Anafilaksi hastalarında, ebeveynlerinde ve eğitim kurumu sorumlularında adrenalin otoenjektör kullanım beceri ve tereddütlerinin değerlendirilmesi

Özet

Anafilaksi tüm korunma yöntemlerine rağmen tekrarlayabilir. Bu nedenle her hastanın adrenalin oto-enjektöre (AOE) sahip olması gerekmektedir. Bu çalışmada hastanemiz Çocuk Alerji polikliniğinde anafilaksi tanısıyla takip edilmekte olan hastaların, ebeveynlerinin ve gidiyorsa okullarındaki sorumlunun AOE kullanımı tutum ve bilgi düzeylerini belirlemek, doğru AOE kullanım oranı ve sıklığını tespit etmek, AOE kullanılamamışsa nedenlerini öğrenmek, pratik uygulama ile hasta ve eğitmenlerine eğitim vererek AOE kullanım becerilerini geliştirmek amaçlanmıştır. Çalışma Mayıs 2022 ve Eylül 2022 tarihleri arasında gerçekleştirilmiş, gözlemsel bir çalışmadır. Anafilaksi tanısı ile AOE reçete edilmiş, 12 yaş ve üzerinde olanların kendilerine, tüm hastaların ebeveynlerine ve okula giden hastaların kreş/okul sorumlularına yüz yüze anket uygulanmıştır. Çalışmamıza toplam 125 anafilaksi tanısı almış hasta dahil edildi, bunların 88 herhangi bir eğitim kurumuna gidiyordu ve 24'ü 12 yaş üzeri hastalardı. Ebeveynlerin %71 kadın, öğretmenlerin %62'si kadındı; eğitim kurumlarının %63'ü devlet okulu, %37'si özel okuldu. Anafilaksi geçirip AOE reçetelenmiş hastaların ilk epizotlarındaki tetikleyicilerinin, 101'i (%81) besin, 9'u (%7) ilaç, 5'i (%4) arı, 2'si (%1,6) soğuk, 1'i (%0,8) egzersiz, 1'i (%0,8) lateks ilişkili saptanırken, 6'sı (%5) idiyopatik anafilaksiye sahipti. Hastaların %42'sinde takip sırasında anafilaksi epizodunun tekrarladığı ve bu epizotların %65'inin evde, %15'inin okulda ve %7,7'sinin hastanede gerçekleştiği tespit edildi. Anafilaksi tekrarında AOE kullanım oranı %19 olarak saptandı. AOE kullanılmamasının nedenleri sırasıyla "Hastaneye yakın olduğumuz için gerek duymadım" (%34,4), "Şikayetler yeterince ağır değildi" (%16) ve "AOE yanımda değildi" (%14)'idi. Gereklilik halinde AOE kullanma konusunda ne hissettikleri sorulduğunda ebeveynlerin %50'si rahatlıkla doğru şekilde uygulayabileceğini, %11'i kesinlikle uygulayamayacağını belirtmişken; 12 yaş üstü hastaların ise %17'si rahatlıkla doğru şekilde uygulayabileceğini, %42'si kesinlikle uygulayamayacağını belirtmiştir. Öğretmenlerin %53'ü öğrencilerinin anafilaksi öyküsünden haberdarken, %94'ü AOE kullanımını bilmediklerini belirtmiştir. Kurumların %29'unda hastalara ait AOE bulunduğu saptanmıştır. Öğretmenlere AOE kullanım eğitimi verildikten sonra ihtiyaç halinde AOE kullanma hakkında ne düşündükleri sorulduğunda %37'si "Güvenle yaparım", %25'i "Hukuki sorumluluktan çekindiğim için uygulamaktan kaçınırım", %27'si "Zorlanabilirim, ancak yapmaya çalışırım", %11'i "Heyecan ve panik ile uygulayabileceğimi sanmıyorum" şeklinde cevap vermiştir. Anafilakside tekrarlama riski yüksektir ve çoğunlukla hastane dışı ortamlarda gerçekleşir ve hastaların ancak beşte birinde AOE uygulanmaktadır. Her muayenede acil durum eylem planı içeriği ve AOE kullanımı konusunda eğitimlerin tekrarlanması, hasta ve ebeveynlerin psikolojik olarak desteklenmesi ve gerektiğinde AOE kullanımı konusunda cesaretlendirilmesi son derece önemlidir. Anafilaksili çocukların yönetiminde hekim-aile-okul iş birliği sağlanması için öğretmenlerin ve/veya okul sorumlularının da eğitim sürecine katılacağı organizasyonlar geliştirilmelidir.

Anaphylaxis can recur despite all prevention methods. Therefore, every patient should have an adrenaline auto-injector (AAI). The aim of this study was to investigate the attitude and knowhow of AAI usage of the anaphylaxis patients, their parents and if applicable their school teachers, to determne the correct rate and frequency of AAI use, to uncover the reasons if AAI could not be used, to train patients and their trainers with practical application, to improve their skills. The study is an observational study carried out between May 2022 and September 2022 in Prof. Dr. Süleyman Yalçın City Hospital Pediatric Allergy outpatient clinic. A face-to-face questionnaire was administered to those with AAI prescription for anaphylaxis, aged 12 and over, parents of all patients and nursery/school supervisors of patients who went to school. A total of 125 patients diagnosed with anaphylaxis were included in the study, 88 of them were attending any educational institution and 24 were patients over 12 years of age. 71% of parents and 62% of teachers were female; 63% of educational institutions were public schools and 37% private schools. The triggers of anaphylaxis were food (81%), drugs (7%), venom (4%), cold (1.6%), latex (0.8%), exercise-related (0.8%) and some considered to be idiopathic (5.1%). It was found that 42% of the patients had recurrent anaphylaxis episodes during follow-up, and 65% of these episodes occurred at home, 15% at school, and 7.7% at the hospital. The rate of AAI use in recurrent anaphylaxis after diagnosis was found to be 19%. The reasons for not using AAI were "I did not need it because we were close to the hospital" (34.4%), "Complaints were not severe enough" (16%) and " AAI was not with me" (14%). When the subjects were questioned in regard of their AAI usage capability under necessary conditions 50% of the parents stated that they could easily apply it correctly, while 11% said that they absolutely could not. On the other hand, 17% of patients over the age of 12 stated that they could easily apply it correctly, and 42% stated that they could not. While only 53% of the teachers were aware of the anaphylaxis history of their students, 94% stated that they did not have any idea about the use of AAI. It was determined that 29% of the schools had AAI belonging to patients. After the AAI training, the teachers were asked what they thought about using AAI if needed, 37% said "I will do it confidently", 25% "I avoid it because of the legal concerns", 27% "I might struggle, but I try", 11% "I will panic and fail to do it". Anaphylaxis has a high risk of recurrence, mostly in out-of-hospital settings and only one-fifth of patients undergoing AAI. It is extremely important to repeat the training on the content of the emergency action plan and the use of AAI in each examination, to support the patients and parents psychologically, and to encourage the use of AAI when necessary. In order to ensure physician-family-school cooperation in the management of children with anaphylaxis, organizations with the participitations of teachers and/or school principals should be planned.
