Bebeklerde idrar toplamada iki farklı invaziv olmayan stimülasyon yönteminin etkinliğinin belirlenmesi: Randomize kontrollü çalışma

Bu çalışma, yaşı 1-6 ay olan bebeklerde idrar örneği toplamada mesane stimülasyonu (MS) ve subrapubik kutanöz stimülasyon (SKS) tekniğinin işlem başarısı, işlem süresi ve ağrı üzerine etkisini belirlemek amacıyla yapıldı. Çalışma, İstanbul'da bir eğitim ve araştırma hastanesinin çocuk acil kliniğinde 28 Ocak – 07 Ekim 2025 tarihleri arasında paralel gruplarda randomize kontrollü olarak yapıldı. Örneklemi, araştırmanın yapıldığı tarihler arasında araştırmaya alınma özelliklerine uyan 72 bebek oluşturdu. Bebekler bloklu randomizasyon ile iki girişim ve bir kontrol grubuna atandı. Kontrol grubuna (n=24) rutin uygulamaya göre idrar toplama torbası uygulandı. MS grubuna (n=24), 5 dakika boyunca MS manevraları uygulandı. SKS grubuna (n=24) 5 dk boyunca suprapubik bölgeye soğuk yaş masaj uygulandı. Veriler, Tanıtıcı Bilgi Formu, İzlem Formu ve FLACC Ağrı Ölçeği ile toplandı. Veriler, SPSS 25.0 paket programında ki-kare testi, tek yönlü ANOVA, Kruskal-Wallis testi ve karışık desen ANOVA ile analiz edildi. Etik onay, resmi izin ve ailelerden aydınlatılmış onam alındı ve protokol kaydı (Clinical Trial No: NCT07075094) yapıldı. İşlem başarısına göre gruplar arasında istatistiksel olarak anlamlı fark vardı (p<0.05). Grupların işlem süresi ortalamaları benzerdi (p>0.05). İşlemden hemen önce (T1) ve işlemden 5 dk sonra (T3) grupların ağrı puanları benzerdi (p>0.05); işlemin 1. dk'sında (T2) SKS grubunun ağrı puanları kontrol grubundan daha yüksekti (p<0.05). Bebeklerden idrar örneği toplamada MS ve SKS yöntemlerinin işlem süresini artırmaksızın idrar örneği toplama başarısını artırdığı saptandı. Bebeklerde üriner sistem enfeksiyonu tanı ve tedavi süresini kısaltmak için idrar örneği toplamada MS ve SKS tekniklerinin kullanılması önerilir.
This study was conducted to determine the effects of bladder stimulation (BS) and subrapubic cutaneous stimulation (SCS) on the procedural success, the duration of the procedure, and the level of pain during urine sample collection in infants aged 1-6 months. The study was conducted as a randomized controlled trial in parallel groups between 28 January and 7 October 2025 in the pediatric emergency department of a training and research hospital in Istanbul. The sample consisted of 72 infants who met the inclusion criteria. The infants were assigned to two intervention and one control group by block randomization. According to routine practice, a urine collection bag was applied to the control group (n=24). BS manoeuvres were applied to the BS group (n=24) for 5 minutes. The SCS group (n=24) received a cold wet massage to the suprapubic region for 5 minutes. Data were collected with the Introductory Information Form, Follow-up Form and FLACC Pain Scale. Data were analyzed using the chi-squared test, one-way ANOVA, the Kruskal–Wallis test, and mixed design ANOVA in SPSS 25.0 package program. Ethical approval, official permission and informed consent were obtained from the families and protocol registration (Clinical Trial No: NCT07075094) was made. There was a statistically significant difference between the groups regarding urine sample collection success rate (p<0.05). The mean procedure times were similar between the groups (p>0.05). Pain scores were similar between the groups immediately before (T1) and 5 minutes after (T3) the procedure (p>0.05). At the 1st minute of the procedure (T2), the SCS group had higher pain scores than the control group (p<0.05). The BS and SCS methods were found to increase the success of urine sample collection without extending processing time. Using the BS and SCS techniques for urine collection is recommended to reduce the time taken to diagnose and treat urinary tract infections in infants.
