Acil servis yeşil alan başvurusu bulunan hastaların organizasyondan ve hekimden beklentilerinin değerlendirilmesi

Özet

AMAÇ: Bu çalışmada acil servis yeşil alan başvurusu bulunan hastaların acil servis memnuniyet ve beklentilerini araştırmak ve bu beklentilerini organizasyon ile ilişkilendirmek amaçlanmıştır. YÖNTEM: Bu çalışmaya İstanbul Medeniyet Üniversitesi Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesi Tıp Fakültesi Acil Tıp Kliniği'nde, yeşil alan başvurusu bulunan 250 hasta dahil edildi. Hastaların yaş, cinsiyet, eğitim düzeyi, semptomları, eşlik eden hastalıkları, acil serviste konulan tanısı, tetkik, tedavi, yatış taburcu, sevk bilgileri, konsültasyon, görüntüleme, laboratuar tetkik istem ve sonlandırılma süre verileri hastanenin kullanmış olduğu nucleus sistemi üzerinden ulaşılarak kaydedildi. Literatür taramalarından oluşturulan hasta memnuniyet anketi ile prospektif tek merkezli bir çalışma yapıldı. Hasta memnuniyet anket formu taburcu, yatış ve sevk işlemi planlandıktan sonra uygulandı. Çalışmaya dahil etme kriterlerine uygun olan hastalara anket, yüz yüze anket uygulama yöntemi ile yapıldı. İstatistiksel analizler için NCSS (Number Cruncher Statistical System) 2007 (Kaysville, Utah, USA) programı kullanıldı. BULGULAR: Çalışma %55,2 (n=138) kadın, %44,8 (n=112) erkek, yaş ortalaması yaşları 45.48±17.57 yıl olan 250 hastanın katılımı ile gerçekleşti. Hastaların %52.8'inden laboratuar tetkik, %36.4'ünden Grafi, %6.8'inden BT ve %5.2'sinde USG görüntüleme istemi yapıldı. Grafi, laboratuvar ve konsültasyon istenen hastaların memnun olma durumu istatistiksel olarak anlamlı bulundu. Konsültasyon istenen hastaların bekleme süresi en fazla (241.60±147.18 dk) genel cerrahi kliniği, en az ise (30 dk) ortopedi ve beyin cerrahi kliniklerinin olduğu saptandı. Hastaların acil servisten %90.4, acil servis doktorlarından %92.8, acil servis hemşirelerinden %88 genel memnuniyet bildirdikleri saptandı. Mahremiyete özen gösterilmesi, muayene alanının fiziki koşulları ve muayene eden hekimin göstermiş olduğu ilgi saygı ve güler yüzlü olma durumu hastaların en çok memnun oldukları durumlar olarak bulundu. Laboratuvar tetkiklerinin yapılması ve sonuçlanması için geçen süre, acil servis içinde bulunan tuvalet ve lavaboların yeterliliği ve temizliği, konsültasyon için gelen diğer bölüm doktorlarının göstermiş olduğu ilgi saygı ve güler yüzlü olma durumu hastaların en çok memnuniyetsizliklerini bildirdiği durumlar olarak saptandı. Hastaların en çok memnuniyet bildirdikleri meslek grubu acil hekimi, en az memnuniyet bildirdikleri meslek grubu ise konsültasyon hekimi olduğu saptantı. Hekimler ise muayene için hastaya ayrılan süreden memnun kalmadıklarını bildirdi. SONUÇ: Bireylerin kişisel özelliklerinden çok kurumsal özelliklerin ve hizmet verenlere ilişkin özelliklerin memnuniyet ile ilişkilendirilmesi, acil servis organizasyon kalitesinin geliştirilmesi ve kaliteli hizmet verilmesine olanak sağlamaktadır. Daha çok İletişim ve hijyen ile ilgili bildirilen memnuniyetsizliklerin giderilmesi için; değişen hasta sayılarına göre personel sayıları güncellenmeli, denetleme sistemleri oluşturulmalı, iletişim için hizmet içi eğitimler planlanmalıdır. Görüntüleme ve konsültasyon bekleme sürelerinin azaltılmasına yönelik çalışmalar yapılmalıdır.

AIM: In this study, it was aimed to investigate the emergency service satisfaction and expectations of patients who applied to the emergency service green area and to associate these expectations with the organization. METHOD: Istanbul Medeniyet University Göztepe Prof. Dr. 250 patients who applied for green field in Süleyman Yalçın City Hospital Medical Faculty Emergency Medicine Clinic were included. The age, gender, education level, symptoms, comorbidities, diagnosis made in the emergency department, drugs used, examination, treatment, hospitalization, discharge, referral information, consultation, imaging, laboratory test request and termination time data of the patients were accessed and recorded through the nucleus system used by the hospital. . A prospective single-center study was conducted with a patient satisfaction questionnaire created from literature reviews. The patient satisfaction questionnaire was applied after the discharge, hospitalization and referral procedures were planned. The patients who met the inclusion criteria in the study were surveyed using the face-toface survey method. NCSS (Number Cruncher Statistical System) 2007 (Kaysville, Utah, USA) program was used for statistical analysis. RESULTS: The study was conducted with the participation of 250 patients, 55.2% (n=138) female, 44.8% (n=112) male, with a mean age of 45.48±17.57 years. Laboratory examination was requested in 52.8% of the patients, Xray in 36.4%, CT in 6.8% and USG imaging in 5.2%. The satisfaction status of the patients who were asked for X-ray, laboratory and consultation was found to be statistically significant. It was determined that the waiting time of the patients for whom consultation was requested was the general surgery clinic (241.60±147.18 minutes) the most, and the orthopedics and neurosurgery clinics the least (30 minutes). It was determined that the patients reported general satisfaction with 90.4% from the emergency room, 92.8% from the emergency room doctors, and 88% from the emergency room nurses. Paying attention to privacy, physical conditions of the examination area and the interest, respect and smiling face of the examining physician were found to be the most satisfactory conditions of the patients. The time taken for laboratory examinations to be performed and concluded, the adequacy and cleanliness of the toilets and washbasins in the emergency room, the attention, respect and friendliness shown by the doctors of the other departments who came for consultation were determined as the situations in which the patients reported the most dissatisfaction. It was determined that the occupational group with which the patients reported the most satisfaction was the emergency physician, and the occupational group with which they reported the least satisfaction was the consultation physician. Physicians, on the other hand, reported that they were not satisfied with the time allocated to the patient for the examination. CONCLUSION: Associating the institutional characteristics and the characteristics of the service providers with satisfaction rather than the personal characteristics of the individuals enables the improvement of the quality of the emergency service organization and the provision of quality service. In order to eliminate the dissatisfaction reported more about communication and hygiene; The number of personnel should be updated according to the changing number of patients, monitoring systems should be established, and in-service trainings should be planned for communication. Studies should be carried out to reduce waiting times for imaging and consultation.
