Covid-19 pandemisi süresince hipertansiyon hastalarının kan basıncı kontrolünün ve anti-hipertansif kullanımının değerlendirilmesi

dc.description.abstractAMAÇ: RAAS inhibitör kullanımı ile COVID-19 riski arasındaki ilişki açısından farklı görüşler ileri sürülmektedir. COVID-19 riski yüksek olan hipertansiyon hastalarında pandemi sürecinde kan basıncı değişikliğinin gözlenip gözlenmediği, hekim başvurusu olup olmadığı ve başvurusu varsa antihipertansif tedavisinde bahsedilen riske bağlı hekim tarafından değişiklik yapılıp yapılmadığının ve yapıldıysa kan basıncındaki değişikliğin buna bağlı olup olmadığının değerlendirilmesidir. GEREÇ VE YÖNTEMLER: Kesitsel nitelikteki araştırmaya 01/03/2020 öncesinde hastanemiz İç Hastalıkları Ana Bilim Dalı Polikliniği'ne başvuran 500 hasta dahil edildi. Araştırmaya dahil edilen hastalara telefon ile ulaşılarak anket formu dolduruldu. Görüşmede kişilerin yaş, hipertansiyon senesi ve hastaların kullandıkları ilaçlar, pandemi başlangıcı öncesi ve sonrası kan basıncı değerleri, bu süre içinde hekim başvurularının olup olmadığı ile antihipertansif ilaç kullanımında değişiklik yapılıp yapılmadığına dair veriler kaydedildi. BULGULAR: Çalışmaya katılan olguların %58,8'i (294) kadındı. Yaş ortalaması 63,16 ±10,87, HT (hipertansiyon) senesi ortalaması ise 7,87±3,58 sene saptandı. Pandemi süresince hastaların %20'sinin (n=100) hekim başvurusu varken, sadece 1 kişinin antihipertansif tedavisinde değişiklik yapıldığı ve mevcut tedaviye tiyazid diüretik ve spironolakton eklenmiş olduğu görüldü. Pandemi başlangıcı öncesi sistolik kan basıncı (SKB) ortalaması 131,38±9,37mmHg, diyastolik kan basıncı (DKB) ortalaması 75,3±6,07mmHg olarak saptandı. Pandemi başlangıcı sonrası SKB ortalaması 130,58±9,04mmHg, DKB ortalaması ise 75,22±6,05mmHg olarak bulundu. Çalışmaya katılan kişilerin, %13,8'inde (n=69) SKB değerinde azalma, %5'inde (n=25) artış, %81,2'sinde (n=406) değişiklik yaşanmadığı gözlendi. Öncesi ve sonrası diyastolik kan basıncı değerlendirildiğinde %16,6'sında (n=83) DKB değerinde azalma, %15,2'sinde (n=76) artış ve %68,1'inde (n=340) değişiklik olmadığı görüldü. Pandemi öncesi kan basıncı regüle olan kişi sayısı 329 (%65,8) iken pandemi başlangıcı sonrası kan basıncı regülasyonu olan kişi sayısı 352 (%70,4) olarak saptandı. SKB ölçümlerinde pandemi başlangıcı öncesi ve sonrası dönemler kıyaslandığında, önceki SKB değerinin istatistiksel olarak anlamlı yüksek saptandı (p=0,001;p<0,01). Dönemler arası DKB ölçümleri arasında ise istatistiksel olarak anlamlı farklılık saptanmadı (p=0,853;p>0,05). SONUÇ: Çalışmamızda pandemi süresince antihipertansif ilaç kullanan hipertansif hastalarda, pandemi başlangıcı öncesi ve pandemi başlangıcı sonrası hastaların kendi takiplerinde her iki sistolik kan basıncı değeri de hedef kan basıncı seviyesinde olduğu görülse de sistolik kan basıncındaki minimal düşüş istatistiksel olarak anlamlı kabul edilmiştir (p=0,001;p<0,01). Diyastolik kan basıncında ise istatistiksel anlamlı fark gözlenmemiştir.
dc.description.abstractOBJECTIVE: There are different views on the relationship between RAAS inhibitor use and the risk of COVID-19. The purpose of this study is to evaluate whether a change in blood pressure was observed during the pandemic process in hypertensive patients who have a high risk of COVID-19, and whether there is a physician's application, and if there is an application, whether the physician has made a change in the antihypertensive treatment, due to the risk mentioned, and if so, whether the change in blood pressure is related to it. MATERIAL AND METHOD: 500 patients who applied to our hospital's Internal Diseases Department Polyclinic before 01/03/2020 were included In this cross-sectional study. The patients included in the study were contacted by phone. In the interview, the age, hypertension year and the drugs used by the patients, blood pressure values before and after the beginning of the pandemic, whether there were physician applications during this period, and whether there was a change in antihypertensive treatment were asked, and the data were recorded. RESULTS: 58.8% (294) of the subjects participating in the study were women. The mean age was 63.16 ± 10.87 years, and the average year of HT (hypertension) was 7.87 ± 3.58 years. During the pandemic, 20% (n = 100) of the patients had a visit with a physician, while only 1 person had a change in their antihypertensive treatment. Before the onset of the pandemic, the mean systolic blood pressure (SBP) was 131.38 ± 9.37mmHg and the mean diastolic blood pressure (DBP) was 75.3 ± 6.07mmHg. After the onset of the pandemic, the average SBP was 130.58 ± 9.04mmHg and the mean DBP was 75.22 ± 6.05mmHg. It was observed that while 13.8% (n= 69) of the patients participating in the study experienced a decrease in their SBP value, 5% (n = 25) had an increase in their SBP, 81.2% (n = 406) had no change in their SBP. When the pre and post diastolic blood pressure was evaluated, it was seen that 16.6% (n = 83) patients had a decrease in DBP, while 15.2% (n = 76) of them had an increase and 68.1% (n = 340) had no change. While the number of people whose blood pressure was regulated before the onset of pandemic was 329 (65.8%), the number of people with blood pressure regulation after the onset of pandemic was 352 (70.4%). When the before and after the onset of pandemic blood pressures were compared in SBP measurements, the previous SBP value was found to be statistically significantly higher (p = 0.001; p <0.01). There was no statistically significant difference in DBP measurements between the periods (p = 0.853; p> 0.05). CONCLUSION: Both before and after the onset of pandemic, systolic blood pressure values were seen at the target blood pressure level. Minimal decrease in systolic blood pressure was considered statistically significant (p = 0.001; p < 0.01). There was no statistically significant difference in diastolic blood pressure.
