antiheliksten tragusa olan uzaklık erkeklerde 1 yaşta erişkin değere ulaştığı, kızlarda ise hiçbir yaş grubunda erişkin değerle istatistiksel anlamlı farklılık vermediği izlendi. Konkal derinliğin kız ve erkeklerde 8 yaşta erişkin değere ulaştığı görüldü. Supraheliksten mastoide olan uzaklık erkek ve kızlarda 7 yaşta erişkin değere ulaşırken tragal düzeyde heliksten mastoide uzaklığın erkeklerde 4, kızlarda 5 yaşta erişkin değere ulaştığı görüldü. Erişkin kulak boyutları erkekler ve kadınlar için sırasıyla 59,83 mm ve 56,25 mm olarak bulundu Sonuç: Down sendromlu bireylerin kulakları literatürde belirtilen erişkin kulaklarına göre daha küçük izlenmiş olup gelişme yaşı da daha geç izlenmektedir. Bu verilerin, Down sendromlu hastaların kulak cerrahisi kararı ve zamanlaması, işitme cihazı gibi endüstriyel üretim konularında önemli olabileceği düşünülmektedir. Aurikula boyutları etnik kökene göre çeşitlilik gösterebildiği için çeşitli toplumlardaki Down sendromlularda kulak boyut ölçümlerinin yapılması literatüre bu verilerin sağlanması açısından önemlidir.

Objectives: Down syndrome is a genetic disorder that can be seen frequently in the community. These patients have many anatomical malformations and comorbidities. They often require medical intervention and need a specialized approach. In this study, we aimed to measure the auricle dimensions and evaluate the ear development of Down syndrome patients, who can have various diseases related to the ear Methods: A total of 19 groups were formed, consisting of all ages from the neonatal period to the age of 18 and the adult group that consist 18 years and over. 20 patients in each group, a total of 380 patients participated in the study. After all patients and / or their relatives were informed, measurements were taken from their right ears in six dimensions. These measurements were planned as suprahelix to infralobule, helix to tragus, antihelix to tragus, concha attachment point to tragus (conchal depth), suprahelix to mastoid and helix to mastoid at tragus level. After the measurements were taken, all groups were divided into girls and boys and statistical analyzes were performed. Results: It was observed that the height of the infralobule auricle from suprahelix reached adult age at 13 years of age in boys and girls. The width of the auricle from the helix to the tragus reached adult age at the age of 5 in boys and girls. The distance between antihelix to tragus reached the adult value at 1 year of age in boys and did not differ significantly from the adult value in any age group in girls. It was observed that the conchal depth reached the adult value at the age of 8 in both genders . The distance from the suprahelix to the mastoid reached the adult value at the age of 7 in boys and girls and distance from the helix to mastoid at the tragal level reached the adult value at the age of 4 for boys and 5 for girls. Adult ear sizes were 59.83 mm and 56.25 mm for men and women, respectively. Conclusion: The ears of individuals with Down syndrome are smaller than those of adults mentioned in the literature, and the age of development is observed lately. These data are thought to be important in terms of decision and timing of auricular surgery in patients with Down syndrome, and medical instruments such as hearing aids. Since the auricula size can vary according to ethnic origin, gathering the data from different societies is valuable for the literature.
