Üst solunum yolu enfeksiyonu olan çocuklarda burun tıkanıklığının giderilmesinde kullanılan yöntemlerin etkinliğinin belirlenmesi

Bu araştırma çocuklarda burun tıkanıklığının giderilmesinde kullanılan farklı yöntemlerin işlem ağrısı, ağlama süresi, işlem süresi ve fizyolojik parametreler üzerine etkisini incelemek amacıyla gerçekleştirildi. Randomize kontrollü, deneysel tipteki araştırma 03 Ocak 2022-13 Temmuz 2023 tarihleri arasında İstanbulda bir eğitim ve araştırma hastanesinin çocuk acil kliniğinde gerçekleştirildi. Araştırmanın örneklemini, belirtilen kliniğe başvuran ve araştırma grubu seçim kriterlerine uyan 126 çocuk oluşturdu. Örneklem büyüklüğünün yeterliliğini belirlemek için yapılan güç analizinde çalışmanın gücünün %99 olduğu belirlendi. Katılımcılar 4 gruba atandı: (1) İzotonik salin ile nazal irrigasyon grubu (İzoNİ), (2) hipertonik salin ile nazal irrigasyon grubu (HiperNİ), (3) izotonik salin ile nazal irrigasyon+ nontravmatik nazofarengeal aspirasyon grubu (İzoNİ+NNA), (4) hipertonik salin ile nazal irrigasyon+ nontravmatik nazofarengeal aspirasyon grubu (HiperNİ+NNA). Veriler Çocuk ve Aile Tanıtıcı Bilgi Formu, Çocuk İzlem Formu ve FLACC Ağrı Ölçeği ile toplandı. Verilerin değerlendirilmesi SPSS 24.0 paket programında uygun analizler ile yapıldı. Araştırmanın yapılabilmesi için etik onay, resmi izin ve ailelerden aydınlatılmış onam alındı. İzoNİ grubu ve İzoNİ+NNA grubundaki çocukların FLACC ağrı ölçeği puan ortalamaları diğer gruplardan daha düşük bulundu (p<0.05). İzoNİ grubundaki çocukların ağlama süresinin ve işlem süresinin diğer gruplardan daha kısa olduğu saptandı (p<0.05). Nazal irrigasyon işleminden önce, işlemden hemen sonra ve 5 dk sonrasında tüm grupların SpO2, KAH/dk ve SS/dk ortalamalarının benzer olduğu belirlendi (p>0.05). Çocuklarda burun tıkanıklığının giderilmesinde izotonik salin solüsyonu ile yapılan nazal irrigasyonun daha az ağrıya neden olduğu bulundu. Çocuklarda burun tıkanıklığının giderilmesinde İzoNİ ve İzoNİ+NNA yöntemlerinin kullanılması önerilir.
The study was carried out to examine the effects of different methods used in the relief of nasal congestion in children on the procedure pain, crying duration, duration of the procedure and physiological parameters. This randomized controlled study was conducted between January 03, 2022 and July 13, 2023 in the pediatric emergency clinic of a training and research hospital in Istanbul. The population of the study consisted of 126 children who applied to the unit and met the study selection criteria. In the power analysis performed to determine the adequacy of the sample size, the power of the study was determined to be 99%. Participants were assigned to 4 groups: (1) isotonic saline nasal irrigation group (IsoNI), (2) hypertonic saline nasal irrigation group (HyperNI), (3) isotonic saline nasal irrigation+ nontraumatic nasopharyngeal aspiration group (IsoNI+NNA), (4) nasal irrigation with hypertonic saline+ nontraumatic nasopharyngeal aspiration group (HyperNI+NNA). Data were collected with the Descriptive Information Form, Follow-up Form and FLACC Pain Scale. Evaluation of the data was done with appropriate analyzes of SPSS 24.0 package program. Ethical approval, official permission and informed consent from the families were obtained to conduct the study. The FLACC pain scale mean scores of the children in the IsoNI group and the IsoNI+NNA group were found to be lower than the other groups (p<0.05). It was determined that the crying time and the procedure time of the children in the IsoNI group were shorter than the other groups (p<0.05). It was determined that SpO2, pulse rate/min and respiratory rate/min averages of all groups were similar before, immediately after and 5 minutes after the nasal irrigation procedure (p>0.05). Nasal irrigation with isotonic saline solution was found to cause less pain in the relief of nasal congestion in children. It is recommended to use IsoNI and IsoNI+NNA methods for the relief of nasal congestion in children.
